Is There a Relationship Between Borderline Personality Disorder and Sexual Masochism in Women?

Sexual masochism disorder is considered the most prevalent paraphilia among women. However, little is known about the etiology and clinical correlates involved in this disorder. We aimed at addressing this issue through a potentially high-risk clinical cohort. This case-control study consisted of 60 women who met DSM-IV criteria for borderline personality disorder (BPD) and 60 women with other personality disorders. For both groups, sexual masochism disorder comorbidity was assessed through the Structured Clinical Interview, Sexual Disorders Module. Several etiological, psychosexual, and perso…

Borderline personality disorder: patterns of self-harm, reported childhood trauma and clinical outcome.

SUMMARY: Consecutive admissions of 214 women with borderline personality disorder were investigated for patterns of specific forms of self-harm and reported developmental experiences. Systematic examination of clinical notes found that 75% had previously reported a history of childhood sexual abuse. These women were more likely to self-harm, and in specific ways that may reflect their past experiences. Despite this, treatment within a dialectical behaviour therapy-informed therapeutic community leads to relatively greater clinical gains than for those without a reported sexual abuse trauma his…

Prospective associations between prenatal adversities and borderline personality disorder at 11-12 years.

BACKGROUND: The aetiological pathways to borderline personality disorder (BPD) remain only partly elucidated. Retrospective research indicates that prenatal adversity may be an important early risk factor in the development of BPD. This requires corroboration with prospective longitudinal studies. METHOD: A community sample of 6050 mothers and their children (born between April 1991 and December 1992) were assessed. Maternal anxiety and depression and maternal alcohol and tobacco consumption were assessed during pregnancy (18 and 32 weeks gestation). Postnatal risks, including maladaptive par…

Oxytocin and reduction of social threat hypersensitivity in women with borderline personality disorder.

OBJECTIVE: Patients with borderline personality disorder are characterized by emotional hyperarousal with increased stress levels, anger proneness, and hostile, impulsive behaviors. They tend to ascribe anger to ambiguous facial expressions and exhibit enhanced and prolonged reactions in response to threatening social cues, associated with enhanced and prolonged amygdala responses. Because the intranasal administration of the neuropeptide oxytocin has been shown to improve facial recognition and to shift attention away from negative social information, the authors investigated whether borderli…

Depressive-type emotional response pattern in impulsive-aggressive patients with borderline personality disorder.

INTRODUCTION: Borderline personality disorder (BPD) is typically characterized by severe affective dysregulation leading to impulsive behaviors. Accordingly, preliminary data suggest the hypothesis that BPD patients could have a specific and altered pattern of subjective emotional response to stimuli. The nature of the emotional response in BPD can be compared with other affective disorders and provide further insight on the nosological proximity with other psychiatric disorders. METHODS: Subjective emotional response was investigated in 19 patients with DSM-IV BPD with no current depressive …

Effectiveness of education programs in changing clinicians' attitudes toward treating borderline personality disorder.

OBJECTIVE: This exploratory study was conducted in Australia and New Zealand and examined the impact of two types of education programs (cognitive-behavioral and psychoanalytic) on clinicians' attitude toward deliberate self-harm behaviors in borderline personality disorder and toward working with patients with this disorder. METHODS: The Attitudes Towards Deliberate Self-Harm Questionnaire was used to assess the attitudes of mental health and emergency medicine clinicians (N=65) before and after attending an education program and at the six-month follow-up. RESULTS: Compared with participan…

A voxel-based morphometric MRI study in female patients with borderline personality disorder.

Subtle prefrontal and limbic structural abnormalities have been reported in borderline personality disorder (BPD). In order to further validate the previously reported findings and to more precisely describe the nature of the structural change we performed a voxel-based morphometric (VBM) study in patients with BPD. Twenty female patients with BPD and 21 female healthy controls were investigated. High-resolution 3-D datasets were acquired and analyzed following an optimized protocol of VBM in the framework of statistical parametric mapping (SPM99). Gray matter volume loss was found in the left…

Childhood abuse as a risk factor for suicidal behavior in borderline personality disorder.

A childhood history of sexual or physical abuse is highly prevalent in borderline personality disorder (BPD) and is associated with self-destructive behavior in clinical and nonclinical samples. Viewing BPD as a "high risk" disorder, we asked if childhood abuse was a risk factor for adult suicidal behavior or if it was related to other known risk factors for suicide in BPD. A semistructured Abuse History was obtained in 61 criteria-defined BPD patients, who were characterized by structured interviews and self-reports for Axis I disorders, Suicide History, BPD severity, hopelessness, impulsivit…

Olanzapine safety and efficacy in patients with borderline personality disorder and comorbid dysthymia.

