Evaluating the structure of the Patient Assessment of Chronic Illness Care (PACIC) survey from the patient's perspective.

BACKGROUND: The Patient Assessment of Chronic Illness Care (PACIC) survey is a widely used instrument to assess the patient experience with healthcare delivery. PURPOSE: This study aims to evaluate the factorial structure of PACIC from the patient perspective. METHODS: A postal survey was mailed to 4,796 randomly selected adults with diabetes from 34 primary care clinics. Internal consistencies of PACIC subscales were assessed by Cronhach's α. Factorial structure was evaluated by confirmatory and exploratory factor analyses. RESULTS: Based on responses of 2,055 patients (43% response rate),

Predictors of progression through the cascade of care to a cure for hepatitis C patients using decision trees and random forests.

BACKGROUND: This study uses machine learning techniques to identify sociodemographic and clinical predictors of progression through the hepatitis C (HCV) cascade of care for patients in the 1945-1965 birth cohort in the Southern United States. METHODS: We compared sociodemographic and clinical variables between groups of patients for three care outcomes: linkage to care, initiation of antiviral treatment, and virologic cure. A decision tree model and random forest model were built for each outcome. RESULTS: Patients were primarily male, African American/Black or Caucasian/White, non-Hispanic

Multilayered Polysaccharide Nanofilms for Controlled Delivery of Pentoxifylline and Possible Treatment of Chronic Venous Ulceration.

Local drug delivery systems made from nontoxic polysaccharide nanofilms have an enormous potential in wound care. A detailed understanding of the structural, surface, physicochemical, and cytotoxic properties of such systems is crucial to design clinically efficacious materials. Herein, we fabricated polysaccharide-based nanofilms onto either a 2D model (SiO2 and Au sensors) or on nonwoven alginate 3D substrates using an alternating assembly of N,N,N-trimethylchitosan (TMC) and alginic acid (ALG) by a spin-assisted layer-by-layer (LbL) technique. These TMC/ALG multilayered nanofilms are used f

Feasibility of an App-Supported Monitoring System for Cognitive-Behavioral Interventions in Fibromyalgia Patients in Public Settings.

Fibromyalgia (FM) is a chronic condition marked by widespread pain, psychological distress, and significant societal costs. This study evaluates the feasibility of a mobile health (mHealth) application designed to support cognitive-behavioral therapy for FM by enabling real-time tracking of patient-reported outcomes and adaptive treatment modifications within public health care settings. Thirty-six adults with FM participated in this feasibility study, which assessed the app's usability, acceptability, and clinical utility for both patients and therapists. Results demonstrated high usability r

Comorbidities as treatable traits of chronic airway diseases.

Chronic airway diseases, including asthma, chronic obstructive pulmonary disease, and bronchiectasis, are increasingly recognized as heterogeneous conditions influenced not only by airway pathology but also by a wide range of extrapulmonary and behavioral comorbidities. The treatable traits (TT) model, as it has emerged in recent medical literature, offers a precision medicine framework that redefines comorbidities as clinically relevant, identifiable, and modifiable traits. This paradigm shifts the focus from conventional disease labels to a multidimensional approach that considers the indivi

Organizing a Rational Approach to Treatments of Disorders of Consciousness Using the Anterior Forebrain Mesocircuit Model.

Organizing a rational treatment strategy for patients with multifocal structural brain injuries and disorders of consciousness (DOC) is an important and challenging clinical goal. Among potential clinical end points, restoring elements of communication to DOC patients can support improved patient care, caregiver satisfaction, and patients' quality of life. Over the past decade, several studies have considered the use of the anterior forebrain mesocircuit model to approach this problem because this model proposes a supervening circuit-level impairment arising across DOC of varying etiologies. Integrated Alcohol Use Disorder and Liver Disease Management.

Alcohol-associated liver disease (ALD) is a leading cause of liver disease worldwide, caused by hazardous alcohol use. Many patients with ALD also have alcohol use disorder (AUD), a chronic mental health condition characterized by a cluster of behavioral, cognitive, and physiological symptoms that indicate continued alcohol use despite significant alcohol-related problems. Comprehensive care of ALD often requires treatment of AUD, and evidence has demonstrated that treating the latter improves patient outcomes. However, only a minority of patients with AUD/ALD receive treatment. Integrative ca

Perceptions and attitudes toward obesity care among nursing professionals.

BACKGROUND: The understanding of obesity is evolving from a perceived lifestyle-related condition to a chronic, biologically driven disease. Perception of obesity as a disease among health care professionals is important for effective evidence-based obesity care. Nurses have the most sustained patient contact and play an important role in obesity prevention and management, yet their role in this context remains underexplored. We therefore assessed the perceptions and attitudes towards obesity and obesity care in a Slovenian nursing care cohort. METHODS: Non-interventional, descriptive, cross-

Economic impact of limb-salvage strategies in chronic limb-threatening ischaemia: modelling and budget impact study based on national registry data.

