The value of early postoperative bone scan in slipped capital femoral epiphysis.

The purpose of this study was to evaluate the sensitivity and predictive value of early postoperative bone scan for detection of avascular necrosis (AVN) of the femoral head after surgical treatment of slipped capital femoral epiphysis. We reviewed records of 49 patients (64 hips) operated on between 1980 and 1997 with a mean follow-up of 3 years. Sixty-one out of 64 hips went through an early postoperative bone scan. The three hips that developed AVN showed significant loss of radionuclide uptake. There were neither false-positive or false-negative cases in this series. Early postoperative bo

Sequential magnetic resonance imaging in slipped capital femoral epiphysis: assessment of preslip in the contralateral hip.

Thirty-three images using magnetic resonance imaging (MRI) in 10 patients with slipped capital femoral epiphysis were evaluated. The MRI of affected hips delineated typical widening of the physis. Sequential MRI demonstrated the process of premature closure of the physis developing from the posterior portion anteriorly. In 4 of the 10 asymptomatic contralateral hips, physeal widening was clearly observed in the center or posteromedial region of the physis on T1-weighted images. We performed prophylactic pinning for the four cases showing physeal widening on MRI, and none of the other cases dev

Current concepts in the management of pediatric hip disease.

A wide variety of disorders affects the pediatric hip. The most common of these are developmental dysplasia of the hip, Legg-Calvé-Perthes disease, and slipped capital femoral epiphysis. Magnetic resonance imaging and ultrasound improve the imaging of these diseases, but the ultimate impact of this technology on treatment is unknown. Complications of treatment for these and other pediatric hip diseases can be devastating; so much recent research concentrates on the etiology, prevention, and effects of these complications. Finally, these diseases occur in skeletally immature individuals, and lo

[Consideration of the legitimacy for connecting the opposite femoral head and neck in juvenile slipped capital femoral epiphysis].

Forty seven patients were treated because of slipping of the capital femoral epiphysis, in 23 patients (48.9%) both hips were involved. In one third of these cases both hips were diagnosed at the same time, in one third second hip was diagnosed from 6 to 24 months later, in one third only late diagnosis of juvenile symptom free--thus the late diagnosis. In 18 hips slipping was less than 30 degrees, in 5 hips more than 30 degrees. On the basis of authors own experience and review of literature the conclusion was made to fix with K--wires epiphysis of the opposite hip as a prophylaxis against sl

An ultrastructural study of slipped capital femoral epiphysis: pathogenetic considerations.

Two normal proximal femoral growth plates and core biopsies from six patients with slipped capital femoral epiphysis (SCFE) were studied by electron microscopy. In these SCFE patients, chondrocytes from all the zones of the plate were frequently smaller than normal, more irregular in shape, and many of them were degenerating, with formation of matrix vesicles and cellular debris. Floccular electron-dense material, most likely abnormal proteoglycan, was present in the hypertrophic rough-surfaced endoplasmic reticulum and Golgi apparatus as well as in the extracellular matrix, intermingled with

[Radiometry of epiphyseolysis capitis femoris. A comparison of conventional roentgen study with axial computerized tomography].

CT scans are proposed for the measurement of tibial, femoral and humeral torsion. The value of computerised tomography in the evaluation of slipped capital epiphysis is yet not discussed. Conventional roentgenographic studies have shown that the capital femoral epiphysis slips by rotating backwards. In a study of 5 cases with a right site slip of the capital femoral epiphysis of different extent a comparison of conventional radiography with CT-scan was undertaken. True lateral films of both hips were obtained at the same time when CT scan of the femoral head and the condylar region of the femu

Chondrolysis complicating slipped capital femoral epiphysis.

In a detailed study of 249 patients, 329 cases who had slipped capital femoral epiphysis treated at the Campbell Clinic during the period from 1935 to 1973, chondrolysis was found in 79 hips in 71 patients (28.9%). The incidence of chondrolysis is correlated with the number of factors, including sex, race, chronicity and severity of displacement, and method of treatment. On the basis of biopsy observations upon 23 patients who had chondrolysis, the natural history of the disease includes self-perpetuating synovitis, and progresses with abnormal mechanical stress on the articular cartilage and

Chondrolysis of the hip (idiopathic and secondary forms).

The writers describe one case of idiopathic chondrolysis of the hip and four cases of chondrolysis secondary to slipped upper femoral epiphysis, all followed up from two to six years. So far as the writers know, idiopathic chondrolysis has not previously been reported in the literature in a male of the white race after the end of skeletal growth. In this case biopsy disclosed necrosis and fibrosis of the articular cartilage with extensive infiltration of lymphocytes and plasma cells in the synovial membrane and in the subchondral medullary spaces. In all the cases described there was gradual r

Slipped capital femoral epiphysis with hypothyroidism treated by nonoperative method.

