Late brain abscess years after severe cerebrocranial trauma with fronto-orbitobasal fracture.

An 8-year-old boy suffered severe craniocerebral trauma with left-sided fronto-orbitobasal fracture. The CT scan showed minor subdural air i … into the bone defect, which prevented liquorrhea. Due to the lack of bone and dura barrier, a late brain abscess developed in the course of sinusitis. In such cases, primary surgical revision seems to be indicated.

Cure of brain abscess caused by Nocardia asteroides resistant to multiple antibiotics.

A 29-year-old man with pulmonary infiltrates due to pulmonary alveolar proteinosis had Nocardia asteroides pneumonia and brain abscess. The nocardial infections persisted after therapy with sulfonamides, minocycline, and cefotaxime. Based on the susceptibility studies and serum antibiotic concentrations, he was treated with minocycline, 400 to 600 mg daily, cefotaxime, 12 to 30 gm daily, and probenecid for 15 months. Proteinuria observed near the completion of therapy was transient. Three years after therapy, the infections appear to have been cured.

A model of brain abscess: septic homologous blood clot emboli in rats.

Brain abscesses in rats were produced by intra-arterial injection of septic homologous blood clot emboli. The production rate was 100% and the histopathological features closely resembled those seen in other animal models and in spontaneously occurring brain abscesses in humans. This small-animal model may be useful for systematic study of the development of brain abscesses as well as for evaluation of various therapeutic procedures.

Lactate dehydrogenase and aspartete transaminase of the cerebrospinal fluid in patients with brain tumours, congenital hydrocephalus, and brain abscess.

The diagnostic value of CSF lactate dehydrogenase and aspartate transaminase in cases of brain tumours (except for CSF AST in the benign tumours), congenital hydrocephalus, and brain abscess is established. Tumour cyst fluids show a higher enzymatic activity than does the CSF. The two enzyme estimations do not help in differentiating the supratentorial from the infratentorial tumours. CSF AST is superior to CSF LD in discriminating the malignant and benign tumours, in so far as the AST is increases selectively in malignancy. Estimates of CSF LD are slightly superior to those of CSF AST, both i…

Nocardia brain abscess mimicking brain tumour in immunocompetent patients: report of two cases and review of the literature.

Nocardia brain abscess is a rare intracranial lesion and has been reported in immunocompromised patients. An optimal treatment approach has not been established. However, early diagnosis and appropriate antimicrobial therapy are very important factors for a good outcome. We report two unusual cases of Nocardia brain abscess simulating brain tumour in immunocompetent patients. One of the cases was presumed to be a primary brain tumour and the other a metastatic brain tumour. They underwent surgical gross total resection. After Nocardia asteroides was seen on Gram's stain and subsequently identi…

Brain abscess mimicking a brain tumor only realized during surgery: A case report in a resource strained environment.

BACKGROUND: Diagnosis of brain tumors increased in sub-Saharan Africa since the advent of computed tomography (CT)-scans and magnetic resona … The follow-up was unremarkable. CONCLUSION: Brain MRI is essential to differentiate some pyogenic brain abscesses from tumors. However, meticulous history taking is important to gather as much information as possible about any medical pathology, which would then be corroborated with the physical examination findings and imaging to increase diagnostic accuracy …

[Clinical presentation of brain abscesses: from classic triad to silent forms].

UNLABELLED: Despite relatively small incidence, brain abscesses are life-threatening with high risk of death and persistent neurological deficits. A key factor limiting early diagnosis is polymorphic and nonspecific clinical presentation. OBJECTIVE: To analyze the structure and incidence of clinical symptoms in a cohort of patients with brain abscesses treated at the Burdenko Neurosurgical Center. MATERIAL AND METHODS: A single-center study was based on medical records of patients with newly diagnosed and confirmed brain abscesses hospitalized for surgical treatment at the Burdenko Neurosurg…

Nocardia brasiliensis in brain abscess of a young immunocompetent patient: A case report.

Nocardia mainly affects immunocompromised individuals. We report a case of immunocompetent male, with neurological complaints for the past eight months. Contrast-enhanced MRI (CE-MRI) revealed space-occupying lesion with frontal lobe abscess. The patient underwent craniotomy, and pus was sent for microbiological diagnosis. Direct KOH-calcofluor white mount, and modified Ziehl-Neelsen (ZN) staining demonstrated thin, branching filamentous structures suggestive of Nocardia spp. Nocardia brasiliensis was confirmed through 16S rRNA sequencing. The patient received targeted antimicrobial therapy. D

Fusobacterium brain abscess as a complication of sinusitis in an immunocompetent adolescent: A case report.

