Neuroimaging of Koolen-De Vries Syndrome: A Rare Genetic Disorder.

Koolen-de Vries syndrome (KdVS) is a rare neurodevelopmental genetic disorder involving multiple organ systems. It is characterized by several abnormal neurological, behavioral, and skeletal features. This case describes a 12-year-old male who had been previously diagnosed with Koolen-de Vries syndrome and has a complex medical history. The patient exhibits many complications of KdVS, such as congenital heart disease, epilepsy, cryptorchidism, and cardiomegaly. Neuroimaging displayed a dysplastic corpus callosum, colpocephaly with mild lateral and third ventriculomegaly, and an ovoid dysmorphi…

A rapidly progressive case of tuberculous pleurisy and pericarditis in a patient with non-small cell lung cancer that developed one month after receiving pembrolizumab monotherapy.

We report a rapidly progressive case of tuberculous pleurisy and pericarditis. A 59-year-old, male patient with non-small-cell lung cancer commenced pembrolizumab monotherapy one month before but soon thereafter had fevers and dyspnea. Radiography revealed increased right pleural effusion, novel left pleural effusion and cardiomegaly, which had been absent 10 days earlier when a reduction in the target lesion was confirmed. Computed tomography revealed the presence of pericardial fluid. Analysis of the pleural effusion didn't detect malignancy; however, the culture was positive for tuberculosi…

Cantù syndrome: Report of a patient with a novel variant in KCNJ8 and revision of literature.

Cantù syndrome (CS) is a rare multisystemic disorder, characterized by congenital hypertrichosis, macrocephaly, facial dysmorphisms, cardiomegaly, vascular, and skeletal anomalies. From the cognitive point of view, most of the patients show a mild speech delay and a few of them present intellectual disability and learning difficulties. To date, most CS-reported cases are caused by heterozygous ABCC9 gene mutations. Only three patients with CS and heterozygous KCNJ8 gene variants have been reported. The authors here present the fourth case of CS with a variant in KCNJ8 in a 6-month-old baby. Di…

Estimation of vertebral heart size and cardiothoracic ratio in Persian cats.

The study aimed to determine the reference values related to vertebral heart size (VHS) and cardiothoracic ratio (CTR), which are effective in the evaluation of the thorax diseases and especially cardiomegaly of the brachycephalic cats using the morphometric data of thorax and heart obtained from the radiographic images of the thoracic region of the Persian cats. It was determined that lying position during radiography did not affect VHS and CTR values. Also, the sex factor did not have a significance on these values. The fact that the VHS value of 8.16 thoracic vertebrae averagely found in th…

Incidental congenitally corrected transposition of the great arteries (ccTGA) in an adult with suspected coronary artery disease: review on radiological features and pathophysiology.

We report a case of a 46-year-old woman who has presented to a peripheral hospital with progressive exertional dyspnoea and chest discomfort. The resting ECG showed features of left-sided ventricular hypertrophy. The initial chest radiograph was reported as cardiomegaly. Initial echocardiography revealed left atrial dilatation and 'left ventricular' hypertrophy with normal ejection fraction. She was treated as possible coronary artery disease and was subsequently referred to our centre for CT coronary angiography. Findings from the CT scan were consistent with congenitally corrected transposit…

[Giant left atrium].

Woman 48 years old with progressive dyspnea, fatigue, dysphagia, lower limb edema, palpitations and decreased physical ability that made her to seek medical attention. The chest film showed severe cardiomegaly or "cor bovis" with cardiothoracic index of 0.78. The findings suggest a dilatation of both atria but mostly in the one. The echocardiogram shows severe mitral regurgitation consistent with rheumatic valve disease and "giant" left atrium. This was first described in 1938 by Semans and Taussig, is considered a rare finding and the principal cause is rheumatic mitral disease. This type of …

Diagnosis and treatment of foetal heart failure: foetal echocardiography and foetal hydrops.

