This is a retrospective analysis of the clinical and radiological outcome in 11 patients with complex acute posttraumatic elbow instability after dislocation. These patients had also been treated with a hinged external fixator after open reduction, capsular and ligamentous reconstruction and internal fixation, because of an expected diminished compliance, to avoid a secondary dislocation of the internal fixation. Concentric stability and a sufficient range of motion of the elbow joint were achieved in all cases. Non-compliant patients were classified by the surgeon as not compliant or not able

The surgical management of ankle fractures among the diabetic population is associated with higher complication rates compared to the general population. Efforts toward development of better methods in prevention and treatment are continuously evolving for these injuries. The presence of peripheral neuropathy and the possible development of Charcot neuroarthropathy in this high risk patient population have stimulated much surgical interest to create more stable osseous constructs when open reduction of an ankle fracture/dislocation is required. The utilization of multiple syndesmotic screws (p

BACKGROUND: Gustilo IIIB fractures involve high-energy tibial fractures for which there is inadequate soft tissue coverage. In addition to orthopedic fixation, these injuries require soft tissue reconstruction, often in the form of a microvascular free flap. Although the majority of orthopedic literature favorably compares intramedullary rod fixation to external fixation in open tibial fractures, these studies have not focused on the role of either method of fixation in relation to the soft tissue reconstruction.
METHODS: Because we had noted numerous complications after providing free-flap c

Traffic accidents are the leading cause of traumatic fractures worldwide, with tibial fractures being the most common lower-extremity injury. Open tibial fractures pose significant clinical challenges due to their high risk of infection and non-union, requiring effective stabilisation. External fixators are widely used for this purpose, but their biomechanical performance must be evaluated under both standardised and patient-specific conditions. This study presents a finite element analysis of an external fixator tailored to Indonesian patients, using two frameworks: the ASTM F1541-02 standard…

Two juvenile small mammals, 1 ferret (Mustela putorius furo) and 1 rabbit (Oryctolagus cuniculus), were diagnosed respectively with a humeral and a femoral fracture. A safety pin tie-in, a type I stainless steel fixator without proximal transfixation pin, was used to stabilize the fracture in each individual. Changes to clinical examination findings, including lameness and bone callus palpation, as well as radiographic images, were followed. Complete osteosynthesis and implant removal were achieved in both cases after 4 wk. Both cases were reexamined 12 mo after surgery and both had recovered

BACKGROUND: Recurrent clubfoot (CF) remains a challenging orthopedic condition, often requiring surgical intervention due to deformity rigidity and scarring from previous treatments. External fixation, particularly the Ilizarov technique, has emerged as a promising approach to correct recurrent and complex CF deformities. However, there is considerable variability in reported results regarding success and recurrence rates.
OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the efficacy and safety of external fixation with a focus on success rates, recurrence rates, and comp

INTRODUCTION: The acromioclavicular (AC) ganglion cyst first described by a shoulder arthrogram by Craig in 1984, still remains an uncommon presentation of a shoulder pathology. Classified as a type one cyst by Hiller in the presence of a rotator cuff pathology, the management of these lesions is governed by ill-defined guidelines. Literature shows the variability of surgical management ranges from simple cyst excision and lateral end clavicular excision to reverse shoulder arthroplasty. Type two cysts are usually benign ganglion cysts, filled with mucinous putty material, and arise from the A

The midfoot is the most frequent location of foot involvement in Charcot neuroarthropathy (CNA), and there is heterogeneity in clinical presentations depending on the joints involved and degree of bone destruction The present review encompasses the current indications and an algorithm for the management of minimally invasive correction of CNA midfoot deformities in both active and inactive settings using multiplanar circular external fixation. The latter is part of a 2-step procedure, including arthrodesis with intramedullary beaming, to maintain the achieved 3-dimensional correction.

CASE: A 41-year-old male presented with an insidious onset of pain and swelling about the dorsal wrist, and was found to have a Brodie's abscess in the distal radius. The patient had a history of a distal radius fracture, treated with external fixation, nineteen years prior, which we believe contributed to the infection. The patient was treated surgically with abscess irrigation, debridement, bony curettage, bioactive glass S53P4 allograft, with concurrent antibiotic therapy.
CONCLUSION: Brodie's abscesses can have atypical presentations, and a thorough history must be obtained from patients

BACKGROUND: Evidence concerning the complex foot trauma, especially its definitive management, is scarce. Soft tissue envelope sequalae are the primary parameters that delay or make internal fixation implausible. Stability conferred by external fixators makes them a reasonable initial treatment choice. Although AO or circular fixators can be applied around the foot, this can involve a learning curve and substantial costs, especially for the circular fixator. There is little evidence as to how well external fixators work as a definite method of fixation in patients where progression to internal…

INTRODUCTION: The therapeutic management of calcaneal fractures is currently a major source of controversy in the literature. There is no consensus on the need to treat these injuries conservatively or surgically, nor on the criteria for deciding one option or the other. Although the gold standard has classically been the open approach and osteosynthesis, there are currently minimally invasive techniques that also report good results. Our objective is to present our results and experience with the MBA® Orthofix external fixator in a series of cases of calcaneal fractures.

Joint-spanning external fixation in acute ankle trauma is a means to provide temporary stability and restoration of length, alignment, and articular congruency. This allows for soft-tissue consolidation before definitive fixation to decrease the risk of wound complications. Traction is commonly used during definitive fixation to aid in fracture reduction and to maintain reduction during placement of internal hardware. Ankle arthroscopy for ankle fractures is also becoming increasingly popular to identify and treat intra-articular injury and uses traction for visual assistance and increased wor

OBJECTIVES: External skeletal fixation is an established technique in cats for biological fixation of long bone fractures, stabilisation of the joints, and treatment of shearing injuries and angular deformities. As appropriate and accurate pin insertion is imperative for a successful outcome, knowledge of topographic anatomy and areas that are safe (safe corridors) for pin placement is integral to successful surgery. At present, however, safe corridors have not been determined fully in feline orthopaedics, with surgeons having to rely on knowledge based on canine orthopaedics. This study was p
