Spontaneous internal carotid artery dissection (ICAD) is a rare disease and an important cause of stroke in young patients. The presentation of ICAD is variable and atypical. We have used three-dimensional T1-weighted volume isotropic turbo spin-echo acquisition to help diagnose ICAD. ICAD should be considered in young patients presenting with relevant symptoms in an emergency setting. We herein report ICAD with an unexplained onset in two patients. Neither had a history of large or small cervical trauma, but both had a history of hypertension. The first patient was a 33-year-old man who prese

PURPOSE: A novel approach was introduced for breast surgery using the BiClamp, a new bipolar thermal energy device, to avoid complications and to shorten the time required for the dissection of the axillary lymph nodes.
METHODS: Thirty-six patients with early breast cancer were assessed. The surgical parameters were compared between the procedures performed using the BiClamp technique (n = 14) and conventional surgery with suture ligation (n = 22). The parameters included the operation time, blood loss, and discharge on the first postoperative day. In addition, each of those parameters was co

BACKGROUND: There are several clinical situations in which large epicardial coronary arteries are deprived of blood flow, such as occurs when an obstructing thrombus or embolus lodges within a vessel or during coronary dissection. There is little information concerning the effect of flow deprivation on large epicardial coronary arteries.
METHODS AND RESULTS: We studied a model in which a segment of a large epicardial coronary artery was deprived of blood flow using both proximal and distal clamps for 3 hours followed by reperfusion. On examination by light microscopy of cross sections of the

Blood-retinal barrier (BRB) and blood-aqueous barrier (BAB) permeability were assessed by various methods to clarify conflicting reports on whether blood-retinal barrier permeability changes occur in diabetic and galactose-fed rats and to assess the potential role of aldose reductase in this process. Different molecular weight probes were utilized in rats fed for 7-11 months either a normal diet or a diet containing 50% galactose with/without the aldose reductase inhibitor AI1576 or Ponalrestat. BRB and BAB were assessed through radiolabelled sucrose permeability studies in eyes where the ante

Background: Spinal cord ischemia is one of the most serious complications after an aortic repair. To date, there is no evidence for arterial changes during an aortic dissection or for the observation of such arteries after an aortic repair. The aim of this study was to compare spinal-cord-supplying arteries in patients with an acute aortic dissection, preoperatively and postoperatively, with patients without an acute aortic dissection. Methods: Preoperative and postoperative contrast-enhanced computed tomography scans of 25 patients who had undergone the frozen elephant trunk procedure to trea

OBJECTIVES: The objective of this study was to compare perioperative and long-term outcomes of hemiarch versus zone 2 arch replacement for DeBakey type I acute aortic dissection.
METHODS: From 2002 to 2023, 743 patients underwent surgical repair for DeBakey type I acute aortic dissection with either hemiarch (N = 605, 81.4%) or zone 2 arch replacement (N = 138, 18.6%). 4:1 propensity-score matching yielded a cohort of 437 hemiarch patients (76.1%) and 137 zone 2 patients (23.9%). Key outcomes included 30-day mortality, 10-year survival, and distal reintervention.
RESULTS: In the matched coho

BACKGROUND AND PURPOSE: Cervicocephalic artery dissection (CCAD) is a well-recognized cause of ischemic stroke. However, complex forms of CCAD, characterized by a wide intimal inlet without a visible intramural hematoma, pose diagnostic challenges and complicate endovascular access to the true lumen when recanalization is required. We aimed to analyze the clinical presentation, outcomes, and feasibility of endovascular treatment of complex CCAD and to propose a novel morphological classification.
METHODS: We retrospectively analyzed consecutive patients treated for acute CCAD at Lariboisière

Percutaneous septal fenestration and stenting is a useful technique in acute aortic dissection when there is persistent malperfusion of renovisceral vessels, despite primary stenting of the thoracic aorta or when coverage of the primary entry tear is not technically feasible. Intravascular ultrasound provides reassurance to the interventionalist that the septal fenestration targets the level of malperfusion. Septal fenestration may promote continued false lumen perfusion into target vessels and aneurysmal degeneration, so this technique should be considered as a temporizing measure.Clinical Im

BACKGROUND: Cervical arterial dissection (CeAD) is a leading cause of stroke in young adults with an early presentation often mimicking musculoskeletal pain. Currently, no validated tests exist and CAD may be missed. A diagnostic support tool could help guide urgent referral for imaging, when to monitor, or when safe to proceed with treatment, and ultimately help stroke prevention.
OBJECTIVES: To validate a 4-criteria diagnostic support tool for early recognition of CeAD in primary care, to refine tool descriptors as needed, and propose optimal cut-offs for clinical application.
DESIGN: Pros

OBJECTIVES: Total arch replacement using the frozen elephant trunk technique remains the gold standard for a definite aortic arch repair. To facilitate surgical management of the left subclavian artery (LSA), a new hybrid frozen elephant trunk device was recently developed.
METHODS: A 62-year-old female patient presented with acute type A aortic dissection and underwent emergent aortic root and total arch replacement using the novel custom-made Evita Neo EDE hybrid arch device for frozen elephant trunk implantation. To facilitate the management of the LSA, a covered stent connected to the dev

Complete thrombosis of the false lumen in chronic aortic dissection is essential to achieve positive aortic remodeling. However, persistent perfusion through aortic collaterals, dissected supra-aortic trunks (SAT), and renovisceral arteries often complicate this process. Our approach to treat chronic dissections integrates TEVAR and custom-made FBEVAR, often combined with supra-aortic trunk debranching or frozen elephant trunk (FET) procedures. Preemptive embolization of aortic side branches (intercostal, lumbar, mediastinal, bronchial, and mammary arteries) is a key strategy to prevent endole

We report a case of acute type A aortic dissection (ATAAD) with an isolated left vertebral artery (ILVA), and our successful surgical treatment with a fenestrated frozen elephant trunk (FET). A 56-year-old man was referred to our department for chest and back pain. Contrast enhanced computed tomography (CT) revealed ATAAD. The left vertebral artery branched directly from the aorta. We performed total arch replacement with fenestrated FET to preserve the ILVA and left subclavian artery( LSCA) blood flow. During surgery, the aorta was transected at zone 2, and FET was deployed into the distal ao

INTRODUCTION: Patients with connective tissue disease and aortic root aneurysm are at risk of dissection and progression of the dilatation. Standard treatment of aortic root aneurysm is either a total root replacement or valve-sparing root replacement. Both procedures have drawbacks, such as the need for anticoagulation and long-term exposure to the chance of reoperation or endocarditis. The ExoVasc personalized external aortic root support (PEARS) is a perivascular stent that has been introduced to reduce these risks while aiming for a similar beneficial outcome on late aortic complications.

Although the management of traumatic injuries to the thoracic aorta has shifted toward endovascular management, the historical standard of care is open reconstruction. Choosing to reoperate when faced with a complication from a prior open repair can be challenging; endovascular management can be a reasonable option in this situation. This report describes a 54-year-old man with a remote history of open surgery for a traumatic injury to the descending thoracic aorta who underwent endovascular aortic stent graft placement for coverage of extrathoracic graft extension with pseudoaneurysm formatio
