New potential morphologic features of the aorta associated with the occurrence of acute type B aortic dissection.

The present study aimed to identify aortic morphological features associated with the occurrence of acute type B aortic dissection (aTBAD) and establish a risk prediction model. Patients in the control group were age- and sex-matched in a 1:1 ratio with the aTBAD group. Morphological parameters of the ascending aorta and aortic arch were measured on the GE Healthcare Advantage Workstation. The length and diameter of the ascending aorta and aortic arch in the aTBAD group increased significantly (all P < 0.001). The tortuosity and angulation of ascending aorta were significantly larger in the aT

Aortic arch type, a novel morphological indicator and the risk for acute type B aortic dissection.

OBJECTIVES: Aortic arch type is a readily recognizable and obtainable morphological feature of the aorta that does not require complex measurements. The goal of this study was to evaluate whether aortic arch type is a comparable and alternative morphological parameter for predicting acute type B aortic dissection (aTBAD) by comparing the prognostic value of the aortic arch type with that of other morphological parameters such as aortic length, angulation and tortuosity index. METHODS: The patients with aTBAD (n = 216) were matched 1:1 with a control group (n = 263) by propensity score matchin

Endarterectomy and external prosthetic grafting of the ascending and transverse aorta under hypothermic circulatory arrest.

A 78-year-old woman presented with acute pulmonary edema, a blood pressure of 250/160 mmHg, and a 4/6 diastolic murmur of probable aortic origin. Aortography revealed 4+ aortic regurgitation, left ventricular dysfunction, a right coronary artery with good distal run-off but complete proximal occlusion, a fusiform aneurysm of the ascending and transverse aorta (with a transverse dissection in the left anterolateral wall of the upper ascending aorta, but no evidence of intramural lumen), and milder, isolated dilatation of the descending thoracic aorta. Upon operation, on 8 September 1987, I disc

Nine years experience of aortic arch repair with the aid of antegrade selective cerebral perfusion.

AIM: Antegrade selective cerebral perfusion (ASCP) is gaining widespread popularity in aortic arch surgery because it has been demonstrated to be an optimal technique of cerebral protection. This study demonstrates the clinical results of aortic arch repair with ASCP. METHODS: Between November 1996 and September 2004, 250 patients underwent thoracic aorta replacement using ASCP under moderate hypothermia. Mean patients age was 63+/-11.5 years. Presenting pathologies were chronic aneurysm in 136 patient (54.4%), type A acute aortic dissection in 80 patients (32%), post-dissection aneurysm in 3

Endovascular Septotomy During Thoracic Endovascular Aortic Repair for Chronic Dissection-Related Aneurysm: All That Glitters Is Not Gold.

Although endovascular repair is preferred over open techniques for chronic dissection-related thoracoabdominal aneurysms, specific adjunctive maneuvers are frequently required to optimize the distal landing zone of the thoracic endovascular aortic repair (TEVAR) or to enlarge the narrowed true lumen to deploy a branched endograft. Recently, transcatheter endovascular septotomy (TES) has gained attention as an evolving minimally invasive technique to optimize complex TEVAR procedures creating a better apposition of the endograft to the native aortic wall with the primary aim to block the retrog

Right Axillary Artery Cannulation for Endovascular Repair of an Acute Type A Aortic Dissection.

We present the case of a 48-year-old woman with an acute type A aortic dissection that was treated with thoracic endovascular aortic repair at our institution. The patient was found to have a focal type A dissection with pericardial effusion but no tamponade physiology and no involvement of the aortic valve or root. We elected to treat the patient's type A aortic dissection with an endovascular stent because of the patient's favorable anatomy and no evidence of neurologic deficits or signs of distal malperfusion. The patient was successfully treated with an abdominal aortic cuff deployed throu

Traumatic type B aortic dissection.

A 37-year-old man was referred with thoracic pain after a deceleration trauma. He also had a cerebral contusion and a wrist fracture. There were no sings of hypovolemic shock. Computerized tomography (CT) of the chest and transoesophageal echocardiography (TEE) demonstrated a type B aortic dissection originating just distal to the left subclavian artery. There was a patent false lumen without rupture or distal ischaemia. Conservative treatment was given. A paralytic ileus developed and abdominal complaints persisted for several months. Angiography showed normal patency of mesenteric vessels. O

Acute type B aortic dissection after ruptured thoracoabdominal aortic aneurysm repair resulting in thoracoabdominal branch endoprosthesis collapse and malperfusion.

We report the case of a patient who developed an acute Stanford type B aortic dissection 4 days after thoracoabdominal branch endoprosthesis repair for a ruptured thoracoabdominal aortic aneurysm. The dissection caused complete endograft collapse and profound malperfusion. Emergent salvage endovascular repair successfully re-expanded the graft; however, the patient ultimately died because of the severity of the systemic injuries. This case highlights the potential for acute type B aortic dissection to occur in a recently stented thoracic aorta segment following thoracoabdominal branch endopros

Giant cell aortitis: clinical presentation and outcomes in 40 patients consecutively operated on.

