What is the Relationship of Traumatic Brain Injury to Dementia?

There is a long history linking traumatic brain injury (TBI) with the development of dementia. Despite significant reservations, such as recall bias or concluding causality for TBI, a summary of recent research points to several conclusions on the TBI-dementia relationship. 1) Increasing severity of a single moderate-to-severe TBI increases the risk of subsequent Alzheimer's disease (AD), the most common type of dementia. 2) Repetitive, often subconcussive, mild TBIs increases the risk for chronic traumatic encephalopathy (CTE), a degenerative neuropathology. 3) TBI may be a risk factor for ot

Preschoolers' emotion knowledge: The role of cognitive flexibility and family interactions in financially vulnerable contexts.

A growing number of studies recognize the importance of emotion knowledge for later child social and academic adaptation, but most studies have not considered the various components of emotion knowledge (i.e., receptive, expressive, stereotypical, and nonstereotypical emotion konwledge). This study was to better understand the role of both family interactions and cognitive flexibility on the different components of emotion knowledge in preschoolers in the context of financial insecurity. Family interactions of 85 families with children aged 3-5 were filmed during a home visit. During the same

Physical activity moderates the association between white matter hyperintensity burden and cognitive change.

OBJECTIVE: Greater physical activity (PA) could delay cognitive decline, yet the underlying mechanisms remain unclear. White matter hyperint … al activity may help maintain reasoning, speed, and vocabulary abilities in face of WMH burden. The cognitive reserve potential of PA warrants further examination.

Frontotemporal dysregulation of the SNARE protein interactome is associated with faster cognitive decline in old age.

The molecular underpinnings associated with cognitive reserve remain poorly understood. Because animal models fail to fully recapitulate the complexity of human brain aging, postmortem studies from well-designed cohorts are crucial to unmask mechanisms conferring cognitive resistance against cumulative neuropathologies. We tested the hypothesis that functionality of the SNARE protein interactome might be an important resilience factor preserving cognitive abilities in old age. Cognition was assessed annually in participants from the Rush "Memory and Aging Project" (MAP), a community-dwelling c…

Dispersion-based cognitive intra-individual variability in dementia with Lewy bodies.

OBJECTIVE: Cognitive fluctuations are characteristic of dementia with Lewy bodies (DLB) but challenging to measure. Dispersion-based intra-individual variability (IIV-d) captures neurocognitive performance fluctuations across a test battery and may be sensitive to cognitive fluctuations but has not been studied in DLB. METHOD: We report on 5,976 participants that completed the uniform data set 3.0 neuropsychological battery (UDS3NB). IIV-d was calculated via the intra-individual standard deviation across 12 primary UDS3NB indicators. Separate models using mean USD3NB score and the Montreal co

Change in self-reported cognitive symptoms after mild traumatic brain injury is associated with changes in emotional and somatic symptoms and not changes in cognitive performance.

OBJECTIVE: To investigate (a) whether self-reported cognitive symptoms after mild traumatic brain injury (MTBI) are associated with cognitive test performances, and (b) whether improvement in self-reported symptoms from 2 weeks to 3 months after MTBI is associated with improvement in cognitive test performances. METHOD: Patients with MTBI (n = 135), aged 16-59, who initially presented to the emergency department, completed the Rivermead Post Concussion Symptoms Questionnaire (RPQ), the Brief Symptom Inventory 18, and cognitive tests (i.e., Controlled Oral Word Association, Coding, Rey Auditor

Catastrophizing is associated with excess cognitive symptom reporting after mild traumatic brain injury.

OBJECTIVE: Persistent cognitive symptoms after mild traumatic brain injury (mTBI) often do not correlate with objective neuropsychological performance. Catastrophizing (i.e., excessively negative interpretations of symptoms) may help explain this discrepancy. We hypothesize that symptom catastrophizing will be associated with greater cognitive symptom reporting relative to neuropsychological test performance in people seeking treatment for mTBI. METHOD: Secondary analysis of baseline data from a randomized controlled trial. Adults with mTBI (N = 77) were recruited from two outpatient mTBI cli

Quantitative estimate of cognitive resilience and its medical and genetic associations.

BACKGROUND: We have proposed that cognitive resilience (CR) counteracts brain damage from Alzheimer's disease (AD) or AD-related dementias such that older individuals who harbor neurodegenerative disease burden sufficient to cause dementia remain cognitively normal. However, CR traditionally is considered a binary trait, capturing only the most extreme examples, and is often inconsistently defined. METHODS: This study addressed existing discrepancies and shortcomings of the current CR definition by proposing a framework for defining CR as a continuous variable for each neuropsychological test

Sex-specificities in the association between social network and cognition in older individuals.

