Restrictive Cardiomyopathies: The Importance of Noninvasive Cardiac Imaging Modalities in Diagnosis and Treatment-A Systematic Review.

Restrictive cardiomyopathy (RCM) is the least common among cardiomyopathies. It can be idiopathic, familial, or secondary to systematic disorders. Marked increase in left and/or right ventricular filling pressures causes symptoms and signs of congestive heart failure. Electrocardiographic findings are nonspecific and include atrioventricular conduction and QRS complex abnormalities and supraventricular and ventricular arrhythmias. Echocardiography and cardiac magnetic resonance (CMR) play a major role in diagnosis. Echocardiography reveals normal or hypertrophied ventricles, preserved systolic…

Functional networks of nucleocytoplasmic transport-related genes differentiate ischemic and dilated cardiomyopathies. A new therapeutic opportunity.

Heart failure provokes alterations in the expression of nucleocytoplasmic transport-related genes. To elucidate the nucleocytoplasmic transport-linked functional network underlying the two major causes of heart failure, ischemic cardiomyopathy (ICM) and dilated cardiomyopathy (DCM), we examined global transcriptome profiles of left ventricular myocardium tissue samples from 31 patients (ICM, n = 10; DCM, n = 13) undergoing heart transplantation and control donors (CNT, n = 8) using RNA-Sequencing and GeneMANIA. Comparative profiling of ICM versus control and DCM versus control showed 1081 and

Altered interactions between cardiac myosin binding protein-C and α-cardiac actin variants associated with cardiomyopathies.

The two genes most commonly associated with mutations linked to hypertrophic or dilated cardiomyopathies are β-myosin and cardiac myosin binding protein-C (cMyBP-C). Both of these proteins interact with cardiac actin (ACTC). Currently there are 16 ACTC variants that have been found in patients with HCM or DCM. While some of these ACTC variants exhibit protein instability or polymerization-deficiencies that might contribute to the development of disease, other changes could cause changes in protein-protein interactions between sarcomere proteins and ACTC. To test the hypothesis that changes in …

Desmoplakin Cardiomyopathy, a Fibrotic and Inflammatory Form of Cardiomyopathy Distinct From Typical Dilated or Arrhythmogenic Right Ventricular Cardiomyopathy.

BACKGROUND: Mutations in desmoplakin (DSP), the primary force transducer between cardiac desmosomes and intermediate filaments, cause an arrhythmogenic form of cardiomyopathy that has been variably associated with arrhythmogenic right ventricular cardiomyopathy. Clinical correlates of DSP cardiomyopathy have been limited to small case series. METHODS: Clinical and genetic data were collected on 107 patients with pathogenic DSP mutations and 81 patients with pathogenic plakophilin 2 (PKP2) mutations as a comparison cohort. A composite outcome of severe ventricular arrhythmia was assessed. RES

Diagnostic dilemma: COVID-19 related cardiomyopathy or peripartum cardiomyopathy?

Peripartum cardiomyopathy is a relatively rare condition, that usually presents with features of heart failure in the peripartum period. The ongoing pandemic caused by coronavirus disease 2019 (COVID-19) has been reported to be associated with myocarditis, with progression to dilated cardiomyopathy and heart failure. Dilated cardiomyopathy in a peripartum patient with COVID-19 infection may present a diagnostic dilemma. We report a case of dilated cardiomyopathy in a peripartum patient with COVID-19 infection. She presented with shortness of breath in the peripartum period. Chest X-ray showed

Development of obstructive hypertrophic cardiomyopathy from nonobstructive hypertrophic cardiomyopathy.

A rare case of obstructive hypertrophic cardiomyopathy (HC) with mitral regurgitation (MR) is reported, which developed over 7 years from nonobstructive HC in an elderly woman. Systolic anterior motion of the anterior mitral leaflet was the most likely cause of the outflow obstruction and mitral valve replacement combined with septal myectomy resulted in complete abolition of the outflow tract gradient and MR.

Case Report: Hypertrophic cardiomyopathy meets left ventricular hypertrabeculation: from mixed cardiomyopathy to hypertrophic cardiomyopathy with hypertrabeculation.

This case report describes a young male patient admitted due to worsening of heart failure with recurrent embolic events. He had previously been diagnosed with hypertrophic cardiomyopathy (HCM) and left ventricular systolic dysfunction at other hospitals and was treated irregularly with medications including a beta-blocker and diuretics. On this admission, echocardiography and cardiac MR examination revealed left ventricular myocardial non-compaction combined with HCM. The coexistence of left ventricular hypertrophy and hypertrabeculation is rare, and the diagnosis has been updated according t

Family screening for genetic cardiomyopathies in ECG-positive phenotype-negative adolescents: a joint clinical and genetic approach.

Adolescents may present with abnormal ECG findings despite having no structural or electrical evidence of cardiomyopathy. The value of extending family screening to this subgroup remains uncertain. To evaluate the impact of family screening in ECG-positive phenotype-negative adolescents, and determine whether a joint clinical-genetic approach improves the diagnostic yield. In this prospective cohort study, adolescents with an abnormal ECG identified through a nationwide screening program underwent comprehensive clinical/genetic evaluation. Phenotype-negative individuals were included. Relative

Genetic Architecture of Pediatric Cardiomyopathies Assessed by Whole-Exome Sequencing: Insights Into Early-Onset and Syndromic Forms.

