Further characterization of the DFNA1 audiovestibular phenotype.

BACKGROUND: Autosomal dominant, nonsyndromic, hereditary hearing impairment in a large Costa Rican kindred is caused by a mutation in the hu … frequencies, normal auditory evoked potentials, and electrocochleographic findings consistent with endolymphatic hydrops. The two adults had severe to profound bilateral sensorineural hearing impairment. Electronystagmography disclosed normal vestibular function. Computed tomography demonstrated normal external, middle, and inner ear structures. CONCLUSIONS: These re …

Otosclerosis associated with Ménière's disease: a histological study.

A histological study of a pair of temporal bones was performed in a case of Menière's disease. A severe endolymphatic hydrops and extensive capsular otosclerosis bilaterally was found. Severe endolympathic hydrops results from otosclerotic endolympahtic duct occlusion. Our unique histopathological findings show that a causal association exists between these two entities.

[Surgical treatment of Meniere's disease and its pathophysiological validation].

The authors analyse their own and literature data on surgical treatment of Meniere's disease (MD) as intervention on the endolymphatic sac with assessment of various modifications by long-term results, on sympathetic intraaural and extraaural lesions. Surgical interventions on the endolymphatic sac are pathophysiologically validated. Insufficient knowledge on hydrodynamics in the internal ear labyrinth and the role of endolymphatic hydrops in MD pathogenesis hampers assessment and further development of surgical treatment of the disease. Future lines of the research in Meniere's disease and it…

Herniation of the membranous labyrinth into the horizontal semicircular canal is correlated with impaired caloric response in Ménière's disease.

INTRODUCTION: Canal paresis of the horizontal semicircular canal in Ménière's disease is a frequent finding. Endolymphatic hydrops is the single most characteristic morphologic finding in Ménière's disease. However, the relationship between endolymphatic hydrops and canal paresis is still not elucidated. METHODS: We used magnetic resonance imaging to visualize herniation of parts of the vestibular membranous labyrinth into the horizontal semicircular canal and correlated this morphologic feature with the caloric function of the horizontal semicircular canal. RESULTS: Patients who showed hern…

Comparison of the diagnostic value of 3 T MRI after intratympanic injection of GBCA, electrocochleography, and the glycerol test in patients with Meniere's disease.

CONCLUSION: 3 T MRI after intratympanic injection of gadolinium-based contrast agent (GBCA) is more useful for the diagnosis of endolymphatic hydrops compared with the glycerol test and electrocochleography (ECoG). OBJECTIVE: To investigate the relationship between 3 T MRI after intratympanic injection of GBCA, the glycerol test, and ECoG in patients with Meniere's disease (MD). METHODS: A total of 20 patients with MD were evaluated. Diluted gadodiamide (a gadolinium-based contrast agent) was administered to the bilateral tympanic cavity by injection through the tympanic membrane. After 24 h…

Posterior fossa meningiomas intimately involved with the endolymphatic sac.

Approximately 10% of all intracranial meningiomas occur in the cerebellopontine angle (CPA). Although their clinical presentation depends on their site of origin, most are first seen with nonspecific otologic symptoms like those of acoustic neuroma. We present two cases of CPA meningioma that were intimately involved with the endolymphatic sac and had symptoms and signs suggestive of endolymphatic hydrops.

Perilymphatic fistula and Meniere's disease. Clinical series and literature review.

There continues to be considerable confusion concerning the diagnosis and treatment of perilymphatic fistulas (PLFs) and Meniere's disease. … d a successful outcome. Since it is often difficult to distinguish patients with PLFs and secondary endolymphatic hydrops (Meniere's syndrome) from patients with Meniere's disease (idiopathic endolymphatic hydrops), PLF repair is a reasonable first operation, reserving an endolymphatic sac-mastoid shunt operation as a secondary procedure if needed. High-resolution MRI of the human cochlea.

The majority of magnetic resonance imaging (MRI) studies in neurotology concern the evaluation of retrocochlear pathologies or temporal bone lesions. Recently, quite a number of investigators have attempted to use imaging diagnosis to obtain a diagnosis and manifest the pathological findings of Meniere's disease. However, there is no evidence that endolymphatic hydrops in patients with Meniere's disease can be identified by imaging techniques. In this study we could depict Reissner's membrane clearer than before with the use of image processing. At the present time, we cannot apply the 2.11T M…

Drugs affecting the inner ear. A review of their clinical efficacy, mechanisms of action, toxicity, and place in therapy.

Many drugs have a site of action within the inner ear. The list includes therapeutic, diagnostic and ototoxic agents. Therapeutic agents are most useful in cases of infections, endolymphatic hydrops, vascular insufficiency, vertigo of peripheral origin, autoimmune disease, otosclerosis (otospongiosis), sudden hearing loss and tinnitus. For infections, the most widely used anti-microbial agents are the penicillins and cephalosporins. There are no antiviral agents that have been proven useful for inner ear viral infections. However, steroids have been of some value for controlling some of the se…

Imaging of Ménière's disease after intravenous administration of single-dose gadodiamide: utility of multiplication of MR cisternography and HYDROPS image.