BACKGROUND: Numerous medications have been tested in patients with borderline personality disorder (BPD) and/or schizotypal personality disorder (SPD). Although many of the medications tested have been demonstrated to be useful, no clear main treatment for BPD has emerged. Despite the efficacy of some of the medicines, acceptability and side effects have proven to be barriers to the use of medication. Therefore, an open-label olanzapine trial utilizing objective ratings was performed. METHODS: Patients suffering from BPD and dysthymia were included in an 8-week, open-label study of olanzapine…

Affective and behavioral features of DSM-III borderline personality disorder. Are they valid?

This study was designed to determine if a set of seven clinical features, closely reflecting Diagnostic and Statistical Manual III criteria, were individually and collectively more common in borderlines compared to patients with schizophrenia, affective disorders, and anxiety disorders. A 7-item borderline subscale was generated from the expanded Brief Psychiatric Rating Scale and administered to 1,762 psychiatric inpatients. Results indicated that scores on the total borderline scale were significantly higher in borderline patients compared to the control groups. Borderline patients manifeste

Temperamental differences between bipolar disorder, borderline personality disorder, and attention deficit/hyperactivity disorder: some implications for their diagnostic validity.

BACKGROUND: The relationship between borderline personality disorder (BPD), bipolar disorder (BD), and attention deficit/hyperactivity disorder (ADHD) requires further elucidation. METHODS: Seventy-four adult psychiatric in- and out-patients, each of them having received one of these diagnoses on clinical assessment, were interviewed and compared in terms of diagnostic overlap, age and sex distribution, comorbid substance, anxiety and eating disorders, and affective temperament. RESULTS: Diagnostic overlap within the three disorders was 54%. Comorbidity patterns and gender ratio did not diff…

The DSM-5 Alternative Model of Personality Disorder's Coverage of Borderline Personality Concepts.

The clinical utility of the DSM-5's Alternative Model of Personality Disorders (AMPD) depends on how well it encodes diagnostic concepts recognized in clinical descriptions of PDs. The present study examined the AMPD's coverage of borderline personality features identified in the clinical literature. In three independent samples (total N = 1166), community adults and university students rated their borderline features, level of personality functioning (AMPD Criterion A), and maladaptive personality traits (AMPD Criterion B). We observed that, collectively, the AMPD dimensions accounted for 76%

Complex Post-Traumatic Stress Disorder and Borderline Personality Disorder: A Systematic Review of Diagnostic Distinction and Comorbidity.

INTRODUCTION: The recognition of Complex Post-Traumatic Stress Disorder (CPTSD) as a distinct diagnosis in ICD-11 has intensified the need to clarify its boundaries with Borderline Personality Disorder (BPD), given their symptom overlap and shared traumatic origins. Therefore, this systematic review aimed to examine whether CPTSD is distinct from BPD, assess their comorbidity and symptom overlap, identify key criteria for differential diagnosis, and explore therapeutic implications. METHOD: A systematic search was conducted in PubMed, Scopus, and Web of Science following PRISMA 2020 guideline…

Re-examining the comorbidity between borderline personality disorder and post-traumatic stress disorder: a systematic narrative review.

BACKGROUND: Borderline personality disorder (BPD) and post-traumatic stress disorder (PTSD) frequently co-occur, yet research on this comorbidity is limited. Recent advancements in this area have led to the emergence of new studies that provide insight into the relationship between these disorders, potential treatment interventions, and the ongoing debate surrounding the diagnosis of Complex PTSD. This review aims to examine these findings and evaluate whether they address key gaps identified in the review by Frías & Palma (Psychopathology 48(1):1–10, 2014). METHODS: A literature search was co…

The influence of sleep on emotion dysregulation in borderline personality disorder, generalized anxiety disorder, and healthy controls.

Emotion dysregulation is central to borderline personality disorder (BPD) and exacerbated by sleep disruptions. This study investigated whether homeostatic (i.e., sleep efficiency), circadian (i.e., chronotype), and subjective (i.e., sleep quality) sleep elements predict emotion dysregulation in BPD, healthy controls (HCs), and a generalized anxiety disorder (GAD) group. Participants (N = 120) with BPD, GAD, and HCs completed daily sleep measures for seven days prior to an experiment wherein baseline emotion, emotional reactions to stressors (i.e., reactivity), and the extent to which they can…
