Classic metaphyseal lesion acquired during physical therapy.

A classic metaphyseal lesion (CML) is highly specific for nonaccidental trauma. Missing CMLs can be devastating to the child as the child can continue to be exposed to inflicted trauma. Yet, there are rare case reports on CMLs that occur due to birth trauma, IV line placement, and treatment for clubfoot. We present a case of a CML in the tibia that occurred in the hospital secondary to physical therapy, that also caused a femoral shaft fracture, in a term child with hypertonic lower extremities secondary to myelomeningocele. Radiologists, as well as child abuse pediatricians, should be aware o

Dissemination of child abuse clinical decision support: Moving beyond a single electronic health record.

BACKGROUND: Child maltreatment is a leading cause of pediatric morbidity and mortality. We previously reported on development and implementation of a child abuse clinical decision support system (CA-CDSS) in the Cerner electronic health record (EHR). Our objective was to develop a CA-CDSS in two different EHRs. METHODS: Using the CA-CDSS in Cerner as a template, CA-CDSSs were developed for use in four hospitals in the Northwell Health system who use Allscripts and two hospitals in the University of Wisconsin health system who use Epic. Each system had a combination of triggers, alerts and chi

'If not through play, I can't imagine how they would cope': The therapeutic potential of play for hospitalised children, their caregivers, and healthcare institutions.

PURPOSE: To explore the therapeutic potential of hospital play for children, their caregivers, and health professionals. DESIGN AND METHOD: This exploratory qualitative study recruited 34 healthcare professionals from four hospitals in different regions of Chile through purposive sampling. Data were collected through semi-structured interviews and analysed using reflective thematic analysis. RESULTS: Play helps children adapt to the hospital environment, reducing their stress and mitigating the risk of medical trauma. It also offers relief to families in distress and assists them in coping w

Massage has no observable effect on distress in children with burns: A randomized, observer-blinded trial.

IMPORTANCE: In a previous observational study we found that massage therapy reduced anxiety and stress in pediatric burn patients. We aimed to test this effect in a randomized controlled trial. OBJECTIVE: To determine whether (1) aromatherapy massage can provide relaxation to hospitalized children with burns; (2) massage with aromatherapy oil is more effective than without; and (3) massage sessions are more effective when repeated. DESIGN, SETTING, AND PARTICIPANTS: Randomized controlled clinical trial with 3 arms conducted in a burns unit from April 2013 to December 2014 in Cape Town, South

Separation and psychological trauma: a paradox examined.

This paper explores the separation of children from their parents that took place in British hospitals in the middle years of the 20th century (Bradley 2001). It is widely believed that this child-parent separation was the product of a failure to understand the child's psychological and social needs (Bowlby et al 1939, Brain and Maclay 1968, Branstetter 1966, Editorial 1957). Paediatric nurses have been blamed for this ignorance and the resulting failure of care (Bowlby et al 1939). However, evidence suggests that such accusations are flawed in logic and in fact: paediatric nurses of the time

Non-accidental childhood injury presenting at a hospital casualty department.

One hundred children who attended a Sydney Paediatric Casualty Department with injuries, burns or ingestions of harmful substances were divided into two groups. One group (30 children) consisted of cases which were considered to be those of child abuse or neglect (CAN). In the other group (70 children) there was no suspicion of child abuse or neglect. This was regarded as the "simple accident" (SA) group. The 100 families were visited at home and interviewed about their child's health, child rearing practices, parental expectations for the child, and socioeconomic status. Factors which had a s

[Nutritional Evaluation of Hospitalized Patients with Hepatic Cirrhosis and the Impact on the Prognosis of the Disease: a Cross-Sectional Study].

INTRODUCTION: Nutritional assessment in patients with cirrhosis is complex; the application of the Royal Free Hospital Global Assessment (RFH-GA) in this population is recommended. On the other hand, sarcopenia represents an independent risk factor for mortality, and it is hard to measure in clinical practice. Currently, it is recommended to evaluate muscle strength as a predictor of probable sarcopenia. OBJECTIVES: To describe the nutritional status by using the RFH-GA and muscle strength; its association with Child-Pugh and complications of the disease in hospitalized patients with hepatic

Using geo-spatial analysis for assessing the risk of hospital admissions due to community-acquired pneumonia in under-5 children and its association with socially vulnerable areas (Brazil).

