Revision surgery in a middle-aged patient with pertrochanteric fracture nonunion due to wedge effect caused by cephalomedullary nail: A case report.

Trochanteric femur fractures have traditionally been treated surgically with compression hip screws or cephalomedullary nails. With the increasing use of cephalomedullary nails, potential complications from this technique have surfaced. One of them is the potential for varus malreduction of trochanteric femur fractures, known as the "wedge effect", which is the distraction of fracture fragments generated during reamer and nail passage resulting in varus malalignment at the neck-shaft angle. Although trochanteric nonunion in the non-elderly is exceedingly rare, we experienced one such case afte

Transport over a flexible nail to treat a tibial defect caused by shelling injury: A case report of a 13-year-old child.

INTRODUCTION: The care of tibial traumatic fractures brought on by shelling injuries is extremely difficult, as these fractures are frequently comminuted, leading to a bone defect, extensive soft tissue damage, and an increased risk of bacterial contamination. CASE PRESENTATION: A 13-year-old male presented with trauma to the right leg following a shelling injury. He had a Gustilo IIIa open fracture with soft tissue destruction and a 7.5 cm bone loss in the distal region of the tibial shaft. Neurovascular exams were unremarkable. Primary treatment by external fixation, wound debridement, and

What Factors Are Associated With Local Metastatic Lesion Progression After Intramedullary Nail Stabilization?

BACKGROUND: Pathologic fracture of the long bones is a common complication of bone metastases. Intramedullary nail stabilization can be used prophylactically (for impending fractures) or therapeutically (for completed fractures) to preserve mobility and quality of life. However, local disease progression may occur after such treatment, and there is concern that surgical instrumentation and the intramedullary nail itself may seed tumor cells along the intramedullary tract, ultimately leading to loss of structural integrity of the construct. Identifying factors associated with local disease prog

Upper leg lengthening on an intramedullary nail: a preliminary report.

Current methods of treatment for anisomelia are based on distraction osteogenesis. This involves creating callus during gradual distraction using an external fixator. The lengthening of long bones by Ilizarov's method requires keeping the external fixator in place for a long period of time, which increases the risk of complications, such as pin - track infection, stiffness of the joint, misalignment, and fracture of the regenerate. In order to shorten the period during which the external fixator must remain in place, we have introduced a method which enables the external fixator to be removed

Functional Surgery for Malignant Subungual Tumors: A Case Series and Literature Review.

BACKGROUND AND OBJECTIVES: Amputation is the conventional treatment for malignant subungual tumors (MSUTs), namely, subungual squamous cell carcinoma (SUSCC) and subungual melanoma (SUM). Functional surgery consisting of wide local excision (WLE) of the nail unit can preserve function without modifying prognosis in such cases. We present a series of MSUTs treated with WLE of the nail unit, describe the technique, and review its indications. MATERIAL AND METHODS: Retrospective observational study of MSUTs treated with WLE of the nail unit between 2008 and 2017. The technique consisted of en bl

[Diaphyseal resection and reconstruction in cases of tumors involving the long bones of the extremities. Apropos of 23 cases].

An analysis has been made of the records of 23 cases of resection-reconstruction of the shafts of long bones for a variety of tumours of bone. Four were in the humerus, four in the forearm bones, five in the femur and ten in the tibia. The reconstructed bone was stabilised by an intramedullary nail in thirteen cases and by a plate or blade-plate in five cases. In five cases, the grafts were simply held by screws. The bone was reconstructed by an autograft in thirteen cases and by allograft in ten cases, four of which were combined with an autograft. Resections in the upper limb gave rise to fe

Fingertip Injuries: A Review and Update on Management.

Fingertip injuries are among the most common injuries seen in the emergency department. Specific considerations in management include ensuring robust soft-tissue coverage over exposed bone or tendon, maximizing functional length, preserving nail function and appearance, and minimizing chronic pain. Bony injuries are generally managed non-operatively, although select operative indications exist-particularly for jersey finger injuries, unstable fractures, or those with significant translation. Nail bed injuries can be managed with trephination for subungual hematomas or formal nail plate removal

Percutaneous method of management of simple bone cyst.

Introduction. Simple bone cyst or unicameral bone cysts are benign osteolytic lesions seen in metadiaphysis of long bones in growing children. Various treatment modalities with variable outcomes have been described in the literature. The case report illustrates the surgical technique of minimally invasive method of treatment. Case Study. A 14-year-old boy was diagnosed as active simple bone cyst proximal humerus with pathological fracture. The patient was treated by minimally invasive percutaneous curettage with titanium elastic nail (TENS) and allogenic bone grafting mixed with bone marrow un

C-Nail versus plate osteosynthesis in displaced intra-articular calcaneal fractures-a comparative retrospective study.