A HYDROPS (HYbriD of Reversed image Of Positive endolymph signal and native image of positive perilymph Signal) image, created by subtracting a positive endolymph image from a positive perilymph image after intravenous injection of single-dose gadodiamide, has been reported to facilitate recognition of endolymphatic hydrops. To further increase the contrast-to-noise ratio (CNR) of HYDROPS images, we multiplied a T2-weighted MR cisternographic image onto the HYDROPS image. Ten patients with suspected Ménière's disease were included. The average CNR of generated images increased to more than 200…

A comparison of dehydration effects of V2-antagonist (OPC-31260) on the inner ear between systemic and round window applications.

V2-antagonist (OPC-31260 (OPC)) application to the scala tympani reduced endolymphatic hydrops. In the present study, we investigated whether systemic administration or local infusion via the round window (RW application) of OPC would be more suitable for clinical use. In Experiment 1, the increase ratios of the cross-sectional area of the scala media of experimentally induced endolymphatic hydrops were quantitatively assessed among four groups of non-OPC application, RW application of xanthan gum, systemic application of OPC and RW application of OPC. In Experiment 2, the effects of systemic …

Current status of surgical decompression and drainage procedures upon the endolymphatic system.

At the present time, neither the etiologic factors nor the pathophysiologic mechanisms that lead to the development of endolymphatic hydrops or Ménière's disease are known; however, the anatomic-pathologic alterations in the labyrinth that are produced by the disease are well-documented. Undoubtedly, what is not known is responsible for the controversial status and questionable benefit that surgical decompression and drainage procedures have upon the endolymphatic system.

The clinical manifestation and treatment of Meniere's Disease from the viewpoint of the water homeostasis of the inner ear.

Endolymphatic hydrops, a pathological feature of Ménière's disease, has been experimentally and clinically confirmed to be influenced by the blood circulation of vasopressin (VP). VP is a well-known hormonal regulator of water homeostasis. In addition, VP is influenced by various environmental changes, dehydration, fluctuation of atmospheric pressure, pregnancy, and other factors. Furthermore, VP is a key regulator of the hypothalamic-pituitary-adrenal (HPA) axis. The HPA axis is a major neuroendocrine system that controls reactions to emotional and physical stresses, as well as the sleep/wake…

[The role of the glycerol test for diagnosis and treatment of Menière disease (author's transl)].

60 patients--25 with different hearing losses and 35 with Menière disease--underwent the glycerol test. It was demonstrated that only the group with endolymphatic hydrops showed in about 60% a positive test result. The meaning of the glycerol test for differential diagnosis of Menière disease from other diseases with attack vertigo is discussed and the role to select the patients for endolymphatic sac surgery is emphasized.

Profound hypokalemia and hypochloremic metabolic alkalosis during thiazide therapy in a child with Pendred syndrome.

Pendred syndrome is a recessive autosomal disorder characterized by thyroid goiter and sensorineural hearing loss. The Pendred syndrome gene (SLC26A4) encodes a new anion exchanger named pendrin which mediates iodide transport by thyrocytes and regulates ion and fluid transport by the endolymphatic sac epithelium. Pendrin defects result in inner ear malformations, with enlargement of the endolymphatic sac and duct in association with a large vestibular aqueduct. Furthermore, patients may develop endolymphatic hydrops requiring diuretic therapy, mainly in the form of thiazides. Pendrin could al…

Submillimeter magnetic resonance imaging of the temporal bone in Meniere's disease.

The pathoetiology of Meniere's disease remains elusive. Histopathologic and imaging studies have implicated congenital or developmental abnormality of the endolymphatic duct as a likely predisposing factor to the development of endolymphatic hydrops and clinical Meniere's disease. Recently, improved high-resolution magnetic resonance imaging (MRI) protocols have allowed better demonstration of the soft tissues of the endolymphatic duct and sac. This study is a prospective evaluation of the ability of submillimeter MRI to detect the endolymphatic duct in Meniere's disease patients and control s…

A preliminary report on the correlation of vestibular Meniére's disease with electrocochleography and glycerol test.

Objective evaluation of vestibular Meniére's disease has been difficult due to the lack of audiometric findings. Thus inevitably, the existence of vestibular Meniére's disease is constantly being challenged. In recent years, electrocochleography (ECochG) has proved to be a useful tool to verify the presence of endolymphatic hydrops electrophysiologically. To further clarify the nature of this disease, we collected 40 patients prospectively who met the following conditions as diagnostic criteria for clinical vestibular Meniére's disease: i) episodic vertigo, ii) aural fullness, and iii) normal …

Effects of Glucocorticoids on the Inner Ear.

Hypothesis: Recently, several lines of evidence have suggested that the inner ear is under hormonal control. It is likely that steroids have some influence on the inner ear. Background: Many clinicians have been empirically using steroids for the treatment of diseases associated with endolymphatic hydrops. The theoretical grounds for this are not clear, and there have been a number of debates on the effectiveness of steroid treatment. Furthermore, there are few reports on histological observations of the influences of steroids on the cochlea. Method: Fifteen guinea pigs (30 ears) were divided …
