BACKGROUND: Historically, porcine bioprosthetic valves have poor durability in pediatric patients; nearly half will require replacement within 5 years. However, our early experience with patients having Ebstein's anomaly suggests that tricuspid bioprostheses in this anomaly might have better durability.
METHODS: One hundred fifty-eight patients who received a primary tricuspid bioprosthesis because of tricuspid valve anatomy unsuitable for repair between April 1972 and January 1997 were reviewed. Results were analyzed and Kaplan-Meier curves were constructed to estimate patient survival and p

BACKGROUND: Various surgical techniques have been used for correction of Ebstein's anomaly. This paper reports our early and intermediate term results for repair of this disease using the technique initially employed by Carpentier.
PATIENTS AND METHODS: Since December 1987 through December 1991, 13 consecutive patients with Ebstein's anomaly underwent a Carpentier's type of repair. Their mean age was 16.3 years ranging from 6 to 51 years. The indications for operation were advanced NYHA functional class (III-IV) in 4 cases, cyanosis in 4, severe arrhythmia in 3 and cyanosis with arrhythmia in

A case of severe stenosis of a tricuspid bioprosthesis was successfully dilated in a 13 year old young male with Ebstein's anomaly. The patient had congestive heart failure and developed cardiogenic shock; angiography revealed absence of emptying of the right atrium. Multiple size balloons (#4 to 23) were sequentially introduced with successful bioprosthesis dilation and shock resolution. Tricuspid bioprosthesis can be dilated even in critical conditions and, although a palliative procedure, balloon valvuloplasty can be a lifesaving procedure in patients with cardiogenic shock.

To clarify the characteristics of the jugular venous pulse and its genesis in Ebstein's anomaly, 6 patients with Ebstein's anomaly whose mean age was 45 +/- 9 years, and 10 normal subjects with a mean age of 27 +/- 6 years were studied by phono-mechanocardiography and echocardiography. The parameters included the relative height of the jugular "c" wave, the relative timing of the upstroke (Q-Cu interval) and the peak of the jugular "c" wave (Q-Cpeak interval), the relative timing of the upstroke of the carotid artery pulse, the relative timing of tricuspid valve closure (Q-Tc interval), the ex

Among 388 cases of congenital heart disease with magnetic resonance imaging (MRI) from September 1990 to February 1992, we came across two cases of Ebstein's anomaly. They had been previously diagnosed as Ebstein's anomaly by echocardiography and cinecardioangiography. The first case was a three-year-old boy with complex congenital heart disease that included Ebstein's anomaly, a double-outlet right ventricle, pulmonary hypertension, tricuspid regurgitation, mitral regurgitation, a ventricular septal defect and an atrial septal defect. The second was a 13-year-old boy who also had Ebstein's an

Ten consecutive patients (age range 4 to 44 years, mean 22) underwent surgical repair of Ebstein's anomaly by vertical plication of the right ventricle and reimplantation of the tricuspid valve leaflets. No patient died during or after operation. Intraoperative postbypass echocardiography documented a good result in nine patients but severe tricuspid regurgitation in one patient, who then underwent prosthetic valve replacement during a second period of cardiopulmonary bypass. Two of four patients who had had right ventricular papillary muscle dysfunction in the early postoperative period showe

An 18-year-old man visited the outpatient cardiology clinic because of a cardiac murmur found at a medical examination for military service. An Ebstein's anomaly was diagnosed, with tricuspid valve insufficiency. Exposure to lithium in utero was established. Literature reports indicate an association between exposure to lithium in utero and cardiac anomalies, notably Ebstein's anomaly.

The prequisite of better therapeutic results in patients with Ebstein's anomaly of the tricuspid valve is more perfect diagnosis not only of the severity of the malformed valve but also of other associated defects incl. the anomalous accessory bundle of Kent which causes the development of dangerous tachydysrhythmias. The authors present their own experience with the localization of the bundle of Kent by mapping the endocardium of the atria in seven children before a cardiac operation. In all ventricular preexcitation type B with the bundle of Kent in the posterior lower portion of the atrial

The authors report a case of Ebstein's anomaly associated to a ventricular septal defect. It is a rare association which leads to a poor prognosis of the disease. They emphasize the clinical findings and the complementary data that lead to the definite diagnosis.

In 23 patients with Ebstein's anomaly of the tricuspid valve, the functional class of the patients has been related to the echocardiographic parameters generally used to diagnose this disorder. These are the extent of apical displacement of the tricuspid valve and the delay in tricuspid valve closure time related to that of the mitral valve. In addition, the functional class of the patients has also been evaluated in relation to the severity of tricuspid valve insufficiency and to the presence or absence of associated cardiac abnormalities. The study revealed that the echocardiographic paramet

Ebstein's anomaly is a rare congenital malformation of the tricuspid valve, often associated with an atrial septal defect. Death occurs usually from cardiac arrhythmias. The successful use of a two-catheter technique for elective Caesarean section with extradural analgesia is described and the hazards associated with Ebstein's anomaly in pregnancy and anaesthesia are discussed.

We describe 13 cases of Ebstein's anomaly of the tricuspid valve. Seventy percent of there cases were detected at one month of age. Ten patients died and 9 presented associated cardiac anomalies. Signs of right heart failure were the clinical date that made possible the diagnosis of the disease in the most severe cases in the neonatal period. Only two patients had alteration of the cardiac rhythm: One had tachycardia that ended with ventricular fibrillation and the other had a 2:1 heart block. Echocardiography was very useful to establish the diagnosis: one hundred percent of the patients show