Patients receiving growth hormone or thyroid hormone are likely to develop a slipped capital femoral epiphysis during the period of spurt in growth. While pin fixation of these hips has been the popular method of treatment, it is possible to treat Grade I slips successfully by nonoperative methods provided that protection from weight-bearing is maintained until fusion of the epiphysis is complete.

[Late results of Imhäuser's osteotomy for slipped capital femoral epiphysis (author's transl)].

The author has reviewed the clinical and radiological results of 55 patients 11 to 22 years following an Imhäuser three dimensional intertrochanteric osteotomy for slipped capital femoral epiphysis. 13 additional patients responded by questionnaire. Of these 68 hips 66 were painfree and functional. One patient complained of pain (due to coxarthrosis proven by x-ray). A second patient was dissatisfied with the functional range of his hip. 40 of the 55 hips which were examined clinically and radiologically had full range of motion, 10 hips showed minimal and 5 hips significant limitation of moti

Intracapsular cuneiform osteotomy compared with in-situ pinning for the management of slipped capital femoral epiphysis.

Intracapsular cuneiform osteotomy was initially introduced to restore the morphology of the proximal femur after slipped capital femoral epiphysis (SCFE). However, whether this procedure results in a higher risk of avascular necrosis (AVN) or lower incidence of cam deformity than in-situ pinning is unclear. The aim of this study was to compare the outcomes of intracapsular cuneiform osteotomy and in-situ pinning to treat SCFE in children. Twenty-three children who suffered from SCFE underwent either intracapsular cuneiform osteotomy (eight patients, eight hips) or in-situ pinning (15 patients,

Comparison of different radiographic methods to measure the slip angle in children with slipped capital femoral epiphysis (SCFE).

BACKGROUND: The management of patients with slipped capital femoral epiphysis (SCFE) requires imaging diagnostics of good quality and accurate measurement of the degree of slippage. In Sweden, three different radiological methods are commonly used: the calcar femorale method; the Billing method; and the Head-shaft angle described by Southwick. PURPOSE: To evaluate whether any of the three most common methods used in Sweden to measure the slip angle was more useful and reproducible than the others. MATERIAL AND METHODS: Two experienced orthopaedists measured the slip angle in preoperative hip

Prolonged Length of Stay and Readmissions Following In Situ Pinning of Slipped Capital Femoral Epiphysis An Analysis of Incidence and Predictive Factors.

BACKGROUND: In situ pinning is one of the primary treat-ments for slipped capital femoral epiphysis (SCFE) in pedi-atric patients, many of whom have multiple comorbidities. Despite the fact that SCFE pinning is a frequently performed procedure in the United States, little is known regarding sub-optimal postoperative outcomes in this patient population. The purpose of this study was thus to identify the incidence, perioperative predictors, and specific causes of prolonged hospital length of stay (LOS) and readmissions following fixation. METHODS: The 2016-2017 National Surgical Quality Im-prov

Public Insurance and Single-Guardian Households Are Associated with Diagnostic Delay in Slipped Capital Femoral Epiphysis.

BACKGROUND: Extensive literature documents the adverse sequelae of delayed diagnosis of slipped capital femoral epiphysis (SCFE), including worsening deformity and surgical complications. Less is known about predictors of delayed diagnosis of SCFE, particularly the effects of social determinants of health. The purpose of this study was to evaluate the impact of insurance type, family structure, and neighborhood-level socioeconomic vulnerability on the delay of SCFE diagnosis. METHODS: We reviewed medical records of patients who underwent surgical fixation for stable SCFE at a tertiary pediatr

Shedding light on slipped capital femoral epiphysis: a nationwide study on Turkish population.

OBJECTIVES: This comprehensive study aimed to describe the epidemiologic and demographic distribution of slipped capital femoral epiphysis (SCFE) in Türkiye, a condition that globally presents at a rate of 10.8 cases per 100 000 children. Utilizing data from the Ministry of Health, we examined the specifics of SCFE in Türkiye, comparing the findings with those of other populations to clarify the country's unique epidemiological profile. METHODS: In this retrospective analysis, the Ministry of Health's database was used to extract medical records of children under the age of 16 diagnosed with

Screw Thread Configuration Has No Effect on Outcomes of In Situ Fixation for Stable Slipped Capital Femoral Epiphysis.

BACKGROUND: No consensus exists regarding the optimal surgical management of slipped capital femoral epiphysis (SCFE). Treatment goals include avoiding slip progression and sequelae such as avascular necrosis (AVN). Factors associated with surgical implants merit further research. This study investigates the effect of screw thread configuration and the number of screws on surgical outcomes. METHODS: A total of 152 patients undergoing cannulated, stainless steel, in situ screw fixation of SCFE between January 2005 and April 2018 were included. Procedure laterality, screw number and thread conf
