Dialysis Disequilibrium Syndrome: brain death following hemodialysis for metabolic acidosis and acute renal failure--a case report.

BACKGROUND: Dialysis disequilibrium syndrome (DDS) is the clinical phenomenon of acute neurologic symptoms attributed to cerebral edema that … erebral edema with effacement of basal cisterns and generalized loss of gray-white differentiation. Brain death was declared. CONCLUSIONS: Death is a rare consequence of DDS in adults following HD. Several features may have predisposed this patient to DDS including: central nervous system adaptations from chronic kidney disease with efficient serum urea remo …

Changing typology of brain death liver donors: characteristics and impact of risk factors on short-term outcome.

BACKGROUND/AIMS: An effective recruitment plan and the use of donors with risk factors are essential means of compensating for the lack of organs, but mean a significant change in the type of donor. This study defines the characteristics of the donors currently used, evaluates their impact on short-term outcome and underlines the need for a different approach to organ selection. METHODOLOGY: Data concerning 244 patients were collected for the study of donor characteristics by age groups, and the multivariate analysis of the associated risk factors. RESULTS: There were no significant differen

Sudden sound-induced death in magnesium-deficient rats after repetitive episodes of seizures result from brain dysfunction.

To clarify the changes occurring in EEG and ECG from seizure induction to sudden death, 25 weanling male Sprague-Dawley rats were fed a severely Mg-deficient diet for 18 days. In nine of 25 Mg-deficient rats, both EEG and ECG were recorded from the beginning of seizures induction until sudden death. When flattening of EEG became after seizure occurrence, all nine Mg-deficient rats did not recover and death occurred within a few minutes. These results may support the idea that sudden death in Mg-deficient rats resulted from brain dysfunction after repetitive episodes of noise-induced seizures a

Safely Expanding the Liver Donor Pool by Utilization of Organs from Donation after Circulatory Death with Comparable Results to Donation After Brain Death, a Large Single-Center Experience.

BACKGROUND: Use of livers donated after circulatory death (DCD) is one way to expand the donor pool. Our center has aggressively incorporated use of DCD liver grafts into practice. We examined our center and national outcomes as well as national DCD liver utilization. METHODS: Liver transplants performed at our center and nationally from 11/2016 through 9/2020 were compared. Primary outcomes were patient and graft survival, and national DCD liver utilization. RESULTS: For our center, DCD and donation after brain death (DBD) donors were similar except DCD donors were younger (37 vs 40 years; …

Incidence of Brainstem Dysfunction and Brain Death and End-of-Life Care Practices: A Two-Year Retrospective Review.

OBJECTIVES: Brainstem dysfunction and brain death are important determinants of End-of-life care (EoLC) decisions in intensive care units (ICUs). In many low- and middle-income settings, withdrawal or withholding of life-sustaining treatment remain ethically and culturally challenging despite high costs and limited critical-care resources. This study reviewed the incidence, causes and outcomes of brainstem dysfunction/brain death and examined relatives' decisions regarding EoL care (EoLC) in a tertiary hospital ICU. MATERIALS AND METHODS: A retrospective chart review was conducted in the ICU …

The role of sex hormones in the intestinal injury after brain death using a surgical menopause model in rats.

Among transplantable organs, the intestine is one of the most challenging organs to transplant. While there is considerable research on the effects of brain death (BD), little is known about the specific intestinal changes that occur, particularly in females. Here we investigated the role of female sex hormones in the BD-induced intestinal inflammation, using an ovariectomy (OVx) model for sex hormones depletion. Wistar rats (female) were divided into four experimental groups: Control non-OVx - non-manipulated; Control-OVx -ovariectomized; BD non-OVx - animals submitted to BD (6h); BD-OVx -ova…

Graft ischemia post cell transplantation to the brain: Glucose deprivation as the primary driver of rapid cell death.

