Surgical treatment is used only for patients with Menière's disease in cases that do not respond to medical therapy. At the present time, the endolymphatic sac shunt operation is performed in cases where the auditory threshold is fluctuant around 30 dB. When the auditory threshold is between 40 and 70 dB we use the vestibular neurectomy by the retrosigmoid approach. In patients with Menière's disease with profound hearing loss the translabyrinthine neurectomy is used. The advantages of the vestibular neurectomy by the retrosigmoid approach compared to the retrolabyrinthine and middle fossa app…

OBJECTIVE: A low-salt diet has been the main treatment modality for Ménière's disease (MD) since the 1930s, although the mechanisms behind this therapy have not yet been elucidated. Salt reduction is associated with a physiological increase in plasma aldosterone concentration. Several experimental reports have suggested that aldosterone may increase endolymph absorption in the inner ear, particularly in the endolymphatic sac. Therefore, aldosterone elevations due to a low-salt diet may increase endolymph absorption in the endolymphatic sac. In this study, urinary sodium excretion, plasma aldos…

In 1974 we reported on 92 patients who had endolymphatic subarachnoid shunt surgery for idiopathic endolymphatic hydrops. That group had a follow-up of at least 5 years, and only 41% had satisfactory control of vertigo. The purpose of this paper is to describe the results after a larger postoperative follow-up (12 1/2 years, with a range of 68 to 182 months), and to discuss some additional observations regarding endolymphatic sac operations. Two patients have died of unrelated causes, one in the 115th postoperative month and the other in the 124th postoperative month. Eight patients have been …

Von Hippel-Lindau (VHL) disease is a rare, autosomal dominant, multisystem cancer syndrome. A variety of benign and malignant tumors can develop in VHL, including cranial and spinal hemangioblastomas, retinoblastoma, endolymphatic sac tumor, pheochromocytoma, renal cell carcinoma, pancreatic neuroendocrine tumors, and renal, pancreatic, and genital cysts. Here we present a 57-year-old woman who was diagnosed with VHL and, in the same 68 Ga-DOTATATE PET/CT, exhibits all these components of the syndrome: pheochromocytoma, pancreatic neuroendocrine tumor, pancreatic cysts, renal cysts, liver hema…

Meningioma's account for around 15% of all primary brain tumors with some 10% of meningiomas arising in the posterior fossa. In rare cases, a meningioma can form around the endolymphatic sac. When formed in the posterior fossa, meningioma tumors can produce vague, non-specific vertiginous symptoms. Research has observed that a subset of these lesions could produce symptoms indistinguishable from those of Meniere's disease. Therefore, we described the clinical features of a case of posterior petrous meningioma with recurrent vertigo as well as the substantial resolution of symptoms after tumor …

High resolution transverse axial CT encompassing the lateral semicircular canal was used to study the visualization of the vestibular aqueduct and to measure the minimum distance between the posterior semicircular canal and the posterior petrous surface (P-P distance) in Meniere's disease and chronic otitis media. The results indicate that the development of the bone between the posterior semicircular canal and the posterior petrous surface, which usually contains the endolymphatic sac, is significantly poorer in affected ears than in nonaffected ears in unilateral Meniere's disease, and much …

The retrolabyrinthine approach was described several years ago, however its use was restricted to functional surgery of the cerebellopontine angle. Its detractors reproach its narrowness. But a large approach to the cerbellopontine angle can be performed after an extended exposure of the posterior and middle fossa dura, and section of the endolymphatic sac. This approach was used in 5 cases of acoustic neurinomas stage I and II, with normal hearing. Hearing preservation was obtained in 2 cases. We had deafness in 2 cases who had extensive tumor in the bottom of the internal auditory canal.

