[Miller Fisher syndrome: report of a case].

The authors report a case of a 27 year old male patient who has exhibited external ophthalmoplegia, anisocoria, ataxia, absent deep reflexes, dysphagia, dysphonia, bilateral peripheral facial paralysis, hypopallesthesia and a mild sensory impairment of the four extremities. In addition, his cerebrospinal fluid showed albuminocytologic dissociation. The otoneurologic examination showed important findings. These symptoms began ten days after a "cold", and quickly subsided after prednisone therapy. Two months after the onset of clinical features, just a deep apendicular areflexia was detected on

The prevalence of Raynaud's syndrome in rheumatoid arthritis.

To test the validity of the putative association between Raynaud's syndrome and rheumatoid arthritis (RA) 277 patients with rheumatic disorders were questioned about cold-induced colour reactions in the extremities. The time to recover digital temperature after brief immersion in ice water was used to confirm the presence of Raynaud's syndrome. The prevalence of Raynaud's syndrome in the 148 patients with RA and the 59 patients with osteoarthritis was 2.7% and 5.1% respectively. It is concluded that Raynaud's syndrome is not a frequent accompaniment of RA, and it is suggested that in view of t

Seasonal migration and healthy ageing in China: protocol for the Frigid-Tropical Migratory Population Health (ftMPH) cohort.

INTRODUCTION: As populations age and climate variability intensifies, seasonal migration has emerged as a strategy among older adults to reduce exposure to climatic extremes and optimise living conditions. In China, millions of older adults migrate annually between cold northern and tropical southern regions, providing a natural setting to examine the health effects of seasonal migration. The Frigid-Tropical Migratory Population Health (ftMPH) cohort was established to assess the short- and long-term health effects of this migration and the underlying biological, behavioural and social pathway

Maternal air temperature exposure and risk of small vulnerable newborn: Identifying critical windows.

BACKGROUND: Small vulnerable newborn (SVN), defined as preterm birth (PTB), small for gestational age (SGA), or low birth weight (LBW), remains a global health challenge. While prenatal temperature exposure may influence pregnancy outcomes, existing trimester-level analyses lack temporal precision to detect sensitive gestational windows. This population-based study aims to explore the relationship between temperature and SVN over time and identify critical exposure periods with weekly resolution. METHODS: Using a retrospective cohort of 1,129,572 singleton births (2011-2022) in Wuhan, China,

Infrared thermography in children: a reliable tool for differential diagnosis of peripheral microvascular dysfunction and Raynaud's phenomenon?

BACKGROUND: Infrared Thermography (IRT) has been used for over 30 years in the assessment of Raynaud Phenomenon (RP) and other peripheral microvascular dysfunctions in adults but, to date, very little experience is available on its use in children for this purpose. The first aim of the study was to assess reproducibility of thermographic examination after cold exposure by comparing inter-observer agreement in thermal imaging interpretation. The secondary aim was to evaluate whether IRT is reliable to diagnose and differentiate peripheral circulation disturbances in children. METHODS: Children

Efficacy of an Infrared Radiator for Hypothermia Prevention in a Simulated Setup of Entrapped Vehicle Accident Victims.

BACKGROUND: Prolonged extrication of entrapped patients after road traffic accidents increases the risk of sustained hypothermia. Accident-related hypothermia increases mortality in severely injured patients, and prehospital efforts to prevent hypothermia are essential. We evaluated various warming measures regarding their preclinical suitability and efficacy for patient warming, tested in realistically-simulated road traffic accident scenarios under cold ambient conditions in a climate chamber. METHODS: The effects of a chemical warming blanket (CWB), forced-air warming (FAW) device, or infr

Pulse oximetry: a useful test for evaluating pulp vitality in traumatized teeth.

BACKGROUND AND AIM: Subjective pulp tests are not trustworthy, particularly in traumatized teeth, and may lead to inaccurate diagnosis. The use of an objective test such as pulse oximetry (PO) could be a more reliable method to properly evaluate pulp status in this condition. The aim of this study was to analyze the effectiveness of PO in determining pulp vitality in traumatized teeth based on oxygen saturation measurements (%SpO2 ). SUBJECTS AND METHODS: Fifty-nine permanent teeth that had undergone lateral luxation, and which were unresponsive to a cold spray test and were free from signs o

Severe Hypothermia and Frostbite Requiring ECMO and Four Limb Amputations.

Severe hypothermia and frostbite can result in significant morbidity and mortality. We present a case of a patient with severe hypothermia and frostbite due to cold exposure after a snowmobile crash. He presented in cardiac arrest with a core temperature of 19°C requiring prolonged cardiopulmonary resuscitation, active internal rewarming, venoarterial extracorporeal membrane oxygenation, and subsequently amputations of all four extremities. Although severe hypothermia and frostbite can be a fatal condition, the quick action of Emergency Medical Services, emergency physicians, trauma surgeons,

Infrarenal Aorta Thrombosis Associated with H1N1 Influenza A Virus Infection.