Replacing cells lost during the progression of neurodegenerative disorders holds potential as a therapeutic strategy. Unfortunately, the majority of cells die post-transplantation, which creates logistical and biological challenges for cell therapy approaches. The cause of cell death is likely to be multifactorial in nature but has previously been correlated with hypoxia in the graft core. Here we use mathematical modelling to highlight that grafted cells experiencing hypoxia will also face a rapid decline in glucose availability. Interestingly, three neuron progenitor types derived from stem

Disparities in end-of-life care and place of death in people with malignant brain tumors-A Swedish registry study.

BACKGROUND: Malignant brain tumors often lead to death. While improving future treatments is essential, end-of-life care must also be addressed. To ensure equitable palliative care, understanding the place of death is crucial, as disparities may lead to inequity of care. This study aims to identify the place of death in adults with malignant brain tumors in Sweden, and the potential associations with official palliative care status by the ICD-10 code Z51.5, sociodemographic factors, health service characteristics, and healthcare service utilization. METHODS: A population-level registry study

Association of Brain Arterial Elongation With Risk of Stroke and Death in Stroke-Free Individuals: Results From NOMAS.

BACKGROUND: Brain arterial dilation and elongation characterize dolichoectasia, an arteriopathy associated with risk of stroke and death. We aim to determine whether brain arterial elongation increases the risk of stroke and death independent of brain arterial diameters. METHODS: We analyzed 1210 stroke-free participants (mean age 71±9 years, 41% men, 65% Hispanic) with available time-of-flight magnetic resonance angiogram from the Northern Manhattan Study, a population-based cohort study across a multiethnic urban community. We obtained baseline middle cerebral artery M1-segment (MCA-M1) and

Necroptosis is one of the modalities of cell death accompanying ischemic brain stroke: from pathogenesis to therapeutic possibilities.

Due to very limited therapeutic options, ischemic brain injury is one of the leading causes of death and lifelong disability worldwide, which imposes enormous public health burden. One of the main events occurring with ischemic brain stroke is cell death. Necroptosis is a type of cell death described as a regulated necrosis characterized by cell membrane disruption mediated by phosphorylated mixed lineage kinase like protein (MLKL). It can be triggered by activation of death receptors (eg, FAS, TNFR1), which lead to receptor-interacting serine/threonine-protein kinase 3 (RIPK3) activation by R

Vaginal delivery in the 30+4 weeks of pregnancy and organ donation after brain death in early pregnancy.

A 28-year-old woman suffered a traffic accident resulting in severe head injuries with deleterious prognosis. Diagnostics further revealed a hitherto unknown pregnancy, at suspected week 9. Based on the patient's wish to donate organs, brain death protocol confirmed irreversible loss of brain function. Yet, vital pregnancy rendered organ transplantation impossible. Multiple ethical and legal issues arose, from invalidation of established legal care after brain death to the delivery of a healthy child after trauma and long-term critical care. After medicolegal and ethical counselling, pregnancy…

Early inotropes use is associated with higher risk of death and/or severe brain injury in extremely premature infants.

Introduction: Extremely premature infants are susceptible to fluctuations in cerebral blood flow due to immaturity of cerebral autoregulation. Inotropes may cause rapid changes to systemic blood pressure and consequently cerebral blood flow, especially within the first 72 hours of life. This period is recognized to carry the greatest risk for cerebral hemorrhage. This study evaluates the incidence of death and/or severe brain injury in extremely preterm infants treated with inotropes in the first 72 hours of life. Methods: Prospective cohort study of infants born ≤29+0 weeks gestational age (GA

The impairment of HCCS leads to MLS syndrome by activating a non-canonical cell death pathway in the brain and eyes.

Mitochondrial-dependent (intrinsic) programmed cell death (PCD) is an essential homoeostatic mechanism that selects bioenergetically proficient cells suitable for tissue/organ development. However, the link between mitochondrial dysfunction, intrinsic apoptosis and developmental anomalies has not been demonstrated to date. Now we provide the evidence that non-canonical mitochondrial dependent apoptosis explains the phenotype of microphthalmia with linear skin lesions (MLS), an X-linked developmental disorder caused by mutations in the holocytochrome c-type synthase (HCCS)gene [corrected]. Computed tomography angiography (CTA) to prove circulatory arrest for the diagnosis of brain death in the context of organ transplantation.