OBJECTIVES: To evaluate the efficacy of a new device, the Meniett, in the treatment of Meniere's disease. The device delivers pressure pulses to the middle ear through a ventilating tube in the tympanic membrane at a frequency of 6 Hz for 0.6 second. After rising to a pressure level of 1.2 kPa, the pressure oscillates between 0.4 and 1.2 kPa. It is believed that the pressure changes are conveyed to the inner ear, inducing a transport of fluids via the pressure outlets and thus reducing the endolymphatic hydrops.
STUDY DESIGN: A clinical, randomized, multicenter, double-blind, placebo-controll

Tuberculous meningitis (TBM) is a severe presentation of the Mycobacterium tuberculosis that can cause sensorineural hearing loss (SNHL) similarly to other etiologies of meningitis. The pathophysiology for this group remains unclear. We report a series with 3 temporal bones of infants who died from TBM, showing acute damage to the inner ear, such as fibrosis of the cochlea, organ of Corti atrophy, neuronal loss and internal auditory canal with abundant pus cells, predominantly mononuclear. The cochlear aqueduct, endolymphatic sac/duct and Rosenthal's canal appear to be routes of dissemination …

This report describes the case history of two male siblings with sensorineural hearing loss and an enlarged vestibular aqueduct (EVA). Sibling 1 presented with a history of intermittent self-limiting ataxia and hearing loss at the age of 25 months and sibling 2 presented with a similar history at the age of 18 months. MRI showed an enlarged endolymphatic duct and sac bilaterally in both children. Perchlorate discharge tests were positive in both infants leading to a diagnosis of Pendred syndrome. A number of conditions associated with EVA are discussed with a view to devising management strate

Animal models of vascular disorders are identified or developed for the evaluation of functional deficits and morphologic alterations. This information will serve a useful purpose for a better understanding of sudden deafness, Meniere's disease, and presbycusis. The study of microcirculation of the inner ear vessels reveals that their responses to various stimuli, such as anoxia, sympathetic nerve stimulation, hypothermia, and drugs, are different from those of the middle ear vessels. In sudden occlusion of the major vascular supply to the inner ear, the cochlea is found to be more vulnerable

A review of the recent literature concerning hearing loss and Meniere's disease is presented. A compilation of the change in hearing resulting from endolymphatic sac surgery, vestibular nerve section, and intratympanic gentamicin injection and a summary of the results of pharmacologic interventions is tabulated for 1986 to 1996. Although vertigo control is variable with the above treatment strategies, hearing loss does not appear altered by any of them when compared with the natural history of hearing loss associated with Meniere's disease. Further investigation is necessary to understand the …

Two patients complained of severe vertigo following eructation or hiccup. One patient was poststapedectomy; the other had idiopathic cochlear degeneration. The symptoms were reproduced by Hennebert's test in the pathologic ear. The tensor tympani was transected in both patients. An endolymphatic sac procedure was performed concomitantly in the second patient. Both patients had immediate relief of symptoms. It is hypothesized that these reflex phenomena, which elicited vertigo, were accompanied by tensor tympani contraction, which resulted in medial movement of the stapes or prosthesis, and tha…

The sensitivity of different MRI sequences in the detection of inner ear malformations in patients presenting with sensorineural hearing loss (SNHL) and/or vertigo was evaluated. We studied 650 patients presenting with SNHL and/or vertigo, clinically not suspected of having inner ear malformations. The sensitivity of T1-weighted, Gd-enhanced T1-weighted and (when available) T2-weighted spin-echo images, and three-dimensional Fourier transformation-constructive interference in steady state (3DFT-CISS) gradient-echo images, to unexpected malformations was assessed. Inner ear malformations were f

OBJECTIVE: Intact-canal-wall mastoidectomy procedures leave an unsightly depression in the postauricular area. Until now, there have been few reports of successful reconstruction of mastoidectomy defects, and none using titanium mesh. When secondary mastoidectomy is not anticipated, as in endolymphatic sac shunt procedures, the postauricular defect resulting from mastoidectomy can be eliminated by reconstruction using titanium mesh. This is a retrospective study of 14 patients who underwent reconstruction of a mastoidectomy defect with titanium mesh.
