Comparison of Minimal Invasive Versus Biportal Endoscopic Transforaminal Lumbar Interbody Fusion for Single-level Lumbar Disease.

STUDY DESIGN: Retrospective study. OBJECTIVE: The authors aimed to compare the clinical outcomes of biportal endoscopic transforaminal lumb … SIONS: Because BE-TLIF yieldeds lesser early postoperative back pain than did MI-TLIF, it may allow early ambulation and a shorter hospitalization period. BE-TLIF may be a viable alternative to MI-TLIF in patients with degenerative or isthmic spondylolisthesis with superior clinical results in the early postoperative period.

Thromboembolic events are uncommon after open treatment of proximal humerus fractures using aspirin and compression devices.

BACKGROUND: Thromboembolic phenomena have long been recognized as a major cause of morbidity and mortality in hospitalized patients, especially those undergoing reconstructive surgery. We have been empirically treating patients with aspirin, early ambulation, and mechanoprophylaxis after operative management of proximal humerus fractures. However, we have not established the incidence of postoperative deep vein thrombosis and pulmonary embolism in this population. QUESTIONS/PURPOSES: We determined the incidence of deep vein thrombosis and pulmonary embolism in patients having surgery for disp…

Early Nontraumatic Periprosthetic Femoral Fracture After Total Hip Arthroplasty: A Case Report.

This report describes a rare case of early nontraumatic periprosthetic femoral fracture following total hip arthroplasty. The patient was a 74-year-old woman with osteoarthritis of the hip who underwent left total hip arthroplasty via a posterior approach using a tapered, rectangular, cementless femoral stem. The patient progressed well through early rehabilitation and was ambulating independently by postoperative Day 8. However, on postoperative Day 9, she experienced sudden-onset thigh pain and was unable to bear weight while ascending stairs. Radiographs demonstrated a short oblique fractur

Association of Hispanic ethnicity with acute ischemic stroke care processes and outcomes.

BACKGROUND: Few studies have examined the actual hospital arrival mode, emergency department (ED) care processes, and early outcomes in Hispanic vs non-Hispanic acute ischemic stroke (AIS) patients. We evaluated processes and prognosis by Hispanic ethnicity among AIS patients encountered in urban setting. METHODS: We retrospectively reviewed prospectively-collected data on 1,117 AIS patients presenting within 12 hours of ictus to five hospitals in a tertiary-level stroke center network in San Diego, California. Variables of interest included pre-hospital factors, ED care processes, and favora

Regaining functional independence in the acute care setting following hip fracture.

BACKGROUND AND PURPOSE: Factors that predict functional recovery in the first few days following hip fracture and that may facilitate discharge to the home directly from the acute care setting have not been identified. This study investigated the attainment of key functional milestones by patients and discharge status from an acute care hospital following hip fracture. SUBJECTS: Subjects were 162 community-based individuals (59 men, 103 women) aged 60 years or older who were admitted to an acute care hospital following unilateral hip fracture. METHODS: Data on personal, medical surgical, hos

Expanded Access use of Intravenous Gene Transfer with AAV9-GLB1 in a Juvenile GM1 Gangliosidosis Participant.

GM1 gangliosidosis is an inherited and progressively neurodegenerative lysosomal storage disorder with no approved therapy. We report 5-year safety and 3-year clinical, biochemical, and neuroimaging efficacy outcomes following expanded-access intravenous AAV9-GLB1 gene therapy in GT01, a 6-10-year-old female with juvenile-onset GM1 gangliosidosis. During participation in a natural history study before gene transfer, GT01 developed seizures, dysarthria, loss of ambulation, and progressive neurodegeneration. She received a single intravenous administration of AAV9-GLB1 at 1.5×10 13 vector genome

Motor function outcome in postnatal insult-related cerebral palsy.