Influenza viruses are members of the Orthomyxoviridae family, of which influenza A, B, and C viruses constitute three separate genera. Arterial thrombosis associated with H1N1 influenza A virus infection has rarely been reported. A Turkish man aged 28 years was admitted to our emergency department with dyspnea, bilateral lower extremity insensitivity, and cold. He reported symptoms of fever, myalgia, and cough, which he had had for fifteen days before being admitted to our hospital. The patient was tested for pandemic influenza A (H1N1) virus using polymerase chain reaction (PCR) tests, which

Limb amputation in hemoglobin SC disease after application of ice and elevation.

A 31-year-old man with hemoglobin SC disease was admitted with acute chest syndrome, treated with antibiotics and ventilator support. His restrained right wrist and hand became swollen and cool, thought due to infiltration of an intravenous line. The hand was elevated, ice packs applied around the clock, and prophylactic low molecular weight heparin given. The arm quickly turned gangrenous. In spite of attempted thrombectomy and red blood cell exchange transfusion, amputation was required. Cold, elevation, and measures that reduce blood flow are known precipitators of sickling and crises. Vaso

Fine structural aspects on the fate of rat black thyroids induced by minocycline.

The fate of the black thyroid induced by minocycline treatment (100 mg/kg daily for 21 days) in the rat was studied by light and electron microscopy after 6 months. The black discoloration of the thyroid gland remained and numerous dense bodies containing highly electron-dense deposits were seen in most of the follicular epithelial cells. It appears that the turnover rate of follicular epithelial cells is very low and the electron-dense deposits are largely retained, though debris derived from degenerate follicular epithelial cells containing the dense deposits may be phagocytosed by macrophag

Peripheral neuropathy with sympathetic overactivity from industrial contact with acrylamide.

This paper describes the experimental and clinical findings of acrylamide intoxication in a human being. It is believed that this is the first such case to be recorded in the medical literature.Acrylamide is widely used as a "chemical grout". It is pumped into dirt, clay and stone walls of excavations in a liquid state together with a catalyst, and it then polymerizes to make a watertight shield. This chemical is neurotoxic in its non-polymerized form and can be absorbed through the intact skin, mucous membranes and lungs. In spite of warnings with regard to its handling, this worker became car

Esophageal cooling for protection during left atrial ablation: a systematic review and meta-analysis.

PURPOSE: Thermal damage to the esophagus is a risk from radiofrequency (RF) ablation of the left atrium for the treatment of atrial fibrillation (AF). The most extreme type of thermal injury results in atrio-esophageal fistula (AEF) and a correspondingly high mortality rate. Various strategies for reducing esophageal injury have been developed, including power reduction, esophageal deviation, and esophageal cooling. One method of esophageal cooling involves the direct instillation of cold water or saline into the esophagus during RF ablation. Although this method provides limited heat-extracti

Extreme temperature and airway dehydration: current understanding and integrative insights into respiratory vulnerability.

INTRODUCTION: Climate change has significantly increased the frequency and intensity of extreme temperature events, posing growing threats to respiratory health. Among the underlying mechanisms, airway dehydration is a critical yet underrecognized pathway that disrupts the airway surface liquid (ASL), which is essential for mucociliary clearance, epithelial integrity, and immune defense. AREAS COVERED: This article examines evidence linking airway dehydration to respiratory disease during temperature extremes. We discuss how hot, dry air and cold, low-humidity conditions disrupt airway hydrat

Challenges in the surgical treatment of neuroma in continuity in the upper extremity using human acellular nerve allografts.

The restoration of nerve function after the injury might be complicated by the development of a disorganized fibrous mass-a neuroma. This results in sensory and/or motor deficits and pain that can be severely debilitating. Surgical excision of the painful neuroma may leave a gap, which can be bridged using autografts or allografts. The main objectives of this study were to obtain 1-year clinical results in patients who underwent excision and reconstruction of a painful neuroma in continuity using decellularized allografts after nerve lesions in the upper extremity. In a prospective cohort stud

Epidemiological and clinical comparisons of paediatric and adult sporotrichosis in Jilin Province, China.

Sporotrichosis is endemic in Jilin Province of Northeast China. While paediatric cases make a substantial contribution to the epidemiological profile of sporotrichosis, the differences in the epidemiology and clinical manifestations of sporotrichosis between paediatric and adult patients remain unclear. We retrospectively reviewed the clinical records of 2968 cases of sporotrichosis (2113 adult patients aged ≥ 15 years and 855 paediatric patients aged < 15 years) over a nine-year period (01/01/2010-31/12/2018-). All the patients were diagnosed with sporotrichosis based on fungal culture of mat
