The application of a dual-lead locking screw could enhance the reduction and fixation stability of the proximal humerus fractures: a biomechanical evaluation.

INTRODUCTION: Bicortical screw fixation, which penetrates and fixes the near and far cortex of bone, has been conventionally used to achieve compressive fixation for fracture using screws. Open reduction and internal fixation using the locking plate are widely used for treating proximal humerus fractures. However, minimal contact between the bone and the locking plate can lead to an insufficient reduction. Theoretically, a dual-lead locking screw with different leads for the screw head and body could enhance the reduction and fixation stability of fragments in proximal humeral fractures withou

Internal anterior fixators for pelvic ring injuries: Do monaxial pedicle screws provide more stiffness than polyaxial pedicle screws?

OBJECTIVES: Little is known about the mechanical properties of internal anterior fixators (known as INFIX), which have been proposed as subcutaneous alternatives to traditional anterior external fixators for pelvic ring disruptions. We hypothesised that INFIX has superior biomechanical performance compared with traditional external fixators because the distance from the bar to the bone is reduced. METHODS: Using a commercially available synthetic bone model, 15 unstable pelvic ring injuries were simulated by excising the pubic bone through the bilateral superior and inferior rami anteriorly a

High-purity weight-bearing magnesium screw: Translational application in the healing of femoral neck fracture.

Magnesium (Mg)-based metals can be used as next-generation fracture internal fixation devices due to their specific properties. We used vascularized bone grafting fixed by degradable pure Mg screws and obtained satisfactory results in the treatment of osteonecrosis of the femoral head. However, the mechanical properties of these screws make them weaker than those made of traditional metals. In particular, one of the main challenges of using screws made of Mg-based metals is their application in fixation at important weight-bearing sites in the human body. Femoral neck fracture is a common clin

Dental Cement Augmentation Increases Pullout Strength With Comparable Removal Torque for Lateral Mass Screws.

STUDY DESIGN: Biomechanical/basic science. OBJECTIVE: To evaluate the efficacy of dental cement augmentation of lateral mass screws in the cervical spine. SUMMARY OF BACKGROUND DATA: Premature loosening and fixation failure are known problems of screw fixation of the spine, especially in cases of poor bone quality. Cement augmentation of screws with PMMA has limited utility in the cervical spine due to the small size of the bone and high risk of extravasation. Augmentation with dental cement may present a viable solution for posterior cervical spinal screw fixation. METHODS: Fifteen cadaver

Effect of hybrid screw placement technique on adjacent segment degeneration: a finite element analysis.

The biomechanical properties of the lumbar pedicle screw technique, cortical bone trajectory screw technique, and modified cortical bone trajectory screw technique when used individually in internal fixation systems and their effects on neighboring segments have been studied. However, there are fewer studies on the combined use of the modified cortical bone trajectory screw technique and the cortical bone trajectory screw technique. This paper focuses on analyzing the stresses in the internal fixation system when the modified and traditional cortical bone trajectory screw techniques are applie

Biomechanical analysis of locking plates for fixation of metacarpal shaft fractures: A finite element analysis.

BACKGROUND: There appears to be a paucity of knowledge about the biomechanics of locking plates for the fixation of metacarpal shaft fractures. A thorough understanding of the biomechanics of locking plates is needed to apply them correctly, optimize outcomes, and avoid complications. The purpose of this study is to investigate the biomechanics of the fixation of metacarpal fractures using locking plate-screw constructs with different numbers of screws. HYPOTHESIS: The difference in the number of screws in the locking plate influenced the biomechanical outcome of the metacarpal fracture. MET

Pullout strength of anterior spinal instrumentation: a product comparison of seven screws in calf vertebral bodies.

A lot of new implant devices for spine surgery are coming onto the market, in which vertebral screws play a fundamental role. The new screws developed for surgery of spine deformities have to be compared to established systems. A biomechanical in vitro study was designed to assess the bone-screw interface fixation strength of seven different screws used for correction of scoliosis in spine surgery. The objectives of the current study were twofold: (1) to evaluate the initial strength at the bone-screw interface of newly developed vertebral screws (Universal Spine System II) compared to establi

A biomechanical comparison of two patterns of screw insertion.

Inserting a screw into a long bone introduces a stress riser that can predispose the bone to fracture. Where multiple screws are inserted this vulnerability may be increased. It is thought that the pattern of screw insertion plays an important part in determining the susceptibility of the bone to stress fracture. In order to study the importance of pattern of screw insertion, third generation composite femora with cannulated screws inserted in two different patterns were tested to failure by a servohydraulic materials test system. The vertical pattern of screw insertion is less apt to predispo

Stability of mini-screws invading the dental roots and their impact on the paradental tissues in beagles.

OBJECTIVE: To examine the stability of mini-screws that invade a dental root by measuring the retention period/failure rate, and to illustrate their effects on paradental tissues. MATERIALS AND METHODS: Three adult male beagle dogs received 48 orthodontic mini-screws. Half of the mini-screws were implanted to invade the roots, and the rest were placed in the middle of the alveolar bone. Half of the mini-screws were loaded immediately. The retention period of the mini-screws was documented. The dogs were euthanized after 8 weeks, and tissue samples were examined histologically. RESULTS: The f

The influence of "kickstand" screws on the mechanical performance of a lateral distal femoral plate : a finite element analysis.