OBJECTIVES: Giant cell arteritis (GCA) may affect mid-size and large-size arteries. Although temporal arteritis is a well-characterized clinical entity, GCA of the thoracic aorta remains ill defined. The aim of the study was to evaluate the clinical presentation, surgical and mid-term outcomes in patients operated for GCA of the thoracic aorta. METHODS: A retrospective review of patients operated for GCA of the thoracic aorta was conducted. The diagnosis of GCA was established by the pathology report. RESULTS: Forty consecutive patients (mean age of 66.6 ± 9.1 years) with a diagnosis of GCA

Stent-assisted balloon-induced intimal disruption and relamination in complicated chronic residual type B aortic dissection after frozen elephant trunk treatment.

CASE REPORT: a 77-year-old man presented to our Emergency Department for worsening dyspnea. A chest CT scan showed a type A aortic dissection with a dilatation of the ascending and descending aorta. After evaluating the case with cardiac surgeons, it was decided to perform an aortic arch replacement through the frozen elephant trunk technique. Three months after cardiac surgery, a TC scan showed a residual 58 mm dissecting aneurysm of the descending thoracic aorta and a collapse of the true lumen so we decided to perform an endovascular treatment of the thoracoabdominal aorta according to the

The frozen elephant trunk technique for the treatment of acute complicated Type B aortic dissection.

OBJECTIVES: Our goal was to report our preliminary results in patients with acute complicated Type B aortic dissection without a suitable landing zone for primary thoracic endovascular aortic repair who were treated with the frozen elephant trunk (FET) technique. METHODS: Within a 25-month period, 14 patients with acute complicated Type B aortic dissection underwent surgical repair using the FET technique. The reasons to perform the FET procedure were an ectatic ascending aorta/arch in 6 patients and the lack of an adequate landing zone in 8 patients. RESULTS: No deaths were observed. A non-

Outcomes of aortic arch repair using the frozen elephant trunk technique: analysis of a Scandinavian center's results over 14 years.

BACKGROUND: The frozen elephant trunk (FET) technique enables repair of aortic arch pathology, with possibility for endovascular treatment of distal thoracic aortic disease. We aimed to study outcomes of FET repair of complex aortic arch disease at a Scandinavian tertiary referral center. METHODS: All patients who underwent FET repair of the aortic arch 2006-2020 were included. Survival, complication and reintervention rate, and aortic remodeling were analyzed. RESULTS: Fifty patients were included: 23 complex thoracic aortic aneurysms involving the aortic arch (TAA), 19 with chronic aortic

Descending aortic diameter of 5.5 cm or greater is not an accurate predictor of acute type B aortic dissection.

OBJECTIVE: The risk of acute type B aortic dissection is thought to increase with descending thoracic aortic diameter. Currently, elective repair of the descending thoracic aorta is indicated for an aortic diameter of 5.5 cm or greater. We sought to investigate the relationship between aortic diameter and acute type B aortic dissection, and the utility of aortic diameter as a predictor of acute type B aortic dissection. METHODS: We examined the descending aortic diameter at presentation of 613 patients with acute type B aortic dissection who were enrolled in the International Registry of Acut

Thoracic aortic aneurysm: unlocking the "silent killer" secrets.

Thoracic aortic aneurysm (TAA) is an increasingly recognized condition that is often diagnosed incidentally. This review discusses ten of the most relevant epidemiological and clinical secrets of this disease; (1) the difference in pathogenesis between ascending and descending TAAs. TAAs at these two sites act as different diseases, which is related to the different embryologic origins of the ascending and descending aorta. (2) The familial pattern and genetics of thoracic aneurysms. Syndromic TAAs only explain 5% of the pattern of inheritance. (3) The effect of female sex on TAA growth and ou

[Thoracic stent graft sizing for endovascular repair in Stanford type B aortic dissection].

Endovascular treatment for Stanford type B aortic dissection uses thoracic aortic stent-graft system, which consists of covered stent and delivery system. The system is different in the configuration of the stent, the delivery system and the mode of release. The principle of endovascular treatment for Stanford type B aortic dissection is different from aortic aneurysm, so there are differences in stent selection. On the basis of preoperative assessment and measurement, it is suggested that the proximal diameter oversize of the stent is 0 to 5%, at 160 to 200 millimeters in length. The distal e

Physician-modified Endograft with Left Subclavian Artery Fenestration for Ruptured Type B Aortic Dissection.

A 56-year-old male patient was transferred to our institution with acute chest and back pain and deteriorating vital signs for 3 days. Emergent computed tomography angiography (CTA) revealed ruptured type B aortic dissection with large left hemothorax. The dissection extended into the left subclavian artery (LSA). Immediate endovascular aortic repair with LSA coverage to extend the proximal landing zone was planned. Fenestrated thoracic endovascular repair (fTEVAR) was performed using a physician-modified endograft (PMEG) to maintain LSA perfusion. The thoracic endograft was modified on a back

Important issues regarding planning and sizing for emergent TEVAR.

In the past decades, treatment of acute thoracic aortic syndrome underwent drastic changes with a central role for thoracic endovascular aortic repair (TEVAR). One of the essential factors in the success of TEVAR is accurate sizing of the endograft, as both under- and oversizing can lead to suboptimal results and disastrous complications. The aim of this review was to give an overview of issues regarding endograft sizing in emergent TEVAR. Sizing of the endograft can be complicated by specific factors related to the underlying disease. For instance, different types of the acute thoracic aortic