BACKGROUND: Missed opportunities to reduce numbers of primary major lower-limb amputation and increase limb-salvage procedures when treating chronic limb-threatening ischaemia have previously been identified in the literature. However, the potential economic savings for healthcare providers when salvaging a chronic limb-threatening ischaemia-affected limb have not been well documented. METHODS: A model using National Health Service healthcare usage and cost data for 1.6 million individuals and averaged numbers of primary surgical procedures for chronic limb-threatening ischaemia from England

Transrectal osteopathic manipulation treatment for chronic coccydynia: feasibility, acceptability and patient-oriented outcomes in a quality improvement project.

CONTEXT: Pain of the coccyx, coccydynia, is a common condition with a substantial impact on the quality of life. Although most cases resolve with conservative care, 10 % become chronic and are more debilitating. Treatment for chronic coccydynia is limited; surgery is not definitive. Osteopathic manipulative treatment (OMT) is the application of manually guided forces to areas of somatic dysfunction to improve physiologic function and support homeostasis including for coccydynia, but its use as a transrectal procedure for coccydynia in a primary care clinic setting is not well documented. What matters to people with chronic conditions when accessing care in Australian general practice? A qualitative study of patient, carer, and provider perspectives.

BACKGROUND: Research underpinning the patient experience of people with chronic conditions in Australian general practice is not well developed. We aimed to ascertain the perspectives of key stakeholders on aspects of patient experience, more specifically with regards to accessing general practice in Australia. METHODS: Using a qualitative design, semi-structured interviews were conducted by telephone and face-to-face with people living with one or more chronic conditions, informal carers, and primary care providers between October 2016 and October 2017. Participants were recruited and select

The Patient Experience of Acute and Post-Acute Care for Mild Traumatic Brain Injury: A Qualitative Phenomenological Study Using Focus Groups.

Follow-up care for traumatic brain injury (TBI) in the United States ranges from inconsistent, at best, to non-existent most predominantly. The present study aimed to gather patient perspectives on acute and post-acute TBI care using a phenomenological approach guided by the Patient Experience Framework, Chronic Care Model, and Health Literacy Framework. Using a pre-defined, semi-structured interview guide, including four groups of 35 diverse people diagnosed by a medical provider with "mild" TBI within 24 months, the study qualitatively explored their patient care experience, the care they re…

When Paying for Care Becomes a Crisis: Affordability Barriers, Coping Strategies, and Financial Consequences of Chronic Illness in Lagos, Nigeria.

BACKGROUND: Chronic non-communicable disease (NCD) care in Nigeria remains largely financed out of pocket, exposing households to affordability barriers and harmful coping strategies. This study examined the prevalence and patterning of affordability barriers, financial coping strategies, and predictors of foregone care and distress financing among chronic NCD patients attending public health facilities in Lagos, Nigeria. METHODS: A facility-based cross-sectional survey was conducted among 480 adults receiving outpatient care for chronic NCDs, such as hypertension, type 2 diabetes, cardiovasc

Protocol for a randomised trial testing a community fibrosis assessment service for patients with suspected non-alcoholic fatty liver disease: LOCal assessment and triage evaluation of non-alcoholic fatty liver disease (LOCATE-NAFLD).

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is the most common type of chronic liver disease in Australia and its recent increase mirrors the obesity and type 2 diabetes epidemics. Currently, many patients who present to primary care with abnormal liver function tests or steatosis on liver ultrasound are referred for assessment in secondary care. Due to the large number of patients with NAFLD, this results in long waits for clinical and fibrosis assessment, placing unnecessary burden on the public hospital system. METHODS: We will conduct a 1:1 parallel randomised trial to compare t

Prospective study on time-to-tertiary care in alcohol-associated hepatitis: space-time coordinates as prognostic tool and therapeutic target.

BACKGROUND AND AIMS: Alcohol-associated hepatitis (AH) frequently triggers acute decompensation (AD) in cirrhosis, with severe AH linked to high short-term mortality, especially in acute-on-chronic liver failure. Current corticosteroid treatments have limited efficacy, highlighting the need for new therapies. We hypothesized that severe AH outcomes are influenced by early specialized care; thus, we examined the impact of time-to-tertiary care (TTTc). METHODS: Adults with cirrhosis or advanced chronic liver disease were enrolled (RH7, NCT04767945). AH was diagnosed using National Institute on

Defining pain trajectories in juvenile arthritis and associated patient characteristics.

BACKGROUND: Juvenile idiopathic arthritis (JIA) impacts up to 400 per 100,000 children, with a substantial number experiencing chronic pain. Despite its prevalence, research and treatment targeting chronic pain in JIA are limited. This study aims to model pain scores as latent pain trajectories (LPT) and explore the association between LPT and clinical and biological characteristics of patients. METHODS: Data collected from the Electronic Health Records (EHR) included patient characteristics (e.g. age and biological sex), disease activity measures such as Active Joint Count (AJC) and Physicia

Acceptance and Commitment Therapy for Addressing Chronic Cancer Pain: A Narrative Review with Considerations for Utilization in Primary Care.

BACKGROUND: Chronic pain is common among cancer survivors and may reflect interacting physical, psychological, social, and existential factors. Acceptance and Commitment Therapy (ACT) is a behavioral intervention that emphasizes psychological flexibility, acceptance, present-moment awareness, and values-guided action. As cancer survivorship care increasingly involves primary care, scalable behavioral approaches may be needed to complement medical pain management, particularly where specialty behavioral health resources are limited. METHODS: This narrative review examined evidence for ACT in c
