Survival in Out-of-hospital Rapid Sequence Intubation of Non-Traumatic Brain Pathologies.

INTRODUCTION: Rapid sequence intubation (RSI) is not only used in traumatic brain injuries in the out-of-hospital setting, but also for non-traumatic brain pathologies (NTBP) such as brain tumors, meningitis, encephalitis, hypoxic/anoxic brain injury, stroke, arteriovenous malformations, tumors, aneurysms, brain hemorrhage, as well as brain injury due to diabetes, seizures and toxicity, metabolic conditions, and alcohol and drug overdose. Previous research suggests that RSI is common in non-traumatic coma, but with an unknown prevalence of NTBP in those that receive RSI. If NTBP is common and

Early versus delayed insulin pump therapy in children with newly diagnosed type 1 diabetes: results from the multicentre, prospective diabetes follow-up DPV registry.

BACKGROUND: Although continuous subcutaneous insulin infusion therapy (ie, insulin pump therapy) is associated with improved metabolic control compared with multiple daily insulin injections in children with type 1 diabetes, it is unclear when it is best to start it after diagnosis. In this study, we aimed to compare the outcomes between early and delayed start of insulin pump therapy in young patients with type 1 diabetes. METHODS: We based the current study on data from the multicentre, prospective diabetes follow-up registry (ie, Diabetes-Patienten-Verlaufsdokumentation [DPV]). The DPV reg

Probable tirzepatide-associated limbic encephalitis with status epilepticus in a patient with extreme obesity: a case report.

A 59-year-old man (240 kg, BMI ≈ 100 kg/m2) with type 2 diabetes developed progressive drowsiness, confusion, behavioral changes (irritability and aggressive behavior), coma and status epilepticus 72 h after the third weekly dose of tirzepatide 2.5 mg subcutaneously. Brain MRI showed left mesial temporal (hippocampal–amygdalar) swelling and restricted diffusion without contrast enhancement; cerebrospinal fluid was acellular with normal biochemistry and negative multiplex PCR and antibody panel. Seizures were partially controlled with benzodiazepines, levetiracetam and lacosamide. High-dose int

Mortality-Associated Factors in a Traumatic Brain Injury Population in Mexico.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of death and disability, with a rising incidence in recent years. Factors such as age, sex, hypotension, low score on the Glasgow Coma Scale, use of invasive mechanical ventilation and vasopressors, etc., have been associated with mortality caused by TBI. The aim of this study was to identify the clinical and sociodemographic characteristics that influence the mortality or survival of patients with TBI in a tertiary care hospital in Mexico. METHODS: A sample of 94 patients aged 18 years or older, from both sexes, with an admitting diag

[Primary amebic meningoencephalitis: 1st case observed in Madagascar].

The primary amebic meningoencephalitis is an acute suppurative infection that involves both the brain and the meninges. It is caused by Naegleria fowleri and is a very rare and fulminating condition, so far nearly always fatal. We report the first case in the area of Southern part of Indian Ocean that occured in a 7-year old French boy living in Madagascar. It is assumed that the disease was contracted by swimming in warm fresh water in a lake of the Madagascar east coast. Clinical signs began 10-12 days after exposure, associating headache, vomiting and pyrexia (39-40 degrees C). Upon admissi

[Early prognosis of severe cranio-cerebral injuries].

Severe head injuries are still associated with a high mortality (30-60%). Depth and duration of coma are poor prognostic parameters because of therapeutic concepts such as sedation and mechanical ventilation. That is the reason why we looked for prognostic parameters that are independent of therapy and can be obtained during the normal routine at an ICU for head and trauma patients. METHODS. The medical records of 59 patients with severe head injury were studied and analyzed by statistical means. Exclusion criteria were, e.g., thoracic trauma or abdominal injury. All data tested are parameters

Adverse reactions to diuretics.

Diuretics can result in various undesired biochemical changes, such as impotence, skin rashes, nausea, dizziness and lethargy as well as subjective side effects. The side effects are mostly predictable, their effects depending on both the circulatory blood volume and on the transport of water and solute in the renal tubules. Two of the commonest side effects are mild hypovolaemia, when any diuretic is used, and mild hypokalaemia when the non-potassium-sparing diuretics, such as thiazides and frusemide are used. Its occurrence is dose dependent and can be corrected by potassium supplements, but

Lithiumogenic disorders of the thyroid and parathyroid glands as surgical disease.

