Three cases are described in which there was concurrent development of acute cholecystitis and a second acute abdominal illness. Acute cholecystitis occurred in patients with acute appendicitis, small bowell obstruction, and acute colonic diverticulitis. Experience with three such cases over the course of eight years by a single surgeon suggests a possible aetiological link between the two diseases. It is suggested that, under some circumstances, exploration of an acute abdomen may need to be more than cursory.

The sigmoid colon differs from the remaining colon anatomically, functionally and by increased incidence of disease. Occasionally it may be very difficult to differentiate diverticulitis of the sigma from a malignant tumor. It is felt that hypotonic sigmoidography improves the diagnostic accuracy. Selective angiography of the inferior mesenteric artery may be of further diagnostic help. Pharmaco-angiographic examinations with Eupaverin yielded no better results.

Colovaginal fistula is infrequently encountered in gynecologic practice, but, when it does occur, diverticular disease is the most common cause. This paper discusses current concepts in etiology, diagnosis, and treatment of patients with colovaginal fistula secondary to diverticular disease. Review of the literature reveals that a majority of patients have a history of hysterectomy and are primarily diagnosed by barium enema. The current trend in treatment is surgical correction with primary resection and anastomosis.

BACKGROUND: Diverticula are bulging pouch-like herniations in the wall of the hollow organ anywhere in the gastrointestinal tract from the oesophagus to the colon. Diverticula are thought to occur more commonly in the colon than in any other segment of the gastrointestinal tract. It may be asymptomatic or complicated by diverticulitis (diverticular inflammation), bleeding, or perforation with peritonitis. It could be potentially life threatening. It was thought to be rare in this part of the world, but is increasingly diagnosed with the advent of modern imaging techniques and colonoscopy. Two

INTRODUCTION: Primary colonic lymphoma is a very rare malignant disease of the gastrointestinal tract, accounting for 14% of all malignant diseases in this location. It is classified in the group of extranodal lymphomas; its long-term asymptomatic progression makes it different from common colorectal carcinomas making its diagnosis very difficult, more often accidental. Gallstone ileus is quite an uncommon complication of cholecystolithiasis diagnosed with difficulty. Up to 50% of cases are diagnosed during surgery. The obturated location depends on the size of the stone, location of the conju

BACKGROUND: The objective of this article is to review the evolving role of laparoscopic surgery in the treatment of complicated diverticulitis.
PURPOSE: The authors attempted to give readers a concise insight into the evidence available in the English language literature. This study does not offer a systematic review of the topic, rather it highlights the role of laparoscopy in the treatment of complicated diverticulitis.
CONCLUSIONS: New level 1 evidence suggest that observation rather than elective resection following nonoperative management of diverticulitis with abscess and/or extralumi

There are many applications to emergency services due to acute colonic diverticulitis. It is necessary to decide whether these patients are complicated, to quickly predict their prognosis, and to decide whether medical or invasive treatment is necessary. We planned to research effectiveness NLR, NLR, SII, and SIRI values calculated using hemogram data can predict the prognosis of acute diverticulitis. We managed a retrospective scanning with patients who applied with a diagnosis of acute diverticulitis between 06/2020 and 04/2023. Demographic data (age, gender, previous surgery, comorbid disea

INTRODUCTION: Improvements in antibiotic and intensive care treatment have facilitated the conservative therapy of patients suffering from complicated diverticulitis of the sigmoid for the time necessary for preparation in view of early elective resection of the sigmoid colon. Consequently, early elective resection of the sigmoid colon with primary anastomosis has become very popular as the treatment of choice in complicated diverticulitis. This has led us to a retrospective evaluation of our own results with both Hartmann's operation and resection with primary anastomosis.

While colonic diverticulosis affects approximately half of adults over 60 years of age in Western populations, jejunal diverticulosis are rare with a reported prevalence of 0.2%-1.9% associated with high mortality rate (up to 40%) when presented as complicated. Performing lifesaving surgery while respecting patient's autonomy and informed refusal of blood transfusion represents a challenge on its own for the medical team. We present a case of a 74-year-old Jehovah's Witness man who was diagnosed intraoperatively with perforated jejunal diverticulitis. Emergency laparotomy with segmental jejuna

Natural history of diverticular disease (DD) through its different subgroups (diverticulosis, symptomatic uncomplicated diverticular disease (SUDD) previous diverticulitis (PD)) is poorly defined. Few studies, mainly retrospective, have analysed its rate of progression and risk factors associated. We assessed the potential progression rate of different spectrum of DD and associated risk factors. Data from the REMAD Registry, a 5-year Italian longitudinal prospective multicentre study, were analysed. A total of 1217 patients were included that were divided into three subgroups (diverticulosis,

From 1984 to 1993, 11 patients (7 men and 4 women, mean aged 70 years) underwent surgical procedure for sigmoido-vesical fistula due to diverticulitis. Fecaluria and/or pneumaturia was present in 10 patients. The diagnosis of sigmoido-vesical fistula due to diverticulitis was confirmed by urologic and colonic investigations. All patients underwent surgical treatment. One patient underwent creation of a diverting colostomy alone because of a poor general status. Definitive surgical correction of the fistula with resection of the diverticular colon was attempted in the remaining 10 patients, inc

At cystoscopy a focal mucosal abnormality may be indicative of a pathological process extrinsic to the urinary bladder and has been termed 'the herald lesion'. The aim of the present pictorial essay was to describe the radiographic counterpart to this cystoscopic finding. Radiographic herald lesions are shown in patients with extravesical inflammatory (Crohn's disease, colonic diverticulitis and pelvic inflammatory disease) and neoplastic (colon carcinoma) processes, and urinary tract complications of these conditions are described and illustrated.

Nonsurgical diagnosis of colonic diverticulosis and most complications of diverticular disease is best established by barium enema study. Multiple diverticula can produce narrowing and shortening of the colon that is unrelated to any complication occurring in the diverticula. The diagnosis of diverticulitis is made radiologically be demonstrating the effects on the lumen of the bowel of the pathologic changes visible on gross inspection, eg, local abscess, intramural dissection, fistula, obstruction. Frequently the radiologic findings are subtle.

BACKGROUND Although diverticular disease is well described and treated in daily clinical practice, there are cases that attract great interest because of their complexity and difficulty in management. Herein, we describe a rare case of colo-colonic fistula-complicated diverticulitis that necessitated urgent surgical intervention. CASE REPORT A 76-year-old female patient with a known history of diverticular disease of the sigmoid colon presented in the Emergency Department for evaluation of left lower quadrant abdominal pain. The clinical and radiological examinations revealed a recurrent episo

BACKGROUND: Although the Hinchey scoring system has guided surgical decision making for perforated diverticulitis, what constitutes optimal surgical management is controversial. We report our experience of selective primary closure of the perforation without use of a transverse colostomy and the specific circumstances in which this may be safe.
METHODS: All cases of perforated diverticular disease of the sigmoid colon with Hinchey grade IV (faecal) peritonitis seen over a 4-year period from one surgical unit were reviewed.
RESULTS: Primary closure without a diverting stoma was performed in s

The classifications of acute uncomplicated diverticulitis and complicated diverticulitis have served us well for many years. However, in recent years, we have noted the prevalence of variations of uncomplicated diverticulitis, which have not precisely fit under the classification of 'acute resolving uncomplicated diverticulitis', which manifests itself with the typical left lower quadrant pain, fever, diarrhea, elevated white blood count, and CT findings, such as stranding, and which resolves fairly promptly and completely on oral antibiotic therapy. For these other variations, we would sugges
