Why anticoagulant studies on sepsis fail frequently --- start with SCARLET.

The Sepsis Coagulopathy Asahi Recombinant LE Thrombomodulin (SCARLET) trial has many defects, and thus cannot be the terminator of recombinant thrombomodulin (rTM). On the contrary, it provides sufficient evidence for further research. Based on analysis focusing on the failure of SCARLET and several previous anticoagulant studies, it is most important for new studies to grasp the following two points: (1) The enrolled cases should have sufficient disease severity and a clear standard for disseminated intravascular coagulation; (2) Heparin should not be used in combination with the investigated

[Coagulation factor VII--new physiopathological and therapeutic aspects].

Factor VII (FVII) is a vitamin K dependent coagulation factor that is synthesized in the liver, where clearance of the unactivated protein also takes place. It is a glycoprotein that following activation plays an important role in initiating coagulation after complex formation with tissue factor. A revised hypothesis of blood coagulation implicating the requirement of intact extrinsic and intrinsic pathways is presented. Increased activity of factor VII is associated with atherogenesis, and FVII deficiency may cause bleeding disorders. Recombinant FVIIa (rFVIIa) may be used in the treatment of

Inherited bleeding disorders in the newborn infant.

This article reviews the neonatal manifestations, diagnosis, and management of inherited bleeding disorders in newborn infants. Also considered are the problems of carrier detection and prenatal diagnosis. Normal values for coagulation factors in fetuses and infants are included in the discussion.

Inherited bleeding disorders: disorders of platelet adhesion and aggregation.

Platelet aggregation at sites of vascular injury is essential for the formation of the primary haemostatic plug. The mechanism of platelet aggregation under conditions of physiological flow is a complex multistep process, which requires the synergistic action of several different platelet receptors. Platelet interaction with collagen at sites of damage to the vascular endothelium involves adhesion, activation, secretion of platelet granular contents and finally aggregation. Other agonists other than collagen, such as fibrinogen, vWF and soluble agonists released from activated platelets (throm

Transient Passovoy defect during a febrile illness.

The Passovoy defect is a recently characterized hemorrhagic diathesis. We describe a patient with a febrile illness, possibly from Epstein-Barr (EB) virus, who acquired this defect transiently. Prothrombin time; assays for factors VIII, IX, XI, XII; and Fletcher (prekallikrein) and Fitzgerald (high molecular weight kininogen) factors were normal. No definite circulating inhibitor could be demonstrated. The transient Passovoy defect could possibly be ascribed to the infectious process or sulfisoxazole, which the patient had received.

Partly intraoperative cell salvage in pediatric craniocerebral trauma: effects on coagulation function, allogeneic blood requirements, and clinical outcomes - a propensity score-matched retrospective cohort study.

BACKGROUND: Traumatic brain injury (TBI) in children frequently necessitates intraoperative blood transfusion owing to the hemorrhagic risk associated with hematoma evacuation. Intraoperative cell salvage (ICS) offers a strategy to reduce allogeneic blood exposure. However, the impact of partial usage of ICS on coagulation function in the pediatric neurosurgical setting remains insufficiently characterized. METHODS: This retrospective cohort study enrolled 136 children (aged 1-18 years) who underwent cerebral hematoma evacuation between February 2016 and April 2022. Patients receiving combine

Heavy Menstrual Bleeding in the Gynecology Clinic: A Call for Awareness and Standardized Screening for Underlying Bleeding Disorders.

IMPORTANCE: Heavy menstrual bleeding (HMB) is a common problem existing in nearly 30% of women worldwide. An underlying bleeding disorder has been identified as a cause of HMB in a significant proportion of women seeking gynecological care for HMB. In practice, however, there is a significant underdiagnosis of bleeding disorders in these patients, highlighting a need for improvement in awareness and detection. OBJECTIVE: To present a comprehensive review of the literature on HMB and bleeding disorders and to increase awareness of their interrelation. In addition, to propose a standardized dia

Coagulation after paediatric miniaturised versus conventional cardiopulmonary bypass: Retrospective cohort study.

BACKGROUND: Cardiopulmonary bypass (CPB) causes coagulation disorders after surgery. This study aimed to compare the coagulation parameters after congenital cardiac surgery with miniaturised CPB (MCPB) versus conventional CPB (CCPB). METHODS: We gathered information about children who underwent cardiac surgery between 1/1/2016 and 12/31/2019. Using propensity score-matched data, we compared the coagulation parameters and postoperative outcomes of the MCPB and CCPB groups. RESULTS: A total of 496 patients (327 with MCPB, 169 withCCPB) underwent congenital cardiac surgery, and 160 matched pair

[Review: hemorrhagic diathesis resulting from acute exposure to ionizing radiation].

The symptoms of the acute radiopathy are chiefly characterized by a severe blood coagulation disorder. The main results and problems of research work on this haemorrhagic diathesis are shortly reviewed.

Haemorrhagic smallpox. I. Preliminary haematological studies.

In an investigation of specific haematological defects that might account for the haemorrhagic diathesis in certain smallpox patients, 93 patients with haemorrhagic and non-haemorrhagic forms of the disease were subjected to a variety of bleeding and coagulation studies. The findings indicate that smallpox patients with no clinical evidence of haemorrhage have no significant clotting defects although many have decreased platelets and clot retraction abnormality. Patients with the late haemorrhagic form of smallpox consistently show thrombocytopenia and associated abnormalities in the bleeding

A novel prognosis-prediction model based on coagulation indicators in secondary hemophagocytic lymphohistiocytosis.

Secondary hemophagocytic lymphohistiocytosis (HLH) is a life-threatening disease. In the present retrospective study, we aimed to investigate coagulation disorders and their outcome implications in patients with secondary HLH. We evaluated clinical characteristics and the relationship between coagulation indices and prognosis in HLH patients (n = 141). The information, including clinical symptoms, laboratory indicators, and coagulation indices, was evaluated. Coagulation disorders and bleeding events occurred in 95 (67.4%) and 60 (42.6%) patients, respectively. A coagulation index analysis pri

Effects of erdosteine on hemostasis: an experimental study.

AIM: In this study, the effects of erdosteine (ED) on the platelet function and coagulation were investigated in adult rats. MATERIALS AND METHOD: Twenty-eight male Wistar albino rats were divided into four groups. The control rats in group I (n = 7) were given only 0.5 cc of normal saline daily through oral gavage. Group II (n = 7) rats were administered 3 mg/kg ED through oral gavage for 3 days; while group III (n = 7) rats were given 10 mg/kg ED through oral gavage for 3 days; and group IV (n = 7) rats were administered 30 mg/kg ED through oral gavage for 3 days. Prothrombin time (PT), act
