In a large collection of human embryos (the Kyoto Collection of Human Embryos, Kyoto University), we encountered five cases with abnormal dilatation of the neural tube at the lumbosacral level. In these examples, the central canal was enlarged, and the roof plate of the neural tube was extremely thin and expanded. The mesenchymal tissue was scarce or lacking between the roof plate and the surface ectoderm. The ventriculus terminalis or "fifth ventricle" is an ependyma-lined residual lumen of the caudal portion of the spinal cord (the conus medullaris). We present the cases of three neonates with asymptomatic cystic dilatation of the ventriculus terminalis as seen on spinal sonography. Over a 4 year period (1996-1999), we prospectively found three cases in which spinal sonograms demonstrated cystic dilatation of the ventriculus terminalis of the conus medullaris in normal term neonates. A rare case of congenital mitral insufficiency characterized by six well-demarcated leaflets of mitral valve and annular dilatation is reported. At operation, the mitral valve was successfully repaired by resection of the posterior median leaflet with subsequent annular plication, closure of unusual valvular tissue divisions, and ring annuloplasty.

Drainage of a persistent left superior vena cava to the coronary sinus is a common congenital lesion associated with dilatation of the coronary sinus. The vascular anatomy of the clinical varicocele, and the incidence of involvement of the external spermatic vein were studied. Dilation of the external spermatic vein (4 mm) was found in 49.5 percent of 93 varicoceles in 67 patients. A 54 year-old man was affected by three successive infarctions in the vertebro-basilar territory. These infarctions were related to a dolichoectatic basilar artery visualized by arteriography and NMR. Deafness occurred first on the left side and then, after the third infarction, on the right side. The authors underline that deafness can be observed after a pontine infarction in the territory of the anterior inferior cerebellar artery. A dolichoectatic basilar artery can be the source of thrombotic or embolic strokes. Their prevention by antiaggregant or anticoagulant therapy is suggested.
