Response to alternative antidepressants in imipramine nonresponders with bulimia nervosa.

The authors report the results of open-label antidepressant trials in a series of normal weight bulimic women who showed an inadequate response to an initial trial of imipramine hydrochloride as part of a treatment comparison study. The results indicate that a significant percentage of subjects responded to a second trial of an alternative agent. Overall, 47% had a complete remission of symptoms during the second trial, usually with a monoamine oxidase inhibitor or fluoxetine. The authors stress the importance of persisting in trials of alternative agents if bulimic patients fail to respond to

A comment on the role of dietary restraint in the development of bulimia nervosa.

The evidence is examined that dietary restraint is causally implicated in the development of bulimic episodes as experienced by some clinical populations, notably patients with bulimia nervosa. An analysis of some of the methodological and conceptual difficulties in this area of research reveals that both the concept of dietary restraint and the phenomenon of overeating in the laboratory are less straightforward than has been assumed, and that conclusions about the causal role of dietary restraint in the development of disordered eating are premature. Future research should be based on sophist…

Outcome in anorexia nervosa and bulimia nervosa. A review of the literature.

Forty follow-up studies of anorexia nervosa and bulimia nervosa are reviewed. Data on recovery, relapse, and crossover are presented in addition to data on mortality, weight, eating behavior, menses, psychological functioning, and psychosexual functioning. Predictors of these outcomes are discussed. Guidelines for future research are suggested to decrease the variability that is created by differences in research methodologies and study designs.

The prevalence of bulimia nervosa in the US college student population.

A phone survey to determine the prevalence of bulimia nervosa was conducted with a national probability sample of 1,007 male and female students from a stratified sample of 53 universities and colleges in the continental United States. Using the revised (DSM IIIR) diagnostic criteria, 1 per cent of the women and 0.2 per cent of the men were classified as bulimic. Bulimia was most prevalent (2.2 per cent prevalence rate) among undergraduate women living in group housing on campus.

Mitral valve prolapse in patients with anorexia nervosa and bulimia.

We studied 43 consecutive individuals with eating disorders (anorexia nervosa and/or bulimia) for the presence of mitral valve prolapse and/or cardiac arrhythmias by physical examination, M-mode and two-dimensional echocardiography, and 24-hour continuous ambulatory electrocardiographic monitoring. Ten of the 43 had findings of mitral valve prolapse on resting cardiac auscultation. Echocardiographic evaluation confirmed the diagnosis of mitral valve prolapse in these ten as well as in six other individuals, giving an overall incidence of 37% (16/43). Similar echocardiographic findings were pre…

Neuropathy and myopathy in two patients with anorexia and bulimia nervosa.

Two adolescent patients with eating disorders and severe weight loss presented with neuromyopathy. The first was female and had a twenty months' history of bulimia nervosa with weight loss and episodic gorging and vomiting. The second was male with a two-year history of anorexia nervosa characterised by vegetarianism and increasing food restriction. Both had severe wasting and asymmetrical weakness of proximal limb muscles. The first patient deteriorated on refeeding and became temporarily paralysed. Both had a purpuric rash and haematological abnormalities. They made a complete recovery on a …

Towards a functional analysis of anorexia nervosa and bulimia nervosa.

This paper is concerned with the description of an aetiological model of anorexia nervosa and one form of bulimia nervosa. The model has been generated within the conceptual framework of 'functional analysis' and is based on a combination of research evidence and clinical observation. The model attempts to explain the development of anorexia nervosa in terms of a set of hypothesized antecedent events/variables and its maintenance/exacerbation in terms of both positive and negative reinforcers. Specifically it is suggested that initial dieting behaviour is triggered by apparently innocuous psyc…

A test of the social withdrawal syndrome hypothesis of bulimia nervosa.

BACKGROUND: The study examined the social withdrawal syndrome (SWS) hypothesis of bulimia nervosa (BN). According to the hypothesis, eating disorders such as BN are associated with a coherent set of social withdrawal cognitions, affect, and behavior. PARTICIPANTS AND PROCEDURE: Eight-eight young female adults completed a standardized measure of bulimic symptoms and measures of social withdrawal (affective withdrawal, trust beliefs in close others, and disclosure). Participants were engaged in a laboratory-based peer interaction which yielded the SWS measure of perceived lack of social connect…

Reinforcement sensitivity and bulimia symptoms: the role of emotion regulation.

