Cure of interfascicular reentrant ventricular tachycardia by ablation of the anterior fascicle of the left bundle branch.

INTRODUCTION: Fascicular reentrant ventricular tachycardia (VT) using the anterior fascicle of the left bundle anterogradely is rare and may produce identical QRS morphology during sinus rhythm and VT. Catheter ablation of this type of VT has not been described in detail. METHODS AND RESULTS: In a postinfarct patient with dilated left ventricle and recurrent VT (showing a QRS configuration of right bundle branch, left posterior fascicular block), endocardial recordings from the His-Purkinje system showed that VT was due to interfascicular reentry. Induction of VT occurred after progressive re

New antiarrhythmic drugs in pediatric use: propafenone.

Propafenone hydrochloride, a class 1C antiarrhythmic agent, combines sodium channel-blocking effects with beta-blocking capacities and a weak calcium antagonism. The drug exerts marked electrophysiologic effects on accessory atrioventricular pathways. In patients with atrioventricular nodal reentry tachycardia, propafenone is able to block conduction in the fast conducting pathway. In addition, propafenone is very effective in young patients with supraventricular tachycardia based on enhanced abnormal automaticity. In pediatric patients, left ventricular performance remains unimpaired. Proarrh

The Role of Paced QRS Morphology in Determining Initial Site for Lead Placement in Left Bundle Branch Area Pacing.

BACKGROUND: Determining proper initial site on the right side of interventricular septum is the first step to implant LBBAP lead. Electrocardiographic features such as discordance of paced QRS polarity in leads II and III (alternatively in aVL and aVR) and the presence of notch in lead V1 are in use but have not yet been evaluated. This study aimed to establish the role of paced QRS morphology in determining the initial site for LBBAP lead deployment. METHODS: A group of 150 LBBAP device implantations containing 327 lead placement attempts were analyzed. Final electrocardiographic results of

Autothreshold algorithm feasibility and safety in left bundle branch pacing.

AIMS: Autothreshold algorithms enable remote monitoring of patients with conventional pacing, but there is limited information on their performance in left bundle branch pacing (LBBP). Our objective was to analyse the behaviour of the autothreshold algorithm in LBBP and compare it with conventional pacing and manual thresholds during initial device programming (acute phase), after 1-7 days (subacute), and 1-3 months later (chronic). METHODS AND RESULTS: A prospective, non-randomized, single-centre comparative study was conducted. Consecutive patients with indication for cardiac pacing were en

Hisian extrasystole mimicking second degree atrioventricular block. a case report.

Hisian extrasystoles originate from the His bundle. They are rare and usually misdiagnosed. In fact, they manifest on the EKG, with a p' wave located before, in or after the QRS complex. More rarely, the extrasystole blocks the propagation of the influx to the ventricles simulating a Mobitz II atrioventricular (AV) block. We report the case of a 36-Year-old woman with no medical History, suffering from presyncope and palpitations at rest. Her physical examination and EKG were normal. The 24-hour Holter monitoring showed some long periods with unexpected blocked p waves but with no significant

Wenckebach Phenomenon in the His-Purkinje System.

The author discusses Wenckebach phenomenon (WP) in the His-Purkinje system (HPS). Subtle PR changes may be interpreted as no change, often seen in the HPS-WP. Changes in the QRS axis help to localize the site of delay and block along fascicles of the left bundle branch. Marked PR changes may occur during HPS-WP. All the QRS complexes that are conducted depict right bundle branch and left anterior superior fascicular block, so the visible changes in the HV interval suggest WP mostly occurring in the left posterior inferior fascicle.

Atrioventricular block precipitated by isoproterenol.

An isoproterenol infusion was administered during an electrophysiologic study (EPS) in a patient with a history of near syncope, left bundle branch block, and no documented atrioventricular (AV) block. Isoproterenol precipitated classic 2:1 Infra-Hisian AV block most probably proximal to the site of recording a His-Purkinje potential consistent with right bundle branch activity. Paroxysmal AV block also occurred during isoproterenol washout at a different site located distal to the presumed right bundle branch potential. Isoproterenol may be valuable diagnostically in an occasional patient sus

[Disorders of intraventricular conductivity in primary myocardial infarct].