Pediatric cardiomyopathies (CM) are rare and heterogeneous heart disorders, including hypertrophic, dilated, restrictive, arrhythmogenic, and non-dilated CM. While their genetic basis is well characterized in adults, it remains less clearly defined in children particularly in early-onset apparently isolated and syndromic forms. We conducted a retrospective study (2018-2024) of 59 pediatric patients who underwent Whole-Exome Sequencing (WES) for CM at Amiens and Lille University Hospitals, aiming to characterize the genetic architecture of pediatric CM. WES identified at least one Variant Of In…

Cardiorenal and Metabolic Dimensions of Cardiomyopathies and Heart Failure: Focus on SGLT2i, GLP1-RA, and ns-MRA.

PURPOSE OF THE REVIEW: Metabolic disorders represent a growing epidemiological and clinical challenge, associated with worsening prognosis of cardiovascular disease. These conditions cluster in patients with heart failure and cardiomyopathies, limiting therapeutic options. Although cardiomyopathies contribute substantially to the burden of heart failure, the impact of metabolic disorders on their progression remains underexplored. This review explores the complex cardiorenal and metabolic dimensions of cardiomyopathies, focusing on pathophysiological mechanisms, therapeutic innovations, and fu…

Artificial Intelligence Application and Future Directions in Infiltrative and Inflammatory Cardiomyopathies: A State-of-the-Art Review.

Infiltrative and inflammatory cardiomyopathies carry substantial morbidity and mortality. Nonspecific clinical presentations can delay recognition; prognostic tools are limited. Artificial intelligence (AI) can mine clinical, imaging, biomarker, and histologic data to reveal multidimensional patterns beyond conventional interpretation. In infiltrative cardiomyopathies, early applications have concentrated on cardiac amyloidosis, with emerging work in Fabry disease and myocardial iron overload. For inflammatory cardiomyopathies, early studies suggest roles for AI-enhanced electrocardiography an…

Human iPSC-derived 3D cardiac models for cardiomyopathies: organoids, spheroids, and engineered heart tissues as translational platforms.

Cardiomyopathies represent a substantial global health burden, yet progress in developing effective therapies was long constrained by the absence of physiologically relevant human models to understand molecular mechanisms and evaluating interventions. The advent of induced pluripotent stem cell (iPSC) technology has driven major advances in cardiomyopathy research, although important limitations persist. Current iPSC-based systems exhibit incomplete cellular maturation, lack functional vasculature and chamber level architecture, and possess an immature extracellular matrix. These factors must …

CT and MR Imaging of Cardiomyopathies in Clinical Practice-An Approach After an Abnormal Echocardiogram or Electrocardiogram.

Cardiomyopathies represent a diverse group of myocardial disorders characterized by structural and functional abnormalities in the absence of significant coronary artery disease or other primary causes. This review highlights the diagnostic and prognostic value of cardiac magnetic resonance and computed tomography in the assessment of cardiomyopathies. While echocardiography remains the first-line imaging modality, cardiac magnetic resonance (CMR) and cardiac computerized tomography (CCT) offer superior tissue characterization, morphological assessment, and functional evaluation, crucial for p…

Comprehensive Proteomics Profiling Identifies Circulating Biomarkers to Distinguish Hypertrophic Cardiomyopathy From Other Cardiomyopathies With Left Ventricular Hypertrophy.

BACKGROUND: Distinguishing hypertrophic cardiomyopathy (HCM) from other cardiomyopathies with left ventricular hypertrophy (LVH), such as hypertensive LVH, transthyretin amyloid cardiomyopathy, and aortic stenosis, is sometimes challenging. Using plasma proteomics profiling, we aimed to identify circulating biomarkers and dysregulated signaling pathways specific to HCM. METHODS: In this multicenter case-control study, plasma proteomics profiling was performed in cases with HCM and controls with hypertensive LVH, transthyretin amyloid cardiomyopathy, and aortic stenosis. Two-thirds of patients…

The Importance of Multimodality Imaging in the Diagnosis and Management of Patients with Infiltrative Cardiomyopathies: An Update.

Infiltrative cardiomyopathies (ICMs) comprise a broad spectrum of inherited and acquired conditions (mainly amyloidosis, sarcoidosis, and hemochromatosis), where the progressive buildup of abnormal substances within the myocardium results in left ventricular hypertrophy and manifests as restrictive physiology. Noninvasive multimodality imaging has gradually eliminated endomyocardial biopsy from the diagnostic workup of infiltrative cardiac deposition diseases. However, even with modern imaging techniques' widespread availability, these pathologies persist in being largely under- or misdiagnose…

Clinical exome sequencing revealed that FLNC variants contribute to the early diagnosis of cardiomyopathies in infant patients.

BACKGROUND: FLNC encodes actin-binding protein and is mainly concentrated in skeletal and cardiac muscle. Mutations in FLNC were found in cardiomyopathies. To date, studies on FLNC-cardiomyopathies have mainly been reported in adults. There are limited studies that have investigated FLNC variants in pediatric patients with cardiomyopathies. METHODS: We summarized the patients who carried rare variants of FLNC from May 2016 to May 2019 in the Center for Molecular Medicine, Children's Hospital of Fudan University, from clinical exome sequencing data. RESULTS: A total of 5 patients with FLNC ra…
