Early Anti-Coagulation after Ischemic Stroke due to Atrial Fibrillation is Safe and Prevents Recurrent Stroke.

BACKGROUND: Patients with acute cardio-embolic stroke from atrial fibrillation (AF) are at risk for recurrence with up to 50% of recurrent stroke occurring within two weeks of the index event. Anti-coagulation with heparinoids within 48 hours of stroke has been shown to increase risk of symptomatic intracranial hemorrhage (ICH) with no clear benefit on early stroke recurrence. METHODS: This study was a retrospective chart review of consecutive patients who were admitted to the stroke service at the Foothills Medical Centre between 2009 and 2011. All patients with an acute stroke with a cardio…

Stroke etiology, thrombus composition, and patient outcomes: A histopathological and clinical perspective.

BACKGROUND: Growing evidence suggests that thrombus composition may influences embolic stroke severity, treatment response, and patient outcomes. While previous studies have characterized histopathological differences between cardioembolic stroke (CES) and atheroembolic stroke (AES), less is known about embolic strokes of undetermined source (ESUS). This study examines the relationship between embolic stroke etiology, thrombus composition, and clinical outcomes to improve classification and guide therapeutic strategies. METHODS: This retrospective study included 323 patients with acute emboli…

Cerebral hemorrhagic infarction at autopsy: cardiac embolic cause and the relationship to the cause of death.

In 48 patients dying within 15 days following a supra-tentorial cerebral infarct, the presence of hemorrhagic infarction at autopsy was related to a cardiac embolic cause of the infarct, and to the cause of death. Hemorrhagic infarcts were more common among patients dying from brain herniation than among those dying from a non-cerebral cause. Cardiac embolic strokes were more often hemorrhagic at autopsy than strokes without such cause; this could be explained by a significant higher rate of brain herniation and death after embolic stroke. On the other hand infarcts with extended hemorrhages m…

Platelet FcγRIIa Expression in Ischemic Stroke: A Marker of Increased Platelet Reactivity.

BACKGROUND: Platelet FcγRIIa amplifies platelet activation and, thus, increased expression identifies patients with increased platelet reactivity. Previous work has demonstrated that platelet FcγRIIa can identify patients at high and low risk of subsequent cardiovascular events after myocardial infarction (MI). This study was designed to compare platelet expression of FcγRIIa in patients with stroke and transient ischemic attack (TIA) with that in patients with a recent MI. METHODS: Patients were enrolled based on an admitting diagnosis of stroke/TIA, and the discharge diagnosis was used to c

Intermittent Interatrial Block in a Patient With Recurrent Transient Ischemic Attacks.

Interatrial block (IAB) is a conduction disorder linked to atrial fibrillation (AF) and ischemic stroke. Intermittent IAB, often triggered by premature atrial complexes (PACs), may precede AF and increase thromboembolic risk. We present a case of embolic stroke of undetermined source (ESUS) with intermittent partial IAB detected on ECG. Serial ECGs and Holter monitoring were analyzed. Intermittent partial IAB with dynamic P-wave changes was observed. No AF or alternative embolic source was identified. Intermittent IAB may indicate atrial vulnerability in ESUS. Recognizing subtle ECG changes co…

Neurologic Complications after Transcatheter Aortic Valve Replacement: An Updated Review of Incidence, Current Treatments, and Future Directions.

Transcatheter aortic valve replacement (TAVR) is the standard treatment for severe aortic stenosis, yet stroke remains a persistent complication with significant morbidity. This review summarizes the incidence, timing, mechanisms, and predictors of TAVR-related stroke, highlighting procedural embolization and patient comorbidities. We examine evidence for cerebral embolic protection devices, pharmacologic strategies, and left atrial appendage closure in mitigating risk. Despite advances in technology, randomized trials have not shown consistent stroke reduction with embolic protection. Optimal

Left atrial appendage strain rate is associated with documented thromboembolism in nonvalvular atrial fibrillation.

BACKGROUND: The left atrial appendage (LAA) strain and strain rate have not yet been studied in the prediction of cardiac thromboembolism. T … el with other conventional TEE predictors were analyzed in terms of their association with previous embolic stroke and peripheral embolization. RESULTS: By comparing the two groups of patients with (22/80) and without embolic events (58/80), patients with embolic events were older, had higher CHA2DS2-VASc scores, higher incidence of coronary artery disease and …

Embolus detection in different degrees of carotid disease.

Carotid atherosclerotic disease is a major source for artery to artery embolism. Stroke incidence highly depends on the degree of carotid stenosis. TCD monitoring for embolic signals may help to identify patients at high risk for an impeding stroke. This study was performed to correlate the degree of carotid stenosis with the occurrence of embolic signals and the clinical outcome. We performed a bilateral simultaneous TCD study on 4 patient groups with unilateral atherosclerotic disease: We studied patients with non-stenotic ICA plaques (n = 21), 50-75% ICA stenosis (n = 20), 75-95% stenosis (

Diagnostic yield of echocardiography in stroke: should we improve patient selection?

INTRODUCTION: Last year the European Society of Echocardiography published recommendations for the use of echocardiography in identifying po … ty of Echocardiography. RESULTS: We found major echocardiographic criteria in 6 patients (5%) with embolic stroke and in 2 (0.7%) non embolic, P=.005. In view of our results, the performance of transthoracic echocardiography in patients with embolic stroke has a low diagnostic yield, which leads us to question systematic use of this technique.

Simultaneously Presented Acute Ischemic Stroke and Non-ST Elevation Myocardial Infarction in a Patient with Paroxysmal Atrial Fibrillation.