Little notice has been paid in the surgical literature to problems with psychoeffective lithium, which by interfering with adenylate cyclase affects thyroid and parathyroid function, causing hypercalcemia, hyperparathyroidism, and hypothyroidism. Seven patients with lithiumogenic hyperparathyroidism occurring after years of lithium therapy underwent treatment and manifested osteoporosis (n = 2), hypertension (n = 2), nephrolithiasis (n = 1), coma (n = 1), rising hypercalcemia (n = 1), goitrous myxedema (n = 4), nephrogenic diabetes insipidus (n = 2), renal failure (n = 2), and hyperlipidemia (

A Retrospective Analysis of Children and Adolescents With Diabetic Ketoacidosis in the Intensive Care Unıt: Is It Significant that the Blood Ketone Level Becomes Negative in Diabetic Ketoacidosis?

INTRODUCTION: Diabetic ketoacidosis (DKA) is the most common cause of acute morbidity and mortality in children and adolescents with type 1 diabetes mellitus (T1DM). Because DKA management is associated with complications, endocrine communities have published guidelines and attempted to set standards for DKA diagnosis and management worldwide. In this study, for the patients followed up in the intensive care unit who have been treated according to DKA protocols, clinical and laboratory characteristics, differences between new and old diagnosed patients, and results of treatment were evaluated.

Report of the committee on the classification and diagnostic criteria of diabetes mellitus.

Diabetes mellitus is a group of diseases associated with various metabolic disorders, the main feature of which is chronic hyperglycemia due to insufficient insulin action. Its pathogenesis involves both genetic and environmental factors. The long-term persistence of metabolic disorders can cause susceptibility to specific complications and also foster arteriosclerosis. Diabetes mellitus is associated with a broad range of clinical presentations, from being asymptomatic to ketoacidosis or coma, depending on the degree of metabolic disorder. Diabetic Ketoacidosis Complicated by Cerebral and Pulmonary Oedema in an Adult.

Diabetic ketoacidosis (DKA) is a life-threatening metabolic emergency. While cerebral and pulmonary oedema are recognised complications in paediatric populations, they are exceptionally rare and poorly characterised in adults. This case report describes an adult patient with severe DKA who developed both cerebral and pulmonary oedema during treatment. A previously healthy 36-year-old man presented with a two-week history of polyuria and polydipsia and progressive dyspnoea. He was alert and tachycardic and exhibited Kussmaul respirations. Initial investigations confirmed severe DKA: serum gluco

Baseline predictors of in-hospital mortality among patients with chronic kidney disease admitted to the emergency department.

BACKGROUND: Chronic kidney disease (CKD) contributes significantly to emergency department (ED) presentations in low- and middle-income countries. These patients frequently have multiple comorbidities and face high in-hospital mortality. However, limited data exist on early predictors of mortality at ED admission. Identifying key clinical and laboratory features associated with adverse outcomes may support timely risk stratification and targeted interventions for acutely ill CKD patients. AIM: To identify baseline predictors of in-hospital mortality in adult Indian patients with CKD admitted

Thiamine status during treatment of diabetic ketoacidosis in children - tertiary care centre experience.

OBJECTIVES: There is a lack of information regarding thiamine status in children with diabetic ketoacidosis (DKA). This study was designed to assess the thiamine status upon admission and 24 h after treatment initiation of DKA, whether newly diagnosed children or with established T1DM diagnosis, who presented with DKA. METHODS: We enrolled 90 children (mean age, 9.8 ± 2.6 years; 58 females and 32 males) with type 1 diabetes mellitus (T1DM), whether newly diagnosed or with an established T1DM diagnosis (from 1 to 5.2 years ago), who presented with DKA. We observed the initial Glasgow Coma Scal

A missed opportunity - consequences of unknown levetiracepam pharmacokinetics in a peritoneal dialysis patient.

BACKGROUND: Levetiracetam is a frequently used drug in the therapy of partial onset, myoclonic and generalized tonic-clonic seizures. The main route of elimination is via the kidneys, which eliminate 66% of the unchanged drug as well as 24% as inactive metabolite that stems from enzymatic hydrolysis. Therefore dose adjustments are needed in patients with chronic kidney disease stage 5 D, i.e. patients undergoing dialysis treatment. In this patient population a dose reduction by 50% is recommended, so that patients receive 250-750 mg every 12 hours. However "dialysis" can be performed in using
