INTRODUCTION: The aim of this study was to verify the hypothesis that the technique of stabilizing the acromioclavicular joint and lateral fractures of the clavicle using a clavicular hook plate can be considered a method of choice in the given types of injury.
MATERIAL AND METHODS: The clinical part of the study included a total of 40 patients. The indication group was formed by patients with type III acromioclavicular joint dislocation, according to Tossy. As for lateral fractures of the clavicle, the patients of group II and types IIa, IIb according to revised Allman and Neers classificati…

Tension band wire and hook plating for the treatment of distal clavicle fractures are each associated with clinical benefits and complications. In this study, we compared outcomes of 2 treatments for short-term distal clavicle fracture management for patients asking for faster improvement. From February 2005 to November 2008, 65 patients with distal clavicle fractures randomly received surgical treatment with tension band wires (n=30) or hook plates (n=35). Postoperative data including complications, radiographic union, sonographic findings and range of motion, and Oxford shoulder score at 3 a…

This is the case of a 46-year-old man who presented 3 months following a fracture of the middle third of the clavicle with an expanding mass in the supraclavicular fossa. Upon admission, a pulsatile mass was identified and X-rays showed delayed union of the fracture. Seven days after admission, skin necrosis developed. Arteriography performed 3 days later demonstrated a round subclavian pseudoaneurysm measuring 5 cm in diameter. It was occluded by means of an endovascular prosthesis. Four days after arteriography, the delayed union was treated by excision of the necrotic skin and internal fixa…

Most series of patients undergoing open reduction and internal fixation (ORIF) of clavicular non-union utilise distant autologous bone graft … at use of distant bone graft is not necessary when performing ORIF for symptomatic non-union of the clavicle with appropriate preparation of fracture ends and adequate fixation.

Stress fractures are relatively uncommon sports injuries and when they do occur, are mostly found in the lower limb. Stress fractures of the clavicle are particularly rare, having been described in a number of non-sport related pathologies, such as nervous tics and post radical neck dissection. In sport, there have only been seven cases reported in the literature. We report on a clavicle stress fracture in a 47-year-old male, partaking in recreational weight lifting activities. This is the first reported case of a non-union stress fracture of the clavicle. The patient underwent an open reducti…

Fractures of the clavicle are common and most often occur in the middle third. The clavicle has several important functions, each of which may be affected after fracture and malunion. In this retrospective study, we reviewed 132 patients with united fractures of the middle third of the clavicle after conservative management. Residual symptoms and overall patient satisfaction after treatment were assessed through a questionnaire. Clavicular shortening after union was calculated on a standardized anteroposterior chest radiograph. Intraobserver variability and interobserver variability of measure…

BACKGROUND: The clinical success of arthroscopic distal clavicle resection for athletes has been well documented. There are, however, no published studies that prospectively compare the recovery rates in athletes as well as the outcomes of the indirect versus direct approaches.
HYPOTHESIS: Both procedures are equally successful; however, the direct approach affords faster return to sports.
STUDY DESIGN: Randomized controlled clinical trial; Level of evidence, 2.
METHODS: Thirty-eight consecutive athletes with osteolysis of the distal clavicle or isolated posttraumatic arthrosis of the acrom…

In a retrospective study, 12 patients with acute acromioclavicular dislocation Tossy stage III were reviewed after operative treatment with a clavicle hook plate. Mean follow-up time was 20 months. Clinical and radiographic results were reviewed. Clinical outcome was superior to the radiographic results. Some questions about this technique remain open.

We report on two children who developed a fixed rotatory subluxation of the atlantoaxial joint due to torticollis being attributed to a fractured clavicle. Appropriate treatment was delayed for weeks to months; only after the fractured clavicle had healed and the torticollis persisted was the problem identified. Persisting rotatory subluxation also predisposes to further anterior displacement of C-1 due to an increased moment arm created by the forward displacement of the center of gravity of the head. The association of atlantoaxial subluxation should always be considered in children presenti…

The cases of 14 adult females with swelling, tenderness, and sclerosis of the inferomedial part of the clavicle are presented. They were seen during a 16-year period, suggesting that the condition is rare. In ten patients strain at the sternoclavicular joint seemed to be an etiological factor. Eleven patients were followed for periods of 1-13 years (mean 5 years). The clavicular sclerosis regressed and they all developed signs of osteoarthrosis. Based upon available biopsy results and review of reported cases, it is hypothesized that the radiographic changes are due to osteonecrosis of the med…

Five consecutive unstable fractures of the distal third of the clavicle were treated by indirect open reduction and internal fixation using a temporary Bosworth-type screw. Coracoclavicular fixation provided and maintained reduction of the fracture. Healing occurred uneventfully within nine weeks in all cases. The screw was removed under local anaesthesia after healing of the fracture and there were no surgical complications. Shoulder function was restored to the pre-injury level. Temporary coracoclavicular screw fixation appears to be a valuable alternative for the treatment of type II fractu…

The cause of pseudarthrosis of the clavicle is obscure. Right-sidedness is an almost constant feature. We have proposed that the lesion is sometimes due to pressure upon the developing clavicle by the subclavian artery which is normally at a higher level on the right side. This may be accentuated in the presence of cervical ribs or unduly elevated first ribs, both of which we have observed in association with pseudarthrosis. We have also noted pseudarthrosis on the left side in association with dextrocardia (when the relative positions of the subclavian arteries are reversed) and in the presen…

CASE: An 80-year-old man with osteoporosis sustained a complex, comminuted medial clavicle (MC) fracture that failed nonoperatively. A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery.
CONCLUSION: Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. Restoring clavicular length through a DO in conjunction with…

BACKGROUND: Minimally invasive treatment of clavicle shaft fractures has become increasingly popular due to reduced soft tissue damage, smaller incisions, lower rates of infections, and minimal scarring. Most commonly described options are titanium elastic nailing and plate osteosynthesis. Clinical studies report promising results with good healing tendencies, particularly in children and adolescents. However, there are still some downsides to consider—titanium elastic nails have been associated with soft-tissue irritation, implant associated discomfort, and notable complications rates includi…

AIMS: Superior plating has long been the routine surgical approach for mid-shaft fractures of the clavicle. However, concerns about the prominence of the plate and neurological risk have led to an increased interest in anteroinferior plating. Whether anteroinferior plating offers comparable or improved outcomes without compromising the stability of fixation has been investigated in several studies. The aim of this meta-analysis was to evaluate the clinical outcomes, complications, and complication-related outcomes following superior compared with anteroinferior plating of mid-shaft fractures o…

BACKGROUND: Kirschner wires (K-wires) remain a simple and cost-effective option for clavicle fixation, especially in resource-limited settings. However, migration of K-wires is a rare but potentially fatal complication that can involve critical structures in the thorax, mediastinum, or neck. Preventive measures such as bending, burying, and early removal are essential but often overlooked. CASE PRESENTATION: A 35-year-old male underwent K-wire fixation for a displaced left clavicle fracture. Three months postoperatively, he presented with progressive shoulder pain and restricted motion. Imagin…

This retrospective study examined the relationship between the clavicle cortical index (CCI) and dual-energy X-ray absorptiometry (DEXA) bone mineral density (BMD). Data from patients undergoing elective spine and hip surgeries were reviewed, focusing on the bilateral clavicles on chest radiographs. Clavicles were divided into distal, middle-distal, middle-proximal, and proximal. Cortical and trabecular thicknesses were measured in these sections, and the CCI was calculated as (cortical + trabecular)/trabecular bone. The Pearson correlation coefficients were used to assess the CCI and BMDs in …
