Injuries to pathologically changed cervical vertebrae.

Three cases of fatal injuries to the upper cervical spinal cord and underlying cervical spine pathology are presented. It is stressed that the slightest suspicion concerning injuries to the upper part of the neck must lead not only to microscopic examination of the cervical spinal cord and medulla oblongata, but also to careful preparation (or maceration) of the upper cervical spine, in order not to overlook fractures or misinterpret old fractures and diseases as fresh fractures.

Sleep attacks--apparent relationship to atlantoaxial dislocation.

Sleep attacks, while being characteristic of narcolepsy, have been reported in several metabolic and central nervous system disorders--most of which are refractory to treatment. We have recently treated a 47-year-old man with disabling sleep attacks who was found to have atlantoaxial dislocation. Despite narrowing of the sagittal cervical canal diameter to 12 mm (behind the dens), symptoms of spinal cord compression were mild and intermittent, while signs were absent. Posterior fusion of the C-1 and C-2 vertebrae resulted in relief of symptoms. We suggest that this sleep disorder was a functio

Appropriateness of Cervical Magnetic Resonance Imaging in the Evaluation and Management of C1 Jefferson Fractures.

BACKGROUND: Traditionally, C1 fractures have been designated as stable or unstable based on the inherent integrity of the transverse altantal ligament. The purpose of our study was to identify adult trauma patients with C1 fractures on cervical computed tomography and evaluate whether C1-C2 alignment differed in patients with and without an associated transverse atlantal ligament injury on follow-up cervical magnetic resonance imaging. METHODS: Adult trauma patients who suffered a C1 fracture were identified retrospectively. The cervical computed tomography examinations for these patients wer

Management of Unique Basilar Invagination Combined with C1 Prolapsing into the Foramen Magnum in Children: Report of 2 Cases.

BACKGROUND: Basilar invagination (BI) is a developmental anomaly and commonly presents with neurologic findings. The incidence of BI associated with other osseous anomalies of the craniovertebral junction is high, including incomplete ring of C1 with spreading of the lateral masses, atlanto-occipital assimilation, hypoplasia of the atlas, basiocciput hypoplasia, and occipital condylar hypoplasia. However, BI combined with C1 prolapsing into the foramen magnum (FM) is an extremely rare condition. CASE DESCRIPTION: A previously healthy 6-year-old girl (case 1) presented with extremities numbnes

[Feasibility of screw placement on posterior occipital condyle assisted by occipital tangent angulation].

Objective: To investigate the practicability of occipital tangent angle in assisting posterior occipital condylar screw placement and to verify the safety and accuracy of self-made screw placement device for auxiliary screw. Methods: Occipito-cervical region specimens of 12 adult corpses were selected and scanned by thin-cut CT examination. The three-dimensional reconstruction of occipital and atlas was simulated for each specimen, and 3-matic software was used to measure the setting parameters, including occipital tangent angle, head obliquity, internal inclination angle, length of the screw a

Vincenzo Quercioli (1876-1939), researcher and pioneer of the atlas fracture.

A review of early 20th century literature regarding fractures of the atlas led the authors to discover a paper written in Italian by Professor Vincenzo Quercioli in 1908, at that time an assistant surgeon at the University of Siena. The work was published in the journal Il Policlinico, which at that time was directed by Professor Francesco Durante. The paper described the first case of a quadripartite fracture of the atlas, and it accurately reported the mechanism of injury, symptoms, neurological examination, treatment, complications, and cause of death of the patient. Quercioli performed an

Direct repair of displaced anterior arch fracture of the atlas under microendoscopy: experience with seven patients.

INTRODUCTION: In general, bony injuries heal well with immobilization when the fractured segments are well apposed in the cases of stable atlas fracture. Osseous nonunion of the displaced anterior arch fracture of the atlas has been reported in the literature. However, there have been no reports published on the treatment of nonunion of the atlas fractures. OBJECTIVE: The objective of this study is to describe a new technique for direct repair of the displaced anterior arch fracture of the atlas in a minimally invasive manner. METHODS: Seven patients with the atlas fracture were treated by a

[X-ray and CT morphology of atlas variants in the dog].

To investigate variations in atlas shape in dogs the image archives of the Department for Small Animals and Horses, University of Vienna, and The Royal Veterinary College, University of London were searched for radiographs and CT images of the canine atlas. 32 radiographic and 78 CT studies, including 51 toy and 54 large breed dogs, were retrieved and analysed. Five dogs had both radiographs and CT scans. Patients with pathologic alterations, such as fractures and incomplete ossification of the atlas, as well as oblique projections were excluded. Compared to large breed dogs (body weight more

Persistent first intersegmental vertebral artery in association with type II odontoid fracture: surgical treatment utilizing a novel C1 posterior arch screw: case report.

OBJECTIVE AND IMPORTANCE: Posterior cervical stabilization for cervical fractures is common, and numerous techniques for fixation have been described. This case describes the novel usage of C1 laminar screws due to a persistent intersegmental artery and congenital fusion of C2-C3. CLINICAL PRESENTATION: A 64-year-old woman presented with loss of consciousness after falling down a flight of stairs. Initial CT scan showed a type II odontoid fracture with significant malalignment, as well as an anomalous congenital fusion of C2-C3 and degenerative spondylolisthesis of C3 to C4. CT angiogram demo

Anatomy and prevalence of atlas vertebrae bridges in a Kenyan population: An osteological study.

Atlas bridges are bony outgrowths over the third segment of the vertebral artery. They may exist as incipient, incomplete, or complete bridges, converting the groove into a deep sulcus, incomplete, or complete foramen respectively. These bridges and their associated foramina display bilateral and sex differences in their prevalence and type. Occurrence of these bridges may predispose to vertebrobasilar insufficiency and Barre-Lieou syndrome. The coexistence of lateral and posterior bridges as well as side predilection is not clear in either sex. Their relative occurrence may also point to some

Head-supporting sign during reclining: an indication of craniovertebral junction involvement.

OBJECTIVES: The aim of our study was to evaluate disease courses and outcomes of sJIA children undergoing tocilizumab (TCZ) treatment, and to establish the predictors which distinguish inactive disease and disease flares. METHODS: Our retrospective study included 48 active sJIA children who were refractory to different anti-rheumatic drugs and who were then started on TCZ. The effectiveness of TCZ was assessed by the changes of sJIA attributed signs and symptoms and the remission was judged according to the Wallace (2004) criteria. RESULTS: The main demographic parameters (Me; IQR) were show

Biomechanical comparison of unicortical versus bicortical C1 lateral mass screw fixation.

STUDY DESIGN: Biomechanical study of pullout strength of unicortical versus bicortical C1 lateral mass screws using a cadaveric cervical spine model. OBJECTIVE: To compare pullout strength of unicortical versus bicortical C1 lateral mass screws. SUMMARY OF BACKGROUND DATA: The internal carotid artery and hypoglossal nerve lie over the anterior aspect of the lateral mass of the atlas and are at risk from bicortical C1 lateral mass screws. Unicortical screws would reduce the risk of injury to these neurovascular structures; however, no data are available on the relative strength of unicortical
