Toward a definition of a successful dieter.

The purpose of the research was to examine the characteristics of successful weight losers and to compare the findings with a control group of unsuccessful weight losers. The project was designed around a definition of successful dieters, based on the Metropolitan Life Insurance weight tables. The definition, in fact, was not particularly useful. This article describes the difficulties associated with using the original definition as the inclusion criteria for sample selection, the reanalysis of data using the Body Mass Index (BMI), and finally, the creation of a new definition developed from

Diabetes mellitus-associated glycogen storage hepatomegaly: report of a case and review of the Japanese literature.

A huge hepatomegaly was seen in a 30-yr-old female diabetic who was treated with high dose of insulin for her uncontrollable food ingestion. The liver function at the peak of the hepatic enlargement showed a moderate increase of transaminases, alkaline phosphatase, and gamma-glutamyl transpeptidase. The histology of the enlarged liver revealed PAS-positive granules in enlarged hepatocytes, indicating the presence of massive glycogen storage. On admission, she was maintained under a calorie-restricted diet and received approximately 15 to 20 units per day of insulin supplement. At one month aft

[Role of modern diet therapy in the prevention of visceral complications in patients with dietary obesity].

The examination and treatment of 547 patients with alimentary obesity revealed in them significant impairments of myocardial contractility, manifest hemodynamic disorders (in 65.5% of patients), marked changes in lipid metabolism (hypercholesterolemia, hypertriglyceridemia, hyperlipacidemia, hyperlipoproteinemia type IIa in 26,5%, type IIb in 14,5%, type IV in 32,5% of patients), as well as reduced activity of the T-immunity system, all these changes being prerequisites for the development of atherosclerosis, ischemic heart disease and essential hypertension. During examination of the patients

Dysphoric mood and overeating.

Three naturalistic studies were conducted to investigate the disinhibitory effect of dysphoric mood states on eating. A clear association between dysphoric mood and overeating was found in patients with bulimia nervosa and in a group of students who were high restrainers on a weight-reducing diet. The effect was much weaker in a third group of non-dieting high restrainers.

The metabolic response to hypocaloric protein diets in obese man.

Exogenous protein in the absence of other calories can cause protein-sparing, but the mechanisms involved are controversial. It has been postulated that low insulin and high fat-derived substrate levels are necessary and sufficient conditions for such protein-sparing. We therefore established such conditions with differing protocols of protein input to define the role of protein input in mediating the response. Three groups of obese, nondiabetic subjects received the following diets: (1) 82.5+/-1.0 g protein/day (400 cal/day) for 21 days, n = 7; (2) the same, but as a refeeding diet for 7 days

Weber-Christian disease after weight loss.

Weber-Christian disease has no known cause and is characterized by a febrile, recurrent, nonsuppurative panniculitis occurring preferentially in young, obese women. The present case is of interest in that it occurred in a young, obese woman after she lost 18.2 kg (40 lb) in eight weeks. It is only speculative as to whether the sudden weight loss precipitated the Weber-Christian disease. However, since Weber-Christian disease is an obscure disease with metabolic and autoimmune overtones, such a hypothesis is not unrealistic in a predisposed individual.

Understanding and managing disturbances in insulin metabolism and body weight in women with polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is a common clinical and metabolic condition in women of reproductive age. It is associated with short-term reproductive and long-term metabolic dysfunction. Treatment has traditionally focused on fertility and hormonal therapy. However, general obesity, central obesity and insulin resistance are strongly implicated in its aetiology and improving these factors has proved highly successful in some clinical situations, reducing the need for costly assisted reproduction. A low-fat, high-carbohydrate diet is thought to improve insulin sensitivity, aid in weight los

Weight loss practices of college wrestlers.