QUESTION UNDER STUDY: For the determination of brain death (BD) in potential organ donors, confirmatory tests that show cessation of cerebral circulation are used in many countries. Conventional angiography is considered the golden standard among these ancillary examinations. In recent years other angiographic techniques such as CT angiography (CTA) have been increasingly employed to establish the diagnosis of BD. We report our experience with CTA in this setting. MATERIAL AND METHODS: From 2007 to 2010, 29 patients were examined in order to determine BD using CTA. The studies consisted of an…

Standardized experimental brain death model for studies of intracranial dynamics, organ preservation, and organ transplantation in the pig.

OBJECTIVES: Brain death impairs organ function and outcome after transplantation. There is a need for a brain death model to allow studies of organ viability and preservation. For neurointensive care research, it is also of interest to have a relevant brain death model for studies of intracranial dynamics and evaluation of cerebral monitoring devices. Therefore, the objective was to develop a standardized clinically relevant brain death model. METHODS: Six pigs of both sexes (10-12 wks old; mean weight, 24.5±1.4 kg) were included. Mean arterial blood pressure, heart rate, intracranial pressur…

Clinical report—Guidelines for the determination of brain death in infants and children: an update of the 1987 task force recommendations.

OBJECTIVE: To review and revise the 1987 pediatric brain death guidelines. METHODS: Relevant literature was reviewed. Recommendations were developed using the GRADE system. CONCLUSIONS AND RECOMMENDATIONS: (1) Determination of brain death in term newborns, infants and children is a clinical diagnosis based on the absence of neurologic function with a known irreversible cause of coma. Because of insufficient data in the literature, recommendations for preterm infants less than 37 weeks gestational age are not included in this guideline. (2) Hypotension, hypothermia, and metabolic disturbances…

Effects of lamotrigine alone and in combination with MK-801, phenobarbital, or phenytoin on cell death in the neonatal rat brain.

The neonatal rat brain is vulnerable to neuronal apoptosis induced by antiepileptic drugs (AEDs), especially when given in combination. This study evaluated lamotrigine alone or in combination with phenobarbital, phenytoin, or the glutamate antagonist (+)-5-methyl-10,11-dihydro-5H-dibenzo[a,d]cyclohepten-5,10-imine hydrogen maleate (MK-801) for a proapoptotic action in the developing rat brain. Cell death was assessed in brain regions (striatum, thalamus, and cortical areas) of rat pups (postnatal day 8) by terminal deoxynucleotidyl transferase dUTP nick-end labeling (TUNEL) assay, 24 h after

A Novel Regulatory System in Brain Development and Death Driven by a Bioactive Peptide Linked to a Specific Isoform of Acetylcholinesterase.

BACKGROUND: Acetylcholinesterase (AChE) demonstrates non-enzymatic actions attributable to a 14mer bioactive peptide derived from its sixth exon, 'T14', a pivotal agent driving Alzheimer's disease (AD). METHODS: To investigate this possible parallel between development and neurodegeneration, we tracked the expression and timeline of T14 in embryonic and neonatal rodent brain tissue in relation to the expression of its parent molecule, AChE, benchmarked against markers of neural maturation (NeuN) and neurodegenerative pathology (pTau) using sedimentation profiling, Western blotting, and qPCR.

Similarities in the Electrographic Patterns of Delayed Cerebral Infarction and Brain Death After Aneurysmal and Traumatic Subarachnoid Hemorrhage.

While subarachnoid hemorrhage is the second most common hemorrhagic stroke in epidemiologic studies, the recent DISCHARGE-1 trial has shown … aumatic brain injury and subarachnoid hemorrhage are the second and third most common etiologies of brain death during continued systemic circulation. Here, we use examples to illustrate that although the pathophysiological cascades associated with brain death are global, they closely resemble the local cascades associated with the development of delayed cere …