Foetal echocardiography has progressed to be able to diagnose many forms of CHD and to assess the prognosis of cardiac lesions based on their anatomy and presentation in utero. This article outlines a straightforward method for the rapid evaluation of foetus that may have congestive heart failure with or without hydrops and for the differentiation of the pre-hydropic state from normal. The presence of signs of foetal heart failure, such as cardiomegaly or valvular regurgitation, gives clues to the aetiology of hydrops. The assessment of the prognosis of hydrops foetalis can be difficult but ca…

Dilated cardiomyopathy as a presenting feature of Cushing's syndrome.

Cardiovascular complications, including cardiomegaly, myocardial ischemia and left ventricular hypertrophy, are some of the major determinants of the mortality rate in patients with Cushing's syndrome. We herein report the case of a patient with Cushing's syndrome caused by an adrenal adenoma who presented with congestive heart failure secondary to dilated cardiomyopathy. Follow-up echocardiography showed a marked improvement in the left ventricular cardiac function, and the plasma B-type natriuretic peptide (BNP) levels regressed after successful treatment. "Reversible" dilated cardiomyopathy…

Constrictive pericarditis localized to left ventricle presented with left pleural effusion: a case report.

An 18-year old woman presented with progressive dyspnea and cough. Physical examination revealed decreased breath sounds at the left hemithorax and distant heart sounds with no murmurs or rub. Electrocardiogram revealed low voltage. Chest X-ray showed unilateral left-sided pleural effusion with no cardiomegaly. Transthoracic echocardiogram showed thickened pericardium localized throughout the left ventricle impairing the diastolic filling. Doppler waveforms were suggestive of localized constrictive pericarditis. A computerized tomographic scan of the chest confirmed the presence of unilateral …

Repair of aortico-left ventricular tunnel originating from the left aortic sinus.

We report on a case of an 11-year-old asymptomatic child with aortico-left ventricular tunnel arising from the left aortic sinus. Preoperative transesophageal echocardiography showed a dilated aortic root with mild aortic valve incompetence and demonstrated the course of the tunnel, which originated from the left coronary sinus entering the outlet portion of the left ventricular outflow tract. Patch closure of the aortic end of the tunnel eliminated left ventricular volume overload with immediate marked reduction of cardiomegaly. At 10-month follow-up the child is asymptomatic and receiving no…

Endoaneurysmal repair of a congenital right coronary sinus aneurysm.

An 82-year-old woman complained of lightheadedness, dizziness, syncope, palpitations, and chest pains of 2 years' duration. Chest X-ray demonstrated cardiomegaly, while transesophageal echocardiography (TEE) disclosed an aneurysm of the right coronary sinus (RCS), 6 x 6 cm in diameter, filled with clots and obstructing the right ventricular (RV) outflow tract. A total cardiopulmonary bypass was instituted with hypothermia to 28 degrees C esophageal temperature. After removing the clots, we applied an endoaneurysmal repair with a synthetic patch to the entry of the aneurysm and closed the aneur…

Plain-film assessment of the neonate with D-transposition of the great vessels.

The presenting chest radiographs of 27 new-born patients with D-transposition of the great vessels (D-TGV) were evaluated for the degree of pulmonary flow as well as other findings classically described in D-TGV (narrow superior mediastinum, radiographically absent thymus, inapparent main pulmonary artery, non-visualization of the malpositioned aortic arch, asymmetric pulmonary blood flow, and cardiomegaly). Of the 27 patients, 22 (82%) demonstrated normal or decreased flow. The majority of the D-TGV patients also failed to demonstrate any of the other classically described radiographic findin…

Cardiac involvement in glycogen storage disease type III.