INTRODUCTION: The Gross Motor Function Classification System (GMFCS) was developed to establish uniform communication between healthcare providers, patients, and the patients' families. It is also used to prognosticate the outcome of motor function. Based on previous reports, prognostication of ambulation status in cerebral palsy is based on the motor development curve, which shows a plateau at a certain known age. CASE REPORT: This report illustrates the case of a boy with spastic triplegic cerebral palsy secondary to postnatal insult at early childhood. The patient was noted to have tremend

Feasibility and safety of passive orthostatic positioning in mechanically ventilated ICU patients: A prospective observational study.

Early mobilization strategies are essential for preventing intensive care unit (ICU) - acquired complications. However, the feasibility of passive orthostatism in mechanically ventilated patients remains underexplored. This study evaluates the safety and acute physiological effects of passive orthostatic positioning using an orthostatic board in critically ill patients undergoing invasive mechanical ventilation. A single-group exploratory study was conducted in an ICU setting with 13 adult patients. Each patient underwent a single passive orthostatic session using a motorized orthostatic board

Increasing Nurse-Led Mobilization of Critically Ill Children Through In Situ Simulation: A Quality Improvement Initiative.

OBJECTIVE: Critically ill children are at risk for preventable morbidities due to immobility. Early mobility, a key component of the ICU Liberation Bundle, improves outcomes and reduces mortality. Internationally, adherence to early-mobility protocols is low and nurses are pivotal for success. This quality improvement (QI) initiative aimed to assess whether an in situ early-mobility simulation for PICU nurses increased nurse-led mobilization of critically ill children. DESIGN: QI initiative with an observational, pre-post design. SETTING: PICU in a tertiary academic hospital in the United St

Early mobilization in postoperative glioma patients real world impact on recovery and long term prognosis.

The implementation of early mobilization in postoperative care has been shown to expedite patient recovery. However, its widespread use among patients undergoing glioma resection is not firmly established. This study aimed to implement an early mobilization protocol in glioma patients and evaluate its effects on early recovery and long-term prognosis. Patients who underwent craniotomy for glioma treatment between January 2018 and December 2019 were enrolled in a randomized controlled trial comparing conventional perioperative care (control group) with conventional care plus a structured early

To cast or not to cast? Postoperative care of ankle fractures: a meta-analysis of randomized controlled trials.

Postoperative care of ankle fractures treated with open reduction and internal fixation (ORIF) is a debated topic. A meta-analysis of Randomized Controlled Trials was conducted with the aim of comparing early mobilization and weightbearing to traditional postoperative protocols. A systematic search of electronic databases was conducted according to the PRISMA guidelines. Only randomized clinical trials were included. Data about clinical outcome, time to return to work and complications were extracted and summarized. Meta-analyses were performed. Twenty studies for a total of 1328 patients were

Is Immobilization Necessary for Early Return to Work Following Distal Biceps Repair Using a Cortical Button Technique?: A Randomized Controlled Trial.

BACKGROUND: Improvements in surgical fixation to repair distal biceps tendon ruptures have not fully translated to earlier postoperative mobilization; it is unknown whether earlier mobilization affords earlier functional return to work. This parallel-arm randomized controlled trial compared the impact of early mobilization versus 6 weeks of postoperative immobilization following distal biceps tendon repair. METHODS: One hundred and one male participants with a distal biceps tendon rupture that was amenable to a primary repair with use of a cortical button were randomized to early mobilization

Postoperative Mobilization After Superior Rotator Cuff Repair: Sling Versus No Sling: A Randomized Prospective Study.

BACKGROUND: Patients are commonly advised to wear a sling for 4 to 6 weeks after rotator cuff repair despite negative effects of early immobilization and benefits of motion rehabilitation. The aim of this study was to compare clinical and radiographic outcomes up to 6 months following rotator cuff repair with and without postoperative sling immobilization. METHODS: We randomized 80 patients scheduled for arthroscopic repair of a small or medium superior rotator cuff tear into sling and no-sling groups (40 patients each). Passive mobilization was performed in both groups during the first 4 pos