Recent knowledge of the system of intraventricular conductivity permits to diagnose much more often the different variants of blocks of the main branches of the bundle of His that develop in acute primary myocardial infarction (41.42%). Clinical and electrocardiographic examinations of 239 patients with acute primary myocardial infarction revealed unilateral conductivity disorders in the right ventricle in 19.24% of the cases, in the left ventricle -- in 13.39%, and bilateral conductivity disorders -- in 8.79%. The main branches of the bundle of His blocks were combined with atrioventricular c…

Catheter technique for closed-chest ablation of the atrioventricular conduction system.

This report describes a catheter technique for ablating the His bundle and its application in nine patients with recurrent supraventricular tachycardia that was unresponsive to medical management. A tripolar electrode catheter was positioned in the region of the His bundle, and the electrode recording a large unipolar His-bundle potential was identified. In the first patient, two shocks of 25 and 50 J, respectively, were delivered by a standard cardioversion unit to the catheter electrode, resulting in an intra-His-bundle conduction defect. Subsequent delivery of 300 J resulted in complete hea

Concealed nonparoxysmal junctional tachycardia.

Electrophysiologic studies were performed in a patient who had an apparently uncomplicated complete trifascicular block. His bundle recordings revealed atrioventricular dissociation with: 1) an atrial rate of 58 beats/min, 2) an idioventricular escape rate of 45 beats/min, and 3) nonparoxysmal junctional tachycardia (His bundle rhythm) at a rate of 65--85 beats/min. The latter arrhythmia was electrocardiographically silent, influencing neither atrial nor ventricular events. The arrhythmia probably reflected digitalis intoxication (digoxin level of 3.3 ng/ml). A repeat electrophysiologic study

A New Electromechanical Wave Imaging Dispersion Metric for the Characterization of Ventricular Activation in Different Cardiac Resynchronization Therapy Pacing Schemes.

Conventional biventricular (BiV) pacing cardiac resynchronization therapy (CRT) is an established treatment for heart failure patients. Recently, multiple novel CRT delivering technologies such as His-Bundle pacing have been investigated as alternative pacing strategies for optimal treatment benefit. Electromechanical Wave Imaging (EWI), a high frame-rate echocardiography-based modality, is capable of visualizing the change from dyssynchronous activation to resynchronized BiV-paced ventricles in 3D. This proof-of-concept study introduces a new EWI-based dispersion metric to further characteriz

Dynamic Changes of the S Wave in Lead V6: A Novel Visual Indicator for Left Bundle Branch Capture.

BACKGROUND: During the electrode screwing process in left bundle branch pacing (LBBP), the significance of the S wave in lead V6 remains elusive. In this study we analyzed the change of the S wave in lead V6 under different patterns of capture and explore its mechanisms. METHODS: Our study included 243 patients with selective LBBP (SLBBP). We used the continuous pacing technique and classified the electrophysiologic characteristics observed during the screwing process into 4 patterns: left ventricular septal pacing (LVSP), nonselective LBBP (NSLBBP) in low output and in lower output, and sele

A shorter R wave peak time in left bundle branch pacing may not be a marker of more physiological ventricular activation: A case report.

Left bundle branch pacing (LBBp) is a promising alternative to conventional biventricular pacing cardiac resynchronization therapy. The left anterior fascicle (LAF) is adjacent to the left ventricular outflow tract, while the left posterior fascicle (LPF) dominates a broader area of the left ventricle. Whether LAF or LPF dominates ventricular activation has not been determined. We present the case of a 76-year-old man who underwent LBBp implantation and propose the left ventricular activation domination in LPF pacing, an alternative when LBBp is unavailable.

Ventricular versus atrial side ablation for treatment of atrioventricular accessory pathways: a randomized controlled clinical trial.

BACKGROUND: The earliest atrial (A)/ventricular (V) activation potentials, or fused A/V potentials, are commonly used as ablation targets for atrioventricular (AV) accessory pathways (APs). However, these targets can be achieved in a relatively wide area of the heart around AV rings at both atrial and ventricular sides. The aim of this study is to analyze the height of intracardiac A and V waves and their correlation to find the most appropriate side for successful delivery of radiofrequency energy, atrial or ventricular edge. METHODS: Ninety patients diagnosed with orthodromic AV re-entrant