Social engagement is thought to support reserve and resilience in aging, yet prior findings suggest it may relate differently, potentially even in opposite directions, in males and females. To further understand these differences, we investigated psychological and biological correlates of social network within each sex, and tested whether its association with cognition differs by sex. Social network was assessed in 104 older adults (age 73.77 ± 3.58, 60 females) using the Social Network Index (network size and number of high-contact roles), alongside perceived social support, perceived stress,

Repetitive mild concussion in subjects with a vulnerable cholinergic system: Lasting cholinergic-attentional impairments in CHT+/- mice.

Previous research emphasized the impact of traumatic brain injury on cholinergic systems and associated cognitive functions. Here we addressed the converse question: Because of the available evidence indicating cognitive and neuronal vulnerabilities in humans expressing low-capacity cholinergic systems or with declining cholinergic systems, do injuries cause more severe cognitive decline in such subjects, and what cholinergic mechanisms contribute to such vulnerability? Using mice heterozygous for the choline transporter (CHT+/- mice) as a model for a limited cholinergic capacity, we investiga

Asymptomatic carotid stenosis is associated with cognitive impairment.

BACKGROUND: Cerebrovascular risk factors (eg, hypertension, coronary artery disease) and stroke can lead to vascular cognitive impairment. The Asymptomatic Carotid Stenosis and Cognitive Function study evaluated the isolated impact of asymptomatic carotid stenosis (no prior ipsilateral or contralateral stroke or transient ischemic attack) on cognitive function. Cerebrovascular hemodynamic and carotid plaque characteristics were analyzed to elucidate potential mechanisms affecting cognition. METHODS: There were 82 patients with ≥50% asymptomatic carotid stenosis and 62 controls without stenosi

Goal-Directed Care: 10 Tips for Resuscitating Older Adults in the Emergency Department.

The resuscitation of older adults in the emergency department (ED) presents unique challenges due to physiological changes with aging, loss of physiologic reserve, polypharmacy, multimorbidity, and often unknown care goals. This review outlines ten high-yield, practical strategies to improve the care of critically ill older adults. We emphasize the importance of a patient-centered, goal-directed approach to care. The article discusses the importance of avoiding cognitive biases, interpreting vital signs in the context of aging, and adapting airway, breathing, and circulatory management techniq

Contributions of neuropathology to understanding schizophrenia in late life.

The neurobiological basis of cognitive and functional deterioration commonly observed in elderly persons with schizophrenia is unclear. Despite superficial similarities in the clinical and neuropsychological profiles of schizophrenia in late life with neurodegenerative dementias, extensive neuropathological investigations have failed to find any evidence of neurodegeneration or neural injury beyond what is typically observed in brains of individuals without neuropsychiatric illness. In contrast, growing neuropathological data indicate aberrant brain development and connectivity in schizophreni

Cognitive functioning in first episode and recurrent major depressive disorder: A systematic review and meta-analysis of comparative performance.

OBJECTIVE: Major depressive disorder is associated with cognitive deficits, yet the specific impact of recurrence remains unclear. This systematic review and meta-analysis compared cognitive performance between individuals with first-episode depression (FED) and recurrent major depressive disorder (rMDD). METHOD: Five databases were searched to identify eligible studies. A series of meta-analyses for each cognitive domain were conducted with inverse-variance, random-effects modeling. Demographic and clinical moderators were explored, with leave-one-out sensitivity analyses. RESULTS: Twenty-f

Using machine learning to derive neurobiological subtypes of general psychopathology in late childhood.

Traditional mental health diagnoses rely on symptom-based classifications. Yet this approach can oversimplify clinical presentations as diagnoses often do not adequately map onto neurobiological features. Alternatively, our study used structural imaging data and a semisupervised machine learning technique, heterogeneity through discriminative analysis, to identify neurobiological subtypes in 9- to 10-year-olds with high psychopathology endorsements (n = 9,027). Our model revealed two stable neurobiological subtypes (adjusted Rand index = 0.38). Subtype 1 showed smaller structural properties, e

Regression-Based Normative Data for Independent and Cognitively Active Spanish Older Adults: Verbal Fluency Tests and Boston Naming Test.

UNLABELLED: An increased cognitive reserve is associated with changes in the pattern of cognitive decline during aging. Thus, normative data adapted to the characteristics of the target population are needed to reduce the possibility of false diagnoses. The aim of this work was to develop normative data for the Phonemic Verbal Fluency test, the Semantic Verbal Fluency test and the Boston Naming Test (BNT). METHOD: Regression-based normative data were calculated from a sample of 118 non-depressed, cognitively active, independent community-dwelling adults aged 55 or older (64.4% women) from SAB…

Adaptive competence impairment and cognitive deficits in acutely ill schizophrenia patients residing in nursing homes.

OBJECTIVES: The loss of long-term inpatient psychiatric hospital beds over the years has caused schizophrenia patients of all ages to be increasingly placed in nursing home environments that were traditionally reserved for elderly patients. Consequently, many nongeriatric patients with chronic schizophrenia are now residing in nursing home settings. The objective of this article is to determine whether many of these nongeriatric patients are placed in nursing homes because of chronicity of illness and severity of impairment, or because of the limited alternative viable housing options. DESIGN