BACKGROUND: Locking plate osteosynthesis via an L-shaped lateral approach is the gold standard in treating displaced intra-articular calcaneal fractures. High complication rates are known for this approach. The most frequent complications are wound edge necrosis and superficial wound infections. To reduce complication rates, a locking intramedullary nail (C-Nail) was developed that can be implanted minimally invasively via a sinus tarsi approach. We compared the postoperative complication rate and the outcome of plate osteosynthesis versus C-Nail in displaced intra-articular calcaneal fracture

[Tibiotalocalcaneal arthrodesis with a retrograde, locked intramedullary nail in treatment of posttraumatic hindfoot and distal tibia deformity--preliminary report].

The authors reviewed their results of tibiotalocalcaneal arthrodesis by retrograde intramedullary nail as a salvage procedure in treatment of the most severe squeal of talar, calcanear, ankle and pilon fractures in twelve patients. Their average age was 55 years. The average follow up ranged from 8 to 25 months. Patients were assessed by AOFAS hindfoot scale and patient satisfaction. The arthrodesis was performed via lateral, transfibular approach. Solid fusion was achieved in all but one patient. In four patients final foot position was not an optimal one for hindfoot arthrodesis. 15 months a

Prophylactic intramedullary nailing in monostotic fibrous dysplasia.

Fibrous dysplasia of bone is an enigma with no known cure. Treatment currently consists of curettage and bone-grafting in an attempt to eradicate the lesion and to prevent progressive deformity. This study presents the results of prophylactic intramedullary nailing in 10 patients with monostotic fibrous dysplasia, pain increasing with movement, and scintigraphically established activity. Ten patients with monostotic fibrous dysplasia in their upper or lower extremities treated between 2001 and 2003 were included in the study. Seven patients were male and 3 were female; their mean age was 26.9

Diaphyseal Femur Fractures in Children and Adolescents-Opportunities and Limitations of the ESIN Technique.

Background: Elastic stable intramedullary nailing (ESIN) is the gold standard for non-overweight children aged 6−12 years. However, the complication rate using elastic stable intramedullary nailing is considerably high. Nevertheless, the question arises of whether the indication for elastic stable intramedullary nailing therapy can be extended and which factors must be taken into account when determining the indication. Methods: A retrospective chart review of patients <18 years admitted with diaphyseal femur fracture at a Level I Trauma Center in Germany between 2005 and 2017 was performed. I

The microanatomic basis of finger clubbing - a high-resolution magnetic resonance imaging study.

OBJECTIVE: Hypervascularization in finger clubbing is recognized, but its microanatomical basis remains unclear. This pilot descriptive study used magnetic resonance imaging (MRI) to explore this further. METHODS: High-resolution MRI acquired with contrast agent was carried out in 4 patients with finger clubbing and 4 healthy volunteers. The anatomy of the nail bed, capsular structures, and bony changes were described. RESULTS: Marked nail bed thickening and contrast enhancement was noted in all clubbed fingers, with bone edema in 3 of the 4 patients. None of the healthy subjects had similar

Nail changes in Langerhans cell histiocytosis: a possible marker of multisystem disease.

We describe a child with a 6-month history of onycholysis, subungual hyperkeratosis, and hemorrhages in most of her fingernails and toenails. Nail involvement preceded the identification of osteolytic lesions at the mastoid on a cranial computed tomography scan, which was performed because of repeated episodes of acute otitis media. Some weeks later, a small number of erythematous papules developed over the trunk and face. The diagnosis of Langerhans cell histiocytosis was made by histopathologic examination of bone, skin, and bed and matrix nail biopsies. Response to treatment with vinblastin

Pitfalls, errors, and complications in the use of locking Küntscher nails.

The addition of percutaneously inserted transfixion screws or internally deployed fins supplements the stabilization potential of the cloverleaf nail. The use of these systems increases the indications for reamed closed intramedullary nailing. Fractures with comminution or bone loss, malunions, and nonunions located outside of the isthmic region of the femur and the tibia can now be stabilized effectively with a Küntscher-type implant. These closed nailing techniques reduce blood loss, surgical stress, and complications associated with open reduction and offer the patient the advantages of ear

Evaluation and treatment of metastases to the humerus.

The humerus is the second most common long bone site of metastatic disease, and the proximal third and diaphysis are frequent sites. Purely lytic disease and cortical bone destruction increase the risk of fracture more than 50%. For most patients, external beam irradiation is effective as a means of pain control and halting bone destruction. Fractures of the head or surgical neck can be treated with standard endoprostheses, whereas extensive proximal bone destruction is treated with custom proximal humeral replacements. Impending and complete diaphyseal fractures can be treated effectively wit

Nail-extraction device mismatch: an issue in developing countries intramedullary nail removal practice.

PURPOSE: Intramedullary nail is the gold standard in the management of long bone diaphyseal fractures of tibia and femur. The jig of these nails has corresponding extraction device whose pitch for nail coupling come in various sizes. This unlike plate and screws may be difficult to predict preoperatively and may pose a problem during removal. Difficulties in removal may arise due to the proliferation of nail brands especially in developing countries. The study aims to identify the incidence of extraction device mismatch among orthopaedic surgeons in Nigeria as well as the indications and compl