BACKGROUND: The concentration of under-5 child morbidity and mortality due to pneumonia in developing countries reflects the social inequities. This study aimed to map and assess the spatial risk for hospitalization due to Community-Acquired Pneumonia in children under 5 years of age and its association with vulnerable areas. METHODS: Ecological study in the city of Ribeirão Preto, state of São Paulo, Brazil. The study population consisted of hospitalized under-5 children, diagnosed with community-acquired pneumonia, in Ribeirão Preto-São Paulo-Brazil, from 2012 to 2013. Data were collected i

Health literacy and disease-specific knowledge of caregivers for children with sickle cell disease.

This study was conducted to measure the health literacy (HL) and disease-specific knowledge (DSK) of caregivers for children with sickle cell disease (SCD) and relate them to their child's health care utilization. The authors conducted a cross-sectional study of caregiver-child dyads attending an urban pediatric sickle cell clinic. Caregivers were administered the Shortened Test of Functional Health Literacy (S-TOFHLA) and a locally developed DSK questionnaire. Retrospective review of the child's electronic medical record (EMR) was performed to determine annual emergency department (ED) visits

"We're teetering on unsteady ground" parents' experiences of accessing 24/7 paediatric end-of-life care: a qualitative study.

BACKGROUND: Providing high quality around-the-clock care, is key to supporting families in their preferred place of care. Changing symptoms and parents' distress cannot wait for 'opening hours'. Yet in the UK, 24/7 children's end-of-life care remains a significant postcode lottery. To inform equitable service development this study explored parents' experiences accessing 24/7 paediatric palliative care, their expectations and needs. METHODS: Qualitative study using in-depth interviews, analysed using thematic analysis. Parents in one region of England, were eligible if their child had a life-

Minimally invasive devascularization for variceal bleeding that could not be controlled with sclerotherapy.

The appropriate therapy for continued bleeding despite sclerotherapy remains controversial. This study evaluates a devascularization procedure performed without the risks of major surgery and general anesthesia. Fifty consecutive patients, each with an endoscopically proven variceal hemorrhage that was uncontrollable with sclerotherapy, were treated with minimally invasive devascularization. The procedure was performed in three stages. First, the portal pressure was sharply reduced by angiographic embolization of the midsplenic artery. Then the esophagogastric variceal network was thrombosed b

Fulminant meningococcal sepsis in a young child--a case report.

We are presenting a case of isolated fulminant meningococcal sepsis with two and a half year old child. Initial symptoms were obscure and common to many medical conditions, but also previously described as symptoms of meningococcal sepsis. Unrecognizing the seriousness of the condition child died at home, within few hours after examination and discharge from the hospital. Autopsy and microbiological findings unquestionably proved that the child died from septic shock caused by fulminant meningococcal sepsis.

The family house-A safe haven: A qualitative study of families' experiences staying in a hospital family house during their children's hospitalisation.

AIMS: To gain knowledge about families' experiences while staying in a hospital family house during their child's hospitalisation. BACKGROUND: When a child is hospitalised with a chronic, serious or life-threatening disease, the entire family is stressed as normal everyday life is disrupted. In Denmark, accompanying the ill child to the paediatric wards is often possible only for one of the parents. DESIGN: This qualitative study takes a phenomenological-hermeneutical approach, exploring the lived experience of families' everyday life in a hospital family house. METHOD: The study comprised

Heavy prenatal alcohol exposure and healthcare use during childhood and adolescence: a Danish nationwide cohort study 1997-2022.

Children with heavy prenatal alcohol exposure may experience increased healthcare needs and access barriers. We aimed to quantify their healthcare utilisation patterns compared to all other Danish children in this nationwide cohort study of children born 1997-2020, followed until Dec 31, 2022. Data was derived from eight national registers. Heavy prenatal alcohol exposure was defined by hospital contacts with 100% alcohol-attributable diagnoses given to mother or child, redeemed prescriptions for drugs to treat alcohol dependence, or enrollment into treatment clinics, one year before or during

Etiology of Diarrhea Among Hospitalized Children in Blantyre, Malawi, Following Rotavirus Vaccine Introduction: A Case-Control Study.

Despite rotavirus vaccination, diarrhea remains a leading cause of child mortality. We collected stool specimens from 684 children <5 years of age hospitalized with diarrhea (cases) and 527 asymptomatic community controls for 4 years after rotavirus vaccine introduction in Malawi. Specimens were tested for 29 pathogens, using polymerase chain reaction analysis. Three or more pathogens were detected in 71% of cases and 48% of controls. Pathogens significantly associated with diarrhea included rotavirus (in 34.7% of cases and 1.5% of controls), enteric adenovirus (in 29.1% and 2.7%, respectively
