Chondrotympanoplasty: a modified technique of cartilage graft tympanoplasty.

BACKGROUND: Although the temporalis fascia has been widely used since 1957as a graft in tympanoplasty, it can eventually become thin and atrophic. In addition, due to the lack of elasticity and resistance to pressure changes in the external ear canal, secondary perforations may also develop. Therefore, several authors have suggested that temporalis fascia should be strengthened by periosteum or replaced by cartilage. MATERIAL/METHODS: In the present paper the term chondrotympanoplasty is introduced to describe modified techniques of cartilage graft tympanoplasty and intratympanal chondroplast…

Candidate's thesis: Revision tympanoplasty utilizing fossa triangularis cartilage.

OBJECTIVES/HYPOTHESIS: A small percentage of fascia graft tympanoplasties fail. Cartilage tympanoplasty has a reputation for excellent graft healing but potentially sacrifices maximum hearing improvement and creates difficulty in postoperative follow-up resulting from opacity and immobility. We sought to use a tissue thicker than fascia but thinner than tragal cartilage to repair tympanic membranes that had failed previous fascia grafting. Our hypothesis was that use of the thinner cartilage would maintain the excellent healing rate and resistance to chronic negative pressure while improving h…

Preplanned correction of enophthalmos using diced cartilage grafts.

A simple technique for correcting post-traumatic enophthalmos is presented. Autogenous costal cartilage was chopped into small pieces and used to fill up the medial orbital wall defect in five patients. The measured degrees of enophthalmos ranged from 2 to 4 mm, and 3-5. 5 ml of filler material was introduced into the periorbital space. A conservative approach via upper and lower eyelid incisions was used. It was found that 1.37-1.5 ml of graft material results in 1 mm advancement of globe position. This correlation appears to be a useful treatment guideline for medial orbital wall blow-out fr…

Deficiency of humoral immunity in cartilage-hair hypoplasia.

OBJECTIVE: Cartilage-hair hypoplasia (CHH), a metaphyseal chondrodysplasia, is usually associated with impaired cellular immunity. This study evaluates humoral immunity in patients with CHH. METHODS: The concentrations of immunoglobulins G, A, and M (IgG, IgA, and IgM) and IgG subclasses were studied in 20 patients. Data for 5 additional patients with recurrent infections were retrospectively reviewed. RESULTS: Seven of the prospectively evaluated patients (35%) had defective humoral immunity. Three patients had IgA deficiency. Four patients had IgG2 deficiency, accompanied by IgA deficiency…

[Do some drugs have adverse effects on cartilage?].

The potential chondrotoxicity of drugs is very difficult to appreciate because of the difficulties involved in evaluating the evolution of cartilage in human beings. This article attempts to summarize the data from the literature concerning the hypothetical chondrotoxicity of non steroidal anti-inflammatory drugs, fluoroquinolones, intra-articular injections of corticosteroids, and other drugs.

p53 expression and DNA ploidy of cartilage lesions.

The aim of this study was to investigate the expression of a tumor suppressor gene (p53) in cartilage lesions of bone and its relationship to their histological grade and DNA ploidy. An immunohistochemical assay for p53 and Feulgen-stained DNA preparations was subjected to computerized image analysis. Enchondromas, synovial chondromatosis, and low grade (grade I and II) chondrosarcomas were diploid. High grade (grade III) chondrosarcomas and high grade sarcomatous components of dedifferentiated chondrosarcomas were aneuploid. Well differentiated cartilaginous components of dedifferentiated cho…

Synovial type (group II) phospholipase A2 in cartilage.

Phospholipase A2 (PLA2) was produced in E. coli by a recombinant technique using a synthetic gene coding for PLA2 found in synovial fluid (SF) of patients suffering from rheumatoid arthritis (RA). Polyclonal antibodies were produced against the recombinant synovial type PLA2 (syn-PLA2) in a rabbit. The IgG fraction of the antiserum was isolated and the specificity was tested by immunoblotting. The antiserum detected a protein with an apparent molecular weight of 15 kDa in extracts of cartilage, but did not react with PLA2 isolated from human pancreas or ascitic fluid. In immunohistochemistry, …

Homozygous achondroplasia: morphologic and biochemical study of cartilage.

We have performed histochemical, immunohistochemical, electron microscopic, and biochemical studies on the upper tibial cartilage from a case of homozygous achondroplasia. The growth zone was narrow and disorganized. Columnization was absent except for a few areas with short rows of cells. Hypertrophy was reduced to scattered clusters of cells. The provisional calcification was patchy and primary trabeculae were thick and irregularly arranged. Islands of fibrous or fibrocartilagineous tissue were found along the growth zone. The matrix did not stain with safranin O and lacked metachromasia, ex…

Biochemical analysis of normal and osteoarthritic human cartilage.