PURPOSE: The purpose of this investigation was to examine the weight management (WM) behaviors of collegiate wrestlers after the implementation of the NCAA's new weight control rules. METHODS: In the fall of 1999, a survey was distributed to 47 college wrestling teams stratified by collegiate division (i.e., I, II, III) and competitive quality. Forty-three teams returned surveys for a total of 741 responses. Comparisons were made using the collegiate division, weight class, and the wrestler's competitive winning percentage. RESULTS: The most weight lost during the season was 5.3 kg +/- 2.8 k

Eating disorders, dieting, and the accuracy of self-reported weight.

OBJECTIVE: This study investigated the accuracy of self-reported weight and height in individuals with an eating disorder (i.e., anorexia nervosa [AN] and bulimia nervosa [BN]) and in individuals without an eating disorder (i.e., dieters and nondieters). METHOD: Self-reported and measured weights and heights were obtained from the eating disorder sample (n = 81) and the college student sample (n = 163) and were compared within and between the groups. RESULTS: Eating disorder patients were extremely accurate at self-reporting their weight. However, there was a significant difference in accura

[Tolerance and acceptability of dexfenfluramine in the long-term treatment of obesity in private practice. ESIM Group].

OBJECTIVE: The value of a one year prescription of dexfenfluramine (dF) was demonstrated by a large multicentric study performed in centres specialised in the treatment of obesity (Guy-Grand 1989). We tried to evaluate the safety and acceptability of this treatment prescribed in routine practice. METHOD: 27 internal physicians or general practitioners recruited 58 overweight patients who received 15 mg of dF twice a day in association with a balanced hypocaloric diet and were followed for one year with two-monthly visits, plus a visit after the first month of treatment. At each visit, the pat

A prudent and practical approach to the treatment of obesity.

Although obesity is difficult to treat, the only effective long-term strategy is an emphasis on adoption of a healthy lifestyle, including exercise, and prudent control over eating habits. Such modifications in behavior can be implemented by some patients with the help and encouragement of their physician, whereas other patients need professional, psychological intervention. VLCDs are useful to attain initial weight loss, but are not (nor were ever intended to be) a substitute for the development of a prudent lifestyle. Thus, for patients with major obesity (BMI > 30), the combination of a VLC

[Effects of hypocaloric diet on respiratory manifestations in Willi-Prader syndrome].

BACKGROUND: Prader-Willi syndrome (PWS) is a multisystem disorder with hyperphagia and obesity. Breathing disorders such as snoring, sleep apnea syndrome, and sleep hypoventilation have also been reported. CASE REPORT: Jonathan was born with the typical features of PWS. He developed exercise dyspnea, cyanosis and nocturnal sweating at the age of 3 years. A few months later, a respiratory failure required his admission to the intensive care unit. His weight was then 48 kg (300% ideal body weight (IBW); physical examination showed polypnea (60 breaths/min), cyanosis, fat deposition on the chest

[Xerophthalmia caused by self-induced deficiency disease].

Xerophthalmia is no longer seen in Europe since the 19th century. We report the case of a female patient, aged 33, of french nationality, who presented a clinical picture of major vitamin A deficiency, self-induced and with ocular complications. An untreated long standing depressive condition appeared to be the major etiologic factor. Perforation of a desmetocele necessitated penetrating keratoplasty. Histologic corneal and conjunctival aspects are reported. Oral prescription of vitamin A resulted in considerable clinical improvement. Only 3 similar cases have been previously reported. Clinica

Effects of diet on insulin and gastric inhibitory polypeptide levels in obese children.

To improve understanding of the relationships between gastric inhibitory polypeptide (GIP) and insulin secretion and food intake in obesity, immunoreactive insulin and immunoreactive GIP were measured in 5 obese children during PO glucose tolerance test carried out before and after diet. Before diet, mean insulin levels were normal at fasting and rose after glucose ingestion. The mean fasting immunoreactive GIP level was very high (1235 +- 209 pg/ml) compared to that of 8 healthy adult controls (411 +/- 44 pg/ml) and remained at this level throughout the test. There was only a short postabsorp
