Evidence for astrocytes as a potential source of the glutamate excess in temporal lobe epilepsy.

Increased extracellular brain glutamate has been implicated in the pathophysiology of human refractory temporal lobe epilepsy (TLE), but the cause of the excessive glutamate is unknown. Prior studies by us and others have shown that the glutamate degrading enzyme glutamine synthetase (GS) is deficient in astrocytes in the epileptogenic hippocampal formation in a subset of patients with TLE. We have postulated that the loss of GS in TLE leads to increased glutamate in astrocytes with elevated concentrations of extracellular glutamate and recurrent seizures as the ultimate end-points. Here we te

[Changes in cerebral perfusion induced by etomidate in patients with temporal lobe epilepsy].

INTRODUCTION: Epilepsy is one of the major neurological disorders, affecting roughly 0.5-2% of the world's population and approximately 20-25% of patients are resistant to medication. AIM: To analyze the response of cerebral perfusion (assessed by SPECT) and bioelectrical activity (measured in scalp and mesial temporal region) to etomidate. PATIENTS AND METHODS: We studied 10 patients presurgically evaluated and studied by video-EEG with foramen ovale electrodes (EFO) and SPECT. Etomidate was administered (0.1 mg/kg), followed by (99)mTc-HmPAO during the study in the video-EEG + EFO. RESULT

Sleep but not hyperventilation increases the sensitivity of the EEG in patients with temporal lobe epilepsy.

PURPOSE: To evaluate the relative impact of 3 and 5 min of hyperventilation (HV) and different sleep stages on the sensitivity of the interictal EEG in focal epilepsy. METHODS: We examined 20 patients with temporal lobe epilepsy (TLE, 85%) or extratemporal epilepsy during EEG-monitoring. We compared 6 min EEG (12 epochs of 30s) during/after each: (a) waking; (b) 5 or 3 min of HV; (c) sleep stages 1, 2, 3/4 and REM regarding the frequency of epileptiform discharges (ED). The Wilcoxon matched pairs signed rank test was used. The main endpoint was the comparison of 5 min of HV with sleep stage 2

Amygdala enlargement in dysthymia--a volumetric study of patients with temporal lobe epilepsy.

BACKGROUND: Previous studies indicated an important role of the amygdala for emotional information processing. We investigated a possible relationship between amygdala volumes, aggressive behavior, and dysthymia, in patients with temporal lobe epilepsy (TLE). METHODS: Patients with TLE with and without aggression or dysthymia and healthy volunteers were assessed using quantitative MRI. Amygdala volumes were measured in a blinded fashion and corrected for total brain volumes. RESULTS: There was a highly significant enlargement of left and right amygdala volumes in patients with dysthymia (rig

Comparing Ictal Cardiac Autonomic Changes in Patients with Frontal Lobe Epilepsy and Temporal Lobe Epilepsy by Ultra-Short-Term Heart Rate Variability Analysis.

Background and Objectives: Abnormal epileptic discharges in the brain can affect the central brain regions that regulate autonomic activity and produce cardiac symptoms, either at onset or during propagation of a seizure. These autonomic alterations are related to cardiorespiratory disturbances, such as sudden unexpected death in epilepsy. This study aims to investigate the differences in cardiac autonomic function between patients with temporal lobe epilepsy (TLE) and frontal lobe epilepsy (FLE) using ultra-short-term heart rate variability (HRV) analysis around seizures. Materials and Method

Alumina gel injections into the temporal lobe of rhesus monkeys cause complex partial seizures and morphological changes found in human temporal lobe epilepsy.

The goal of the present study was to determine whether alumina gel injections into temporal lobe structures cause complex partial seizures (CPS) and pathological changes observed in human temporal lobe epilepsy. Rhesus monkeys with alumina gel injections in the amygdala, perirhinal and entorhinal cortices, or Ammon's horn and dentate gyrus all initially displayed focal pathological electroencephalographic (EEG) slowing limited to the site of injection. After clinical seizures developed, they also displayed widespread pathological EEG slowing over both hemispheres, interictal and ictal epilepti

Voxel-based T2 relaxation rate measurements in temporal lobe epilepsy (TLE) with and without mesial temporal sclerosis.

INTRODUCTION: Quantitative measurements of T(2) relaxation in the hippocampus for focus lateralization in mesial temporal lobe epilepsy (mTLE) are well established. Less is known to what degree such relaxation abnormalities also affect regions beyond the ipsilateral hippocampus. Therefore, the aim of this study was to characterize extent and distribution pattern of extrahippocampal relaxation abnormalities in TLE with (TLE-MTS) and without MRI evidence of mesial-temporal sclerosis (TLE-no). METHODS: Double spin echo images (TE1/2: 20/80 ms) acquired in 24 TLE-MTS and 18 TLE-no were used to ca

Facial emotion recognition in temporal lobe epilepsy: Behavioral performance, reaction time and alexithymia.

OBJECTIVE: Facial emotion recognition (FER) deficits are well documented in temporal lobe epilepsy (TLE), but most studies have focused on recognition accuracy, with limited attention to reaction time (RT) and its relationship with alexithymia. This study aimed to compare FER accuracy, RT, and alexithymia levels between patients with TLE and healthy controls (HCs) and to examine associations between FER performance and alexithymia. METHODS: Twenty-two patients with unilateral TLE and 30 age-, sex-, and education-matched HCs were included. FER accuracy and RTs were assessed using a standardize

Language network reorganization in temporal lobe epilepsy: insights from task-based functional MRI.

OBJECTIVE: In temporal lobe epilepsy (TLE), recurrent seizures can cause structural and functional disruptions within the temporal lobe and nearby language regions, resulting in neural reorganization. In this study, we leverage task-based functional magnetic resonance imaging (tb-fMRI) to understand how this language reorganization process varies based on the seizure location, the timing of seizure onset, and the chronicity of epileptic activity. METHOD: 84 drug-resistant TLE patients who completed a tb-fMRI sentence completion task were included in this study. Analysis included group compari

Small sharp spikes precede the development of epileptiform discharges in temporal lobe epilepsy.

Small sharp spikes (SSS), traditionally regarded as benign EEG variants, have increasingly been linked to temporal lobe epileptogenicity. However, their potential role as early EEG biomarkers of temporal lobe epilepsy (TLE) remains unclear. We analyzed 11 patients with or without a prior epilepsy diagnosis whose initial EEGs demonstrated isolated SSS in the absence of interictal epileptiform discharges (IEDs). The cohort included 8 females and 3 males with a median age of 35 years (IQR 26-43). Four patients had a diagnosis of epilepsy at presentation, whereas seven did not. Over a median follo