BACKGROUND: Brain abscesses are uncommon intracranial infections with an estimated incidence of 0.5 per 100000 individuals. Among bacterial causes, Fusobacterium species account for less than 10% of cases, and fewer than 20 pediatric instances have been reported worldwide. These infections are predominantly seen in individuals with poor dental hygiene, sinusitis and pose diagnostic challenges due to their anaerobic nature and frequent culture negativity. However, cases in immunocompetent individuals are rare and often pose diagnostic challenges due to the anaerobic nature of the causative orga…

Nocardia farcinica brain abscess in a patient with Evans syndrome: challenges in management.

Cerebral nocardiosis is a rare but serious infection, often seen in immunocompromised individuals. This report discusses a case of cerebral nocardiosis in a woman with diabetes and Evans syndrome on immunosuppressive therapy. She presented with neurological symptoms, including seizures and hemiparesis, and brain imaging revealed abscesses. Surgical excision was done and culture confirmed Nocardia farcinica, showing resistance to co-trimoxazole. Treatment included a combination of intravenous and oral antibiotics, tailored based on susceptibility. The report highlights the need for prophylaxis

Paediatric brain abscesses and odontogenic infection: a critical analysis of the existing literature.

PURPOSE: Brain abscesses in children are an uncommon, but potentially severe condition requiring rapid diagnosis and treatment. The objectives of this article are (1) to present and discuss the currently available literature on prevalence, aetiology and management of dental brain abscesses in children; (2) to report a series of five clinical cases of intracranial abscesses due to dental pathogens; and (3) to highlight the importance of dental care in temporary and permanent dentition to prevent brain abscesses. METHODS: We report a series of five cases of brain abscesses secondary to dental i…

The characteristics of the cases with brain abscess and the analysis of the predictive factors for poor outcome.

INTRODUCTION: To evaluate the characteristics of patients who have undergone surgical operations due to brain abscess and to assess the risk factors for mortality and the outcomes. METHODOLOGY: Patients who have undergone surgical operations due to brain abscess between January 2014 and January 2024 in our hospital were evaluated retrospectively. Patients were divided into 2 groups to determine poor outcome predictive factors. RESULTS: A total of 57 patients with brain abscess were evaluated. Brain abscess was developed after a surgical procedure in 33% of the patients. Of these patients, 44…

Staphylococcus lugdunensis Endocarditis Presenting with Brain Abscesses and Temporal Vision Deficits.

Staphylococcus lugdunensis is a coagulase-negative staphylococcal bacterium (CoNS) that colonizes the skin. While infectious endocarditis (IE) caused by S. lugdunensis is rare, it is noteworthy because it has been associated with an aggressive clinical course. In this report, we present a case of culture-negative IE complicated by brain abscesses, vision deficits, and progressive heart failure that ultimately required mitral valve replacement. The causative agent was eventually identified as S. lugdunensis through molecular testing of valvular tissue.

Brain abscess caused by Morganella morganii: A case report and review of the literature.

BACKGROUND: Morganella morganii is a Gram-negative enteric rod found in the intestinal tracts of humans, mammals, and reptiles as normal flora. It is highly implicated in urinary tract infections, wound infections, and septicemia. The cerebral nervous system, especially brain abscess attributed to M. morganii, remains extremely rare. To the best of the author's knowledge, only eight documented cerebral brain abscesses caused by M. morganii have been reported in the literature. CASE DESCRIPTION: A 48-year-old man presented with headache, fever, and irritability two months after endoscopic endo…

A case of Listeria monocytogenes brain abscess with a clinical presentation mimicking stroke.

PURPOSE: Kidney transplant recipients are at a high risk of both stroke and infection. We report this case to inform readers of the possibility of the occurrence of Listeria monocytogenes brain abscess despite treatment with low dose trimethoprim-sulfamethoxazole. CONCLUSION: Physicians should monitor unusual stroke symptoms and consider Listeria brain abscess as a differential diagnosis. This case suggests that listeria brain abscesses can potentially cause temporary neurological deficits akin to a stroke, despite trimethoprim-sulfamethoxazole prophylaxis, but these symptoms resolve without …
