Cerebral dysfunction following infantile dietary chloride deficiency.

Twenty-two children who were chloride-depleted in infancy due to a chloride-deficient diet and who had resultant hypochloremic alkalosis were analyzed in regard to their signs and symptoms, metabolic studies, and growth parameters. Deceleration of weight, linear growth, and head growth occurred in most, and persistent growth failure occurred in some. The majority had cognitive deficits at follow-up. Comparison with growth parameters in a chronically malnourished group of children who had a variety of disorders revealed a similar degree of deceleration of weight (p = 0.50) and height (p = 0.70)

Cerebellar hemorrhage in the term neonate: developmental and neurologic outcome.

To elucidate the effects of cerebellar hemorrhage on the term neonate, neurodevelopmental assessments were conducted at a mean age of 32 months on six children. In addition to cerebellar hemorrhage, ventriculomegaly was present on each subject's initial computed tomographic scan. All were managed without surgical evacuation. Two patients required shunts for progressive ventriculomegaly. Five patients had follow-up computed tomography indicating mild atrophy of the superior anterior vermis of the cerebellum; however, none had abnormal ventricular size or abnormalities of the cerebrum. On detail

Bender scores and the horse as a distinct item on object assembly of the WISC.

This exploratory study investigated the relationship between two measures of perceptual-motor problems. Ratios derived from the Object Assembly subtest of the WISC with errors greater than 1 SD on the Bender test using Koppitz' scoring system were compared. The sample of 66 white middle-class children was 6 to 11 yr. old. The particular ratio studied was statistically significantly associated with the Bender, whereas ratios of other Object Assembly items were not. The Horse ratio was also significantly associated with the presence of learning problems shown in children's files.

Adaptive behaviour of Reye's syndrome survivors.

Adaptive behaviour was investigated in a group of 35 individuals who survived Reye's syndrome. Specifically, the association between three medical variables: age at onset of disease, months since medical recovery, and stage of coma, and current functioning level was assessed. Results indicated that these variables may be viable predictors for some of the behavioural domains. Sequelae related to this disease may include a diminished level in the rate of learning as well as long term maladaptive behaviours.

Impairment of voice and face recognition in patients with hemispheric damage.

Voice and face recognition were tested in 21 left- and 9 right-hemisphere-damaged patients. Test materials were photographs and recordings of famous political and entertainment personalities. Pathological face recognition (prosopagnosia) and voice recognition (phonagnosia) were both significantly more prevalent in the right-hemisphere group. Only one instance of prosopagnosia and one of phonagnosia were observed in the left-hemisphere group, all of whom were aphasic. Of the right-hemisphere cases, there were four instances of each agnosia, with three patients showing a dual impairment. These f

Traumatic head injury: restlessness and agitation as prognosticators of physical and psychologic improvement in patients.

Eighty-seven traumatic head injury patients were followed from admission to post-discharge, spanning a period of 5 years or more. Using objective criteria, post-traumatic activity level (PTAL) was evaluated on admission and compared with cognitive and overall status at discharge. Further comparisons were made with follow-up data. Twelve patients were agitated., 32 restless, 17 appropriately active, 16 sluggish, and 10 immobile. Eighty-three percent of the agitated, 84.3% of the restless, 70.6% of the appropriately active, 18.8% of the sluggish, and 10% of the immobile patients showed improveme

Persistent neuropsychological deficits following whiplash: evidence for chronic mild traumatic brain injury?

OBJECTIVE: To investigate claims of neuropsychological evidence for acquired brain damage (axonal degeneration) in chronic whiplash. DESIGN: Fifteen whiplash patients (Whiplash) were compared with 10 patients who had documented moderate-to-severe head injury (Mod-Sev), and with 24 patients who had chronic pain syndrome (CPS) and no history of head injury on two tests of mental efficiency considered highly sensitive to and specific for the subtle effects of brain trauma. All 3 groups, assessed 4 years after onset in a teaching hospital setting were matched for age, education, and IQ. Exclusion

The hip joint mobilization with movement technique improves muscle activity, postural stability, functional and dynamic balance in hemiplegia secondary to chronic stroke: a blinded randomized controlled trial.

BACKGROUND: People with stroke generally experience abnormal muscle activity and develop balance disorder. Based on the important role of the proximal joints of the lower extremity in balance maintenance, hip joint mobilization with movement technique can be applied to enhance normal joint arthrokinematics. Therefore, the present study aimed to investigate the effectiveness of hip joint mobilization with movement technique on stroke patients' muscle activity and balance. METHODS: Twenty patients aged between 35 and 65 years old with chronic stroke were randomly assigned either to an experimen

Neurological Complications Occurring After Liver Transplantation: Role of Risk Factors, Hepatic Encephalopathy, and Acute (on Chronic) Brain Injury.

Orthotopic liver transplantation (LT) remains the only way to definitively cure patients with the most severe liver diseases. Because the survival rate is now fairly high, important questions about neurological sequelae or quality of life after LT have emerged. Indeed, LT represents a peculiar situation because up to 30% of patients present with neurological symptoms after LT compared with only 4% after cardiac transplant and 0.5% after renal transplant. These postoperative neurological symptoms have long been interpreted as sequelae of hepatic encephalopathy (HE). However, postoperative decom

Acute leukoencephalopathy after buprenorphine intoxication in a 2-year-old child.

Leukoencephalopathies have been reported after heroin inhalation or ingestion, and buprenorphine injection, but the physiopathology remains unclear. We report here the first case of leukoencephalopathy caused by buprenorphine ingestion in a 2-year-old child who was admitted for coma and fever. Due to technical problems, the toxicology screen was delayed, and infectious disease was first suspected. A brain MRI found bilateral and symmetric white matter damages in the cerebral hemispheres and the cerebellum. Rapid recovery and positive toxicology screen for buprenorphine on day 4 confirmed the d

[Cryptococcus neoformans meningoencephalitis among apparently immunocompetent patients: description of two cases].

Cryptococcus neoformans is a ubiquitous fungal pathogen which causes human disease ranging from asymptomatic colonization of the lungs, to severe pneumonia, mediastinitis, meningitis or generalized infection. Although cryptococcal infection shows notably opportunistic features, it is sometimes also found among apparently immunocompetent individuals, with an extremely adverse outcome in the case of SNC involvement. Therefore, when faced with a presumed healthy person with anamnestic, clinical, CSF and instrumental findings consistent with chronic meningitis/meningoencephalitis, we must also con

A diffusion model account of normal and impaired readers.

Acquired aphasics and dyslexics with even very profound word reading impairments have been shown to perform relatively well on the lexical decision task, but direct contrasts with unimpaired participant's data is often complicated by extremely long reaction times for patient data. The dissociation between lexical decision and word naming performance shown by these patients is of theoretical importance, and here we present an analysis of processing underlying the lexical decision task. We are able to determine what aspects of performance are affected by acquired aphasics in the lexical decision
