Malaria medicines to address drug resistance and support malaria elimination efforts.

Antimalarial drugs are essential weapons to fight malaria and have been used effectively since the 17th century. However, P.falciparum resistance has been reported to almost all available antimalarial drugs, including artemisinin derivatives, raising concerns that this could jeopardize malaria elimination. Areas covered: In this article, we present a historical perspective of antimalarial drug resistance, review current evidence of resistance to available antimalarial drugs and discuss possible mitigating strategies to address this challenge. Expert commentary: The historical approach to drug

Antimicrobial interactions: mechanisms and implications for drug discovery and resistance evolution.

Combining antibiotics is a promising strategy for increasing treatment efficacy and for controlling resistance evolution. When drugs are combined, their effects on cells may be amplified or weakened, that is the drugs may show synergistic or antagonistic interactions. Recent work revealed the underlying mechanisms of such drug interactions by elucidating the drugs' joint effects on cell physiology. Moreover, new treatment strategies that use drug combinations to exploit evolutionary tradeoffs were shown to affect the rate of resistance evolution in predictable ways. High throughput studies hav

[The difficult diagnosis of tuberculosis].

In spite of alarming reports from third world and industrialized countries, no increase in the incidence of tuberculosis has been seen in Switzerland. This is mainly attributable to the consistently performed initial treatment with a multiple-drug combination and the good compliance of our patients. In Switzerland the diagnosis of tuberculosis is difficult, because of its low incidence (1990: 1229 reported cases), as well as its nonspecific initial symptoms. Screening tests are only indicated in high risk-populations with direct contact to infectious tuberculosis cases, inhabitants of centres

Antimalarial Drug Resistance: Literature Review and Activities and Findings of the ICEMR Network.

Antimalarial drugs are key tools for the control and elimination of malaria. Recent decreases in the global malaria burden are likely due, in part, to the deployment of artemisinin-based combination therapies. Therefore, the emergence and potential spread of artemisinin-resistant parasites in southeast Asia and changes in sensitivities to artemisinin partner drugs have raised concerns. In recognition of this urgent threat, the International Centers of Excellence for Malaria Research (ICEMRs) are closely monitoring antimalarial drug efficacy and studying the mechanisms underlying drug resistanc

Moxifloxacin and Sulfamethoxazole-Based Nanocarriers Exhibit Potent Antibacterial Activities.

Antibiotic resistance is a major concern given the rapid emergence of multiple-drug-resistant bacteria compared to the discovery of novel antibacterials. An alternative strategy is enhancing the existing available drugs. Nanomedicine has emerged as an exciting area of research, showing promise in the enhanced development of existing antimicrobials. Herein, we synthesized nanocarriers and loaded these with available clinically approved drugs, namely Moxifloxacin and Sulfamethoxazole. Bactericidal activity against Gram-negative (Serratia marcescens, Pseudomonas aeruginosa, Klebsiella pneumoniae,

Molecular cloning and phenotypic analysis of drug-resistance mutants with relevant S-region variants of HBV for a patient during 189-month anti-HBV treatment.

BACKGROUND: A unique chronic hepatitis B patient was followed over 189 months of nucleoside/nucleotide analogue (NA) therapies with the analysis of multiple drug-resistance HBV mutants. METHODS: Clonal sequencing (≥20 clones/sample) was performed on sera sampled at 41 time points, and the phenotypic features of eight representative mutants were analysed. RESULTS: Lamivudine (LAM)-, adefovir dipivoxil (ADV)-, entecavir (ETV)- and repeat ADV-resistance mutants emerged upon individual sequential NA monotherapy. The efficacy of NA combination rescue therapies ranked as LAM+ADV < ETV+ADV < ETV+ t

Evolution of Fitness Cost-Neutral Mutant PfCRT Conferring P. falciparum 4-Aminoquinoline Drug Resistance Is Accompanied by Altered Parasite Metabolism and Digestive Vacuole Physiology.

Southeast Asia is an epicenter of multidrug-resistant Plasmodium falciparum strains. Selective pressures on the subcontinent have recurrently produced several allelic variants of parasite drug resistance genes, including the P. falciparum chloroquine resistance transporter (pfcrt). Despite significant reductions in the deployment of the 4-aminoquinoline drug chloroquine (CQ), which selected for the mutant pfcrt alleles that halted CQ efficacy decades ago, the parasite pfcrt locus is continuously evolving. This is highlighted by the presence of a highly mutated allele, Cam734 pfcrt, which has a

Global mapping of bedaquiline-resistant Mycobacterium tuberculosis: a systematic review.