Twenty patients with enzymatically proven glycogen storage disease type III (GSD III) aged 3-30 years underwent cardiological evaluation. Seventeen showed subclinical evidence of cardiac involvement in form of ventricular hypertrophy on ECG. Of 16 patients in whom an ECG examination was performed, 13 had abnormal echocardiographic features. Only 2 patients had cardiomegaly on X-ray. The cardiac findings in 1 of the patients, a 25-year-old female with clinically evident cardiomyopathy are described in detail. In view of our findings, patients with established GSD III, should not only be investi…

Idiopathic cardiomyopathy in C3Hf/Bd mice.

Male and female 16 to 18 month old C3Hf/Bd mice in a dermal carcinogenicity study were moribund or died at earlier time points than the expected 24 to 30 months. Clinical signs observed in both treated and control animals included dyspnea, lethargy, and death. Lesions seen in treated as well as control mice were cardiomegaly with myocardial degeneration and necrosis, hydrothorax and pulmonary edema, and ascites and chronic passive congestion of the liver. Mice were negative for serologic, bacteriologic and microscopic evidence of viruses, bacteria and protozoa which can induce heart lesions. P…

Thyrotoxic myopathy in mice: accentuation by a creatine transport inhibitor.

To demonstrate the importance of creatine and phosphocreatine in skeletal muscle during periods of metabolic stress, thyrotoxicosis was induced in mice fed the creatine transport inhibitor, beta-guanidinopropionic acid (beta-GPA). Adding 2% of beta-GPA to the diet of normal mice inhibited weight gain and caused a 75% reduction of creatine and phosphocreatine concentrations in skeletal muscle. Addition of 0.25% or 2% of thyroid powder to the diet of normal mice was associated with hyperactivity, cardiomegaly, and a high mortality rate. Superimposing thyrotoxicosis on mice already depleted of cr…

Echocardiographic assessment of cardiac function and morphology in patients with generalised lipodystrophy.

Because cardiomegaly has been observed in lipodystrophic patients we studied cardiac morphology and function with one- and two-dimensional echocardiography in addition to general cardiologic examination in a series of seven patients. Muscular hypertrophy with increased chamber size and myocardial indentations were found. Two patients had asymmetrical septal hypertrophy (ASH), and two patients demonstrated systolic anterior movement (SAM) of the mitral valve. Wall motion analysis showed anomalities in four patients during contraction, in three during the early relaxation phase. Since pathologic…

[Uhl's anomaly associated with atresia of the pulmonary valve].

A case of Uhl's anomaly in a newborn who died in congestive cardiac failure at 18 days of age is described. There was severe cardiomegaly on chest X-ray, right atrial hypertrophy on E.C.G., and a delayed tricuspid valve closure of 0,8 seconds on M-mode echocardiogram. The necropsy study showed a huge right atrial and right ventricular dilatation, a very thin right ventricular wall (1 mm.) with an important reduction in the number of muscular fibers, a pulmonary valve atresia, and a patent ductus arteriosus and foramen ovale. We have only three cases Uhl's anomaly associated with pulmonary atre…

Congenital absence of the pulmonary valve leaflets associated with complete transposition of the great arteries.

We describe the clinical and surgical findings of a patient with absence of the pulmonary valve leaflets and complete transposition of the great arteries. The main clinical features were a loud, single second heart sound, a loud pansystolic murmur over the entire precordial area, but no diastolic murmurs were heard. The electrocardiogram suggested right ventricular hypertrophy and right bundle branch block. The roentgenographic study showed cardiomegaly with dilatation of both branches of the pulmonary artery. Cardiac catheterization and angiocardiography showed systemic arterial desaturation,…

Sleep apnoea in acromegaly.

Day time somnolence or excessive snoring, or both, occurred in five out of 11 patients with acromegaly. All five had episodes of sleep apnoea, and three had the sleep apnoea syndrome. Growth hormone concentrations were higher (p less than 0.025) in these patients than in the six patients without these symptoms. One patient with daytime somnolence and one asymptomatic patient had flow loop evidence of upper airways obstruction. Two of the patients with the sleep apnoea syndrome had cardiomegaly. Sleep apnoea appears to be common and clinically important in acromegaly, and it may be central, obs…