OBJECTIVE: Reinforcement Sensitivity Theory (RST) provides a theoretical foundation associated with various approach and avoidance behaviors … ationships, as both negative urgency and distress tolerance have been independently associated with bulimia behaviors. Thus, this is an area that requires further investigation. METHOD: The current study utilized various self-report inventories, including the Eating Disorder Inventory-3rd Edition to measure bulimia symptoms, as well as measures of negati …

Severity of somatic symptoms in outpatients with anorexia and bulimia nervosa.

OBJECTIVE: Prior studies investigated objective somatic consequences of eating disorders whereas research on subjective somatic symptom severity, that is, profiles of subjective burden of somatic symptoms in patients with anorexia (AN) and bulimia nervosa (BN), is sparse. METHODS: Somatic symptom severity (Patient Health Questionnaire-15) was investigated in a cross-sectional consecutive sample of outpatients with AN or BN. Using regression and mediation analyses, effects of somatic symptom severity on days of sick leave during the last 2 weeks and quality of life were examined. RESULTS: Com…

[Anorexia and bulimia nervosa: virtual diffusion of the disease as a lifestyle].

Introduction: Adolescence is a vulnerable period for the onset of eating disorders (ED) such as anorexia and bulimia nervosas. Body dissatisfaction, a precipitating factor for ED, leads adolescents to seek information on the Internet about diets. In this context, pro-Ana (proanorexia) and pro-Mia (probulimia) are on-line pages that promulgate highly harmful contents for health related to weight loss and ED. Objectives: The aim of this study was to analyze quantity, quality and social diffusion strategies used by pro-Ana and pro-Mia webpages. Methods: A web search was done in the Google Chrome …

Emotion Recognition in Face and Body Motion in Bulimia Nervosa.

Social cognition has been studied extensively in anorexia nervosa (AN), but there are few studies in bulimia nervosa (BN). This study investigated the ability of people with BN to recognise emotions in ambiguous facial expressions and in body movement. Participants were 26 women with BN, who were compared with 35 with AN, and 42 healthy controls. Participants completed an emotion recognition task by using faces portraying blended emotions, along with a body emotion recognition task by using videos of point-light walkers. The results indicated that BN participants exhibited difficulties recogni…

The impact of bulimia nervosa on oral health: A review of the literature.

Eating disorders are a potentially life-threatening group of mental disorders, which affect a patient's relationship with food and their body. This manifests itself through chaotic and disordered eating habits. One such eating disorder is bulimia nervosa, which has a lifetime prevalence of 1%. While there is consensus that bulimic behaviour directly causes dental erosion due to vomiting and acidic food choices, there is less clear evidence for a direct link between bulimia nervosa and dental caries, although there does still appear to be an association. Reduced salivary flow rate is a common f…

Neural signature of behavioural inhibition in women with bulimia nervosa.

BACKGROUND: Impaired inhibitory control is considered a behavioural phenotype in patients with bulimia nervosa. However, the underlying neural correlates of impaired general and food-specific behavioural inhibition are largely unknown. Therefore, we investigated brain activation during the performance of behavioural inhibition to general and food-related stimuli in adults with bulimia nervosa. METHODS: Women with bulimia and healthy control women underwent event-related fMRI while performing a general and a food-specific no-go task. RESULTS: We included 28 women with bulimia nervosa and 29 h…

Therapeutic alliance in a randomized clinical trial for bulimia nervosa.

OBJECTIVE: This study examined the temporal relation between therapeutic alliance and outcome in two treatments for bulimia nervosa (BN). METHOD: Eighty adults with BN symptoms were randomized to 21 sessions of integrative cognitive-affective therapy (ICAT) or enhanced cognitive-behavioral therapy (CBT-E). Bulimic symptoms (i.e., frequency of binge eating and purging) were assessed at each session and posttreatment. Therapeutic alliance (Working Alliance Inventory) was assessed at Sessions 2, 8, 14, and posttreatment. Repeated-measures analyses using linear mixed models with random intercepts…

Weight suppression in bulimia nervosa: Associations with biology and behavior.

Bulimia nervosa (BN) is a serious eating disorder that can persist for years and contribute to medical complications and increased mortality, underscoring the need to better understand factors maintaining this disorder. Higher levels of weight suppression (WS) have been found to predict both the onset and maintenance of BN; however, no studies have examined mechanisms that may account for the effects of WS on BN. We hypothesized that high WS would lead to reduced leptin levels, which may increase risk of binge eating by modulating reward responses to food. The current study examined the relati…