BACKGROUND: Tuberculosis (TB) is one of the deadliest bacterial infectious diseases worldwide, with rising cases of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains. Bedaquiline (BDQ)-containing regimens have become important for the treatment of MDR/XDR-TB, and resistance to BDQ is increasing. Understanding genetic mutations is crucial for early detection of BDQ-resistant strains and thus maintaining the effectiveness of these drugs. This study aimed to review mutations associated with BDQ-resistant TB globally. METHODS: This study systematically searched the keywords T

Whole-genome sequencing for surveillance of tuberculosis drug resistance and determination of resistance level in China.

OBJECTIVES: Phenotypic drug susceptibility testing for prediction of tuberculosis (TB) drug resistance is slow and unreliable, limiting individualized therapy and monitoring of national TB data. Our study evaluated whole-genome sequencing (WGS) for its predictive accuracy, use in TB drug-resistance surveillance and ability to quantify the effects of resistance-associated mutations on MICs of anti-TB drugs. METHODS: We used WGS to measure the susceptibility of 4880 isolates to ten anti-TB drugs; for pyrazinamide, we used BACTEC MGIT 960. We determined the accuracy of WGS by comparing the preva

Limited fluoroquinolone resistance among Mycobacterium tuberculosis isolates from Rwanda: results of a national survey.

OBJECTIVES: There is an increasing interest in the possible role of fluoroquinolone antibiotics for the treatment of tuberculosis (TB), but widespread use of these antibiotics for the treatment of other bacterial infections may select for fluoroquinolone-resistant Mycobacterium tuberculosis strains. METHODS: We evaluated fluoroquinolone susceptibility using the proportion method (ofloxacin, critical concentration 2.0 mg/L) in isolates from patients enrolled in a national drug resistance survey in Rwanda from November 2004 to February 2005. RESULTS: Of the 701 M. tuberculosis isolates studied

Episome profiles and mobilizable beta-lactamase plasmid in Haemophilus ducreyi.

Chancroid caused by Haemophilus ducreyi has been described as a significantly predisposing factor of HIV heterosexual transmission in an endemic region of both diseases. The fastidious, H. ducreyi has been reported world wide with various antimicrobial susceptibility patterns. A high tendency of drug resistances has generally been found among isolates derived in Thailand. In this study, the plasmids of H. ducreyi were isolated and analysed from 63 clinically derived organisms. Twenty-nine out of 63 isolates (46%) revealed the same plasmid profiles. Plasmid DNA was further cloned into Escherich

Problems with antimicrobial resistance in gram-positive cocci.

The development of antimicrobial resistance has almost invariably accompanied the therapeutic use of antimicrobial agents. Newer antimicrobials have succeeded partly but not entirely in overcoming the problem of resistance. Antimicrobial resistance in gram-positive cocci has achieved its greatest prominence in the past 15 years. There has been a steady erosion of antimicrobial activity against gram-positive cocci. The presentations in this symposium dealt with some of the important problems of antimicrobial resistance in gram-positive cocci, including methicillin resistance and multidrug resis

Expression of NEAT1 can be used as a predictor for Dex resistance in multiple myeloma patients.

OBJECTIVE: Multiple myeloma is a heterogeneous disorder and the intratumor genetic heterogeneity contributes to emergency of drug resistance. Dexamethasone has been used clinically for decades for MM. Nevertheless, their use is severely hampered by the risk of developing side effects and the occurrence of Dex resistance. LncRNA NEAT1 plays a oncogenic role and participates in drug resistance in many solid tumors. Therefore, we investigated a potential usefulness of this molecular as a biomarker for diagnosis of MM and possible correlations of NEAT1 expression with drug resistance and prognosis

Molecular signatures of concurrent mutations in drug resistant Mycobacterium tuberculosis: Association with treatment outcomes.

BACKGROUND: Mutations in genes responsible for resistance to anti tubercular drugs are pivotal in the emergence of drug resistant tuberculosis. Although studies have examined individual resistance mutations, the impact of concurrent mutations on treatment outcomes remains insufficiently understood. This study aimed to investigate the concurrent mutations in genes responsible for first and second line drug resistance in Mycobacterium tuberculosis (MTB) isolates and their association with treatment outcomes. METHODS: A line probe assay was conducted to identify mutations that confer resistance

A simple assay to screen antimicrobial compounds potentiating the activity of current antibiotics.

Antibiotic resistance continues to pose a significant problem in the management of bacterial infections, despite advances in antimicrobial chemotherapy and supportive care. Here, we suggest a simple, inexpensive, and easy-to-perform assay to screen antimicrobial compounds from natural products or synthetic chemical libraries for their potential to work in tandem with the available antibiotics against multiple drug-resistant bacteria. The aqueous extract of Juglans regia tree bark was tested against representative multiple drug-resistant bacteria in the aforementioned assay to determine whether
