Laparoscopic versus open surgical closure of perforated duodenal ulcers: a comparative study.

BACKGROUND: Laparoscopic closure of duodenal ulcer perforation may be an alternative to open surgery due to lower morbidity. Most published … cessful closure of perforation by laparoscopy; one had sealed perforation and did not need closure. Conversion to open surgery was necessary in 6 patients (17.8%). Median operating time was 50 minutes (range 25 to 120) and median hospital stay was 4 days (range 4 to 6) for laparoscopy. There was no postoperative leak. Corresponding figures for open surgery were 55 minutes ( …

Feasibility of laparoscopic cholecystectomy for acute cholecystitis beyond 72 h of symptom onset.

Laparoscopic cholecystectomy (LC) performed for acute cholecystitis (AC) is usually advised within 72 h of symptom onset. This study aimed t … identified LLC, CRP ≥10 mg/dL, and failure to create a critical view of safety as risk factors for conversion to open surgery, while operative time ≥90 min and blood loss ≥100 ml were risk factors for complications. LLC is associated with longer operation times, greater blood loss, longer hospital stays and higher conversion rates than ELC. Using a high CRP value as a crit …

Glove-assisted laparoscopic radical nephrectomy: a novel technique.

BACKGROUND: Laparoscopic surgery has not yet met with widespread acceptance due to its degree of technical difficulty. The laparoscopic radical nephrectomy procedure was improved with the aid of an abdominal wall sealing device, a wound retractor, and a surgical glove. METHODS: A 5 cm skin incision was made at the beginning of the operation. The Alexis wound retractor S was set up through this small incision. The wrist portion of the surgical glove (size 8-0) was then used to cover the outer ring of the wound retractor to maintain pneumoperitoneum. The surgeon can use most of the usual surgic

The impact of pre-operative cholecystostomy on laparoscopic excision of choledochal cyst in paediatric patients.

PURPOSE: This aim of this study was to identify the pre-operative risk factors for conversion during laparoscopic excision of choledochal cyst in paediatric patients. METHODS: A retrospective single-centre study was carried out. All paediatric patients (< 18 years) who had undergone laparoscopic excision of choledochal cyst between 2004 and 2021 were reviewed. The outcome was conversion to open surgery and pre-operative factors that affected the conversion rate were analyzed. RESULTS: Sixty-one patients were included. Conversion was required in 24 cases (39.3%). There was no difference in th…

Transanal vs laparoscopic total mesorectal excision for rectal cancer: initial experience from Denmark.

AIM: Laparoscopic total mesorectal excision (LaTME) has improved short-term outcomes of rectal cancer surgery with comparable oncological results to open approach. LaTME can be difficult in the lowermost part of the rectum, leading potentially to higher rates of complications, conversion to open surgery and probably suboptimal oncological quality. Transanal TME (TaTME) can potentially solve these problems. The aim of this study was to compare the short-term results after TaTME with those after LaTME. METHOD: A prospectively collected database of consecutive patients who underwent TaTME was ma…

Fluorescence ureteral navigation during laparoscopic surgery for clinically suspected stage T4 colorectal cancer: A cohort study.

BACKGROUND: Approximately 15% of patients with colorectal cancer present with locally advanced tumors (T4 stage). Laparoscopic surgery for stage T4 disease has not yet been established. The near-infrared ray catheter fluorescent ureteral catheter (NIRFUC) is a new device that uses near-infrared fluorescence resin. We examined the utility of fluorescence ureteral navigation (FUN) with the NIRFUC during laparoscopic surgery for stage T4 colorectal cancer. MATERIALS AND METHODS: Patients with stage T4 colorectal cancer (n = 143, from January 2017 to March 2021) were divided into a T4FUN + group,

Surgical management of gastrointestinal stromal tumors: a single center experience.