Although atrial fibrillation is the most frequent cause of embolic stroke, coronary embolism from atrial fibrillation is a very rare cause of acute myocardial infarction. Therefore, simultaneously presented acute ischemic stroke and acute myocardial infarction due to atrial fibrillation in the same patient has not been documented. The present report describes the case of a 58-year-old man with paroxysmal atrial fibrillation who initially presented with a large cerebral infarction due to embolic occlusion of the left middle cerebral artery. Four hours after the diagnosis of cerebral embolism, h…

Reduced Left Atrial Rotational Flow Is Independently Associated With Embolic Brain Infarcts.

BACKGROUND: Up to 25% of embolic strokes occur in individuals without atrial fibrillation (AF) or other identifiable mechanisms. OBJECTIVES: This study aims to assess whether left atrial (LA) blood flow characteristics are associated with embolic brain infarcts, independently of AF. METHODS: The authors recruited 134 patients: 44 with a history of ischemic stroke and 90 with no history of stroke but CHA2DS2VASc score ≥1. Cardiac magnetic resonance (CMR) evaluated cardiac function and LA 4-dimensional flow parameters, including velocity and vorticity (a measure of rotational flow), and brain …

Isolated asymptomatic pulmonary arteriovenous malformation presenting with ischaemic stroke.

Young onset stroke is uncommon, and may be due to conditions other than traditional vascular risk factors. A 42-year-old woman with an ischaemic stroke was found to have left atrial bubble study positivity on transthoracic echocardiogram (TTE) suggestive of patent foramen ovale, however she also had low peripheral oxygen saturation. Investigation revealed an isolated pulmonary arteriovenous malformation (PAVM), visible on admission chest radiograph. This can cause embolic stroke and is an alternate cause of the TTE findings. The PAVM was able to be closed via endovascular intervention, removin…

Thrombus on ASD Closure Device Reveals Underlying Thrombophilia.

BACKGROUND: Transcatheter closure of atrial septal defects (ASDs) is indicated to prevent paradoxical embolism, and major complications are uncommon. However, even a small risk of thrombus formation can be amplified by an underlying thrombophilia. CASE SUMMARY: A 25-year-old woman with a history of embolic stroke underwent transcatheter ASD closure. Three years later, she presented with another stroke and was found to have thrombus on the closure device. She was started on long-term anticoagulation, and follow-up imaging showed resolution of the thrombus. Further evaluation revealed antiphosp…

[Atrial fibrillation: incidence and prognostic factors of embolic risk].

Atrial fibrillation (AF) carries a high risk of systemic embolism, in particular stroke. This is true not only when AF is associated with rheumatic valvular heart disease, but also in the so-called nonvalvular AF (NVAF). The recent randomized clinical trials assessing antithrombotic therapy as primary prevention in NVAF have shown that, untreated, disabling stroke occurs in 2.5%/year, ischemic stroke in 5%/year, stroke and transient ischemic attack in 7%/year, and stroke, transient ischemic attack and silent stroke in > 7%/year. All AF does not carry identical stroke risk. A risk stratificatio

Signal change of the substantia nigra on diffusion-weighted imaging following striatal infarction.

Diffusion-weighted imaging can depict secondary signal changes of the substantia nigra in patients with ipsilateral striatal infarction. We report four patients who demonstrated obvious signal changes of the substantia nigra in the subacute phase of stroke. Embolic stroke was diagnosed in all of the cases, and none of the patients presented clinical deterioration in their course. Embolic mechanism might be more closely related to the secondary change of the substantia nigra than thrombosis. The relationship between secondary nigral degeneration and stroke etiology or between the nigral lesions…

[Recurrent transient ischemic stroke in a young patient with papillary fibroelastoma of the mitral valve and hyperhomocysteinemia. A case report].

We describe a case of a young patient admitted for recurrent ischemic stroke caused by a papillary fibroelastoma of the mitral valve and a hyperhomocysteinemia. A papillary fibroelastoma is a benign cardiac tumor which can be associated with serious embolic complications. A moderate plasma level of hyperhomocysteinemia is considered as a risk factor of ischemic stroke. The authors suggest that this association increases the risk of ischemic stroke in their patient. The tumor was surgically removed to avoid new embolic events associated with a vitamin B supplementation. After surgery and acid f

Rare Case of Paradoxical Embolism Through Anomalous Systemic Vein-to-Pulmonary Vein Connection: When Zebras Are Zebras.

We report a unique case of a congenital anomalous systemic vein-to-pulmonary vein connection with bidirectional flow causing paradoxical embolic stroke. A 64-year-old woman with no significant medical history presented with a multiterritorial embolic stroke after ankle fracture surgery. Computed tomography (CT) revealed occlusion of the left middle cerebral artery (MCA) and ischemia in the right MCA territory. Successful endovascular clot retrieval was performed, targeting the left MCA occlusion. Extensive investigation identified a rare connection between the right lower pulmonary vein and su…

[Papillary fibroelastoma: a rare but treatable cause of cerebral infarction].

INTRODUCTION: Papillary fibroelastoma is a benign cardiac tumor which can be associated with serious embolic complications. EXEGESIS: We report on a 42-year-old man admitted for an ischemic stroke in the left middle cerebral artery region. Transesophageal echocardiography revealed a mitral valve tumor. Surgical excision and histological examination showed a papillary fibroelastoma. Clinical course was uneventful. CONCLUSION: We consider the high embolic potential of this tumor, which represents a surgically treatable cause of ischemic stroke.