Non-collagenous proteins from the articular cartilage of normal subjects and patients with degenerative joint disease were extracted sequentially. Proteoglycans and the other glycoproteins were more extractable from the osteoarthritic cartilage at lower ionic strength than those from the normal cartilage. A 50-kD protein which seems specific to osteoarthritic cartilage was identified. Three different populations of proteoglycans were purified from normal and only two from osteoarthritic cartilage. Moreover, greater amounts of albumin and fibronectin were found in the pathological cartilage. No…

Extended and globular protein domains in cartilage proteoglycans.

Electron microscopy after rotary shadowing and negative staining of the large chondroitin sulphate proteoglycan from rat chondrosarcoma, bovine nasal cartilage and pig laryngeal cartilage demonstrated a unique multidomain structure for the protein core. A main characteristic is a pair of globular domains (diameter 6-8 nm), one of which forms the N-terminal hyaluronate-binding region. They are connected by a 25 nm-long rod-like domain of limited flexibility. This segment is continued by a 280 nm-long polypeptide strand containing most chondroitin sulphate chains (average length 40 nm) in a brus…

Alteration of cartilage matrix morphology with histological processing.

An interlacunar network in the extracellular matrix of femoral head articular cartilage of neonatal rats was seen by light microscopy to: (1) consist of elements, 0.5 micron thick, which occurred as individual elements, as bundles of elements, and as fused elements, (2) stain intensely with toluidine blue, methylene blue, and safranin O, and (3) connect chondrocytes by inserting on the chondrocyte capsules which were composed of morphologically and cytochemically similar material. By electron microscopy, the single elements were seen to be composed of thicker, denser staining areas of the hone…

[Patella dislocation with special reference to cartilage damage].

Dislocations of the patella are discussed in the light of their pathogenesis, their possible injury pattern and their consequences. An exemplary case is presented. Especially the injury to the retropatellar cartilage with following chondromalacia--sometimes initiating the development of patellofemoral osteoarthritis--renders the dislocation of the patella a severe injury of the knee region. In case of spontaneous reduction and uncertain history the diagnosis may be difficult. The dislocation of the patella can be suspected if a hemarthrosis is found without an instability of the knee ligaments…

Metabolic responses of cartilage in experimentally induced osteoarthritis.

Serial metabolic responses in developing osteoarthritis induced in the right knees of a rabbit model of partial meniscectomy (PM) were studied. Controls were sham-operated (SH) right knees, left knees of all operated animals, nd right and left knees of a nonoperated series. Glycosaminoglycan and protein synthesis and cell replication were separately analysed utilising 35 SO4, 14C-Glycine, and 3H-thymidine, respectively. Pitting, ulceration, and osteophytes, seen only in the PM knees, increased over the 12-week period of study. 3H-thymidine and 14C-glycine incorporations by PM cartilage were in…

High resolution scanning electron microscopy of elastic cartilage.

The elastic cartilage of the rat epiglottis was studied with a high-resolution SEM technique. The chondrocytes were found to be anchored in their lacunae by fibrils running in from the territorial matrix. This matrix exhibited a dense network of fibrils arranged tangentially around the lacunar cavity. The fibrils of the inter-territorial matrix however, formed a three dimensional network of sheets with interconnecting fibrils. The SEM has shown up for the first time a substructure in the fibrils in the form of circular protuberances arranged circumferentially around the fibrils and forming rid…

Collagen synthesis in normal and osteoarthritic human cartilage.

Collagen metabolism in osteoarthritic human articular cartilage was compared to that in normal cartilage and was also correlated with the degree of severity of the osteoarthritic lesion as determined by a histological-histochemical grading system. No correlation was apparent between the concentrations of DNA, hydroxyproline, and hydroxylysine and the degree of severity of the osteoarthritic lesion (except in far-advanced lesions). Similarly, there was no correlation in levels of these components in tissues from the normal vs. osteoarthritic group. The similarity of the values of the ratio hydr…

Cartilage in hemophilic arthropathy. Ultrastructural and microanalytical studies.

The presence of iron in articular cartilage was investigated in five human and two canine cases of factor VIII-deficiency hemophilic arthropathy. The lesions were mild in three cases. Advanced destruction of the cartilage was present in four cases, in one of which sufficient cartilage was preserved to permit recognition of hemosiderin in chondrocytes. The Fe was present in the form of siderosomes and ferritin granules free in the cytoplasm. Iron and phosphorus were demonstrated in the siderosomes by energy dispersion analysis of x-rays; but no Fe was identified by this means or by light or tra…

Properties of inorganic pyrophosphatase of pig scapula cartilage.

The properties of a highly purified inorganic pyrophosphatase (pyrophosphate phosphohydrolase; EC 3.6.1.1) from pig scapula cartilage were studied. The enzyme had a molecular weight of 66 000 and a pH optimum of 7-8. It was markedly activated by magnesium, but not, or only to a much smaller degree, by other metal ions. PP1 was the only substrate found and had a Km value of 11 muM. The enzyme was not inhibited by phosphate and other inhibitors of alkaline phosphatase such as CN- minus, amino acids and theophylline; it was slightly inhibited by tartrate, formaldehyde and ammonium molybdate and s…