INTRODUCTION: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal (GI) tract. Although the therapy targeted at inhibiting tyrosine kinases has shown dramatic results in metastatic and inoperable GISTs, the mainstay of treatment in primary localized forms remains surgical resection. AIM: To provide an overview of our experience of GIST diagnosis and management, with emphasis on comparison of minimally invasive and open surgical resection for primary GISTs. MATERIAL AND METHODS: We retrospectively reviewed the medical records of all patients w

Impact of previous abdominal surgeries on metabolic and bariatric surgery outcomes.

BACKGROUND: Metabolic and bariatric surgery yields substantial weight loss while maintaining a favorable safety profile. The effects of previous abdominal surgery on metabolic and bariatric surgery outcomes remain underexplored. METHODS: This single-center retrospective cohort study examined patients who underwent metabolic and bariatric surgery between January 2008 and December 2023 at a specialized tertiary center. Patients were stratified into 3 groups: (1) no previous abdominal surgery, (2) major previous abdominal surgery, or (3) minor previous abdominal surgery. Specific surgical variab

Laparoscopic versus open hemicolectomy.

AIM: In the last decade, laparoscopic procedures are applied to the treatment of almost all colonic diseases, including both benign and malignant lesions. Focusing our attention to the laparoscopic operative technique, we compare the perioperative results and the oncological outcomes of laparoscopic hemicolectomy with those after open conventional hemicolectomy. METHODS: This prospective non randomized study is based on a series of 469 consecutive patients (73.6% with malignant lesions) operated on by the same surgical team following the same type of surgical technique, for laparoscopic and o

What are the risk factors of conversion from total cholecystectomy to bailout surgery?

BACKGROUND: Laparoscopic cholecystectomy (LC) is regarded as the first choice for patients with gallbladder diseases, but biliary injury (BDI) still poses serious risks upon implementation of LC. Recently, bailout surgery (BOS; partial cholecystectomy or subtotal cholecystectomy) has been proposed to avoid not only BDI but also major vessels injuries. In this retrospective study, we evaluated the preoperative and perioperative risk factors regarding conversion from total cholecystectomy (TC) to BOS. METHODS: A total of 584 patients who underwent elective LC for gallbladder diseases between Ja

The effect of excess body weight on short-term outcomes following minimally invasive rectal cancer surgery: a systematic review and meta-analysis.

BACKGROUND: The converging trends of a global increase in the incidence of colorectal cancer (CRC) and the prevalence of obesity suggest that surgery for CRC in patients with excess body weight (EBW) will become an increasingly common clinical entity. We aimed to evaluate the effect of EBW, defined as being overweight or obese, on short-term perioperative outcomes following minimally invasive rectal cancer surgery (MIRCS). MATERIALS AND METHODS: The review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. PubMed, EMBAS

Nationwide trends in the use of subtotal cholecystectomy for acute cholecystitis.

BACKGROUND: Subtotal cholecystectomy is a viable alternative approach to the proverbial "difficult" gallbladder. To date, only a few studies have observed the establishment of those bail-out procedures as an increasingly common surgical practice. The purpose of this study is to assess nationwide trends of subtotal cholecystectomy through evaluation of operative variables and patient- and institution-level characteristics in procedure preference. METHODS: Data were obtained from the National Inpatient Sample for the years between 2003 and 2014. Patients with acute cholecystitis were categorize

Laparoscopic Splenectomy Versus Open Splenectomy In Massive and Giant Spleens: Should we Update the 2008 EAES Guidelines?

The objective of this study was to derive some useful parameters to define the feasibility of laparoscopic splenectomy (LS) in massive [spleen longitudinal diameter (SLD)>20 cm] and giant spleens (SLD>25 cm). Between December 1996 and May 2017, 175 patients underwent an elective splenectomy. A laparoscopic approach was used in 133 (76%) patients. Massive spleens were treated in 65 (37.1%) patients, of which 24 were treated laparoscopically. In this subset of massive spleens, the results of laparoscopic splenectomy in massive spleens (LSM) and open splenectomy in massive spleens (OSM) were comp