AIMS: Lateral locked plating of distal femoral fractures is widely reported, yet there remains a 9% to 19% incidence of mechanical failure. Obliquely directed "kickstand screws", from the metaphyseal portion of a plate toward far-sided articular subchondral bone, have been shown to improve construct stiffness. This study explores the impact of kickstand screws in a finite element analysis bone defect model, comparing plate and screw maximum stress and maximum locking screw forces either with or without the addition of kickstand screws. METHODS: A finite element analysis model of a lateral bas

The GETA technique: a novel technique protecting the dura against the distractors for early cranial expansion.

PURPOSE: Posterior cranial vault distraction profoundly enlarges intracranial volume. To perform this procedure during the first year of life, the cranium is too thin to fix the distractors. Sometimes, the screws penetrate the bone, which risks scratching the dura during distraction. To solve this problem, we developed an implant fixation technique using bioabsorbable screws and plates, and named it the GETA technique. METHODS: In cases in which the bone is thinner than the screws, a resorbable sheet is placed between the bone and the dura to protect the dura from the tips of the screws. RES

Achieving interfragmentary compression without special drilling technique or screw design.

UNLABELLED: Traditional fracture fixation teaching suggests that fully threaded screws do not provide interfragmentary compression unless placed through a glide hole. Based on this assumption, pelvic surgeons typically use fully threaded screws in the treatment of comminuted transforaminal sacral fractures to limit iatrogenic neuroforaminal stenosis. Clinical experience with fully threaded screws suggests that interfragmentary compression actually does occur. We hypothesized that the use of a fully threaded screw does not produce any interfragmentary compression and that there is no difference

Risk Factor Analysis of Facet Fusion Following Cervical Lateral Mass Screw Fixation with a Minimum 1-Year Follow-up: Assessment of Maximal Insertional Screw Torque and Incidence of Loosening.

Posterior stabilization is a common surgical procedure, which aims for rigid stabilization by facet fusion. Facet non-union has a potential risk of the screw loosening and malalignment. Although some authors have reported the influencing factors about screw loosening in the lumbar spine, there are few reports about the risk factor contributing to the facet non-union in the cervical spine. In all, 22 patients (78 facets and 122 screws) with degenerative cervical kyphosis or spondylolisthesis who underwent decompression and lateral mass screw (LMS) fixation were analyzed. Age, gender, smoking, b

Surgical Reconstruction of Nonunion after Iatrogenic Scarf Osteotomy.

We present the case of a young patient, 32 years old, with nonunion in the diaphysis of the first metatarsal after scarf osteotomy for correction of hallux valgus. After removal of the failed osteosynthesis material and preparation of the bone fragments, a calcaneal bone autograft, previously extracted from the patient, was placed in the nonunion area. The new physiological position of the first metatarsal in the three planes was checked intraoperatively, and autograft and fragment fixation was performed using a combination of a low-profile plate with six screws and two interfragmentary screws

Effect of acetabular reinforcement ring with hook for acetabular dysplasia clarified by three-dimensional finite element analysis.

The objective of this study was to biomechanically determine the effect of the severity of acetabular dysplasia, number and positions of screws and type of bone graft material used on the initial fixation strength of the acetabular reinforcement ring with hook (Ganz ring) using the finite element method. Relative micromotion increased as the severity of acetabular dysplasia increased and tended to decrease as the number of screws increased, but varied according to screw placement position. Increased strength of the bone graft material led to decreased relative micromotion. Biomechanically, the

Ex Vivo Pullout Strength of Locking and Cortical Screws in the Femur and Tibiotarsus of the Pekin Duck (Anas platyrhynchos domesticus).

The development of smaller orthopedic plates and screws has facilitated their use in avian fracture repair. Avian bone differs from mammalian bone in its biomechanical properties due to adaptations for flight, necessitating avian-specific orthopedic biomechanical studies. Screw pullout strength has not been studied in avian bone. The aims of this study were to 1) compare the axial pullout strength of cortical and locking screw designs in the tibiotarsus and femur of the Pekin duck (Anas platyrhynchos domesticus) and 2) investigate the effects of sex and screw location in bone on screw pullout

Stability analysis of stock alloplastic temporomandibular joint prostheses based on clinical cases of screw loosening.

Screw loosening is one of the common complications that affect stability after stock alloplastic temporomandibular joint (TMJ) replacement. Based on clinically observed cases of screw loosening, this study aimed to evaluate how prosthesis installation patterns influence screw loading and to provide recommendations for installation safety. A three-dimensional finite element model of unilateral total TMJ replacement was constructed to analyze Biomet standard TMJ prosthesis stability under the following situations: 1) fossa component installed at different angles (0°-20°) relative to the bone sur

Can the body slope of interference screw affect initial stability of reconstructed anterior cruciate ligament?: An in-vitro investigation.

BACKGROUND: Superior biomechanical performance of tapered interference screws, compared with non-tapered screws, with reference to the anterior cruciate ligament (ACL) reconstruction process, has been reported in the literature. However, the effect of tapered interference screw's body slope on the initial stability of ACL is poorly understood. Thus, the main goal of this study was to investigate the effect of the interference screw's body slope on the initial stability of the reconstructed ACL. METHODS: Based on the best screw-bone tunnel diameter ratios in non-tapered screws, two different t
