[Actinomycosis--a multidisciplinary approach to a clinical problem].

Actinomycosis is caused by microorganisms of the Actinomyces species, found in the oral cavity, gastrointestinal and genital tract. Infection by Actimomyces species mainly causes chronic supurative disease of the oral, cervico-facial, thoracic, and abdominal regions. Clinical manifestations have been described at every level, but the disease is relatively rarely diagnosed. The causative agent is considered to be an opportunistic pathogen associated with infection, trauma, surgical procedures, or associated with a neoplasm or foreign body. Symptoms include intermittent fever, weight loss, abdom

A case of multiple schwannomas of the trigeminal nerves, acoustic nerves, lower cranial nerves, brachial plexuses and spinal canal: schwannomatosis or neurofibromatosis?

In most cases, while schwannoma is sporadically manifested as a single benign neoplasm, the presence of multiple schwannomas in one patient is usually indicative of neurofibromatosis 2. However, several recent reports have suggested that schwannomatosis itself may also be a distinct clinical entity. This study examines an extremely rare case of probable schwannomatosis associated with intracranial, intraspinal and peripheral involvements. A 63-year-old woman presented with a seven-year history of palpable lumps on both sides of the supraclavicular area and hearing impairment in both ears. On p

Diagnosis and management of facial pigmented macules.

The differential diagnosis of pigmented macules on the mottled chronic sun-damaged skin of the face is challenging and includes lentigo maligna (LM), pigmented actinic (solar) keratosis, solar lentigo, and lichen-planus-like keratosis. Although dermatoscopy improves the diagnostic accuracy of the unaided eye, the accurate diagnosis and management of pigmented facial macules remains one of the most challenging scenarios in daily practice. This is related to the fact that pigmented actinic (solar) keratosis, lichen-planus-like keratosis, and LM may reveal overlapping criteria, making their diffe

Therapeutic strategy for large vestibular schwannomas.

Management of large vestibular schwannoma (VS) remains a difficult problem for which the extent of tumor resection and the role of radiosurgery continue to be the subject of debate. To develop an optimal therapeutic strategy for large VS, various treatment options within the avenues of microsurgery and radiosurgery were evaluated and compared. The authors carefully reviewed the clinical data of 50 surgical cases of large VS, defined as tumors with an average diameter over 3 cm in magnetic resonance imaging (MRI). All patients underwent microsurgery as the primary treatment. Gross total removal

Skin flaps.

Open wounds, particularly around the face, often require complicated techniques for optimal closure. The approach to the closure of the complicated wound depends largely on the nature of the wound, including the location and size of the defect, the functional outcome after closure, the medical comorbidities of the patient, neighboring structures, and whether the defect is secondary to a malignancy or trauma. The goals of wound management are optimal aesthetic outcome, preservation of function, and patient satisfaction. The authors briefly review basic skin closure options and discuss use of sk

Facial neuroma in the internal auditory canal.

Authors presented two cases of facial neuromas in the internal auditory canal, one without facial palsy and the other with facial palsy. In both cases neuromas were occult and undiagnosed. Although in the first case neuroma was greater than the other, facial palsy was not developed. The mechanism of the facial palsy due to neuromas could not be clearly clarified.

[Reoperation of tumors of the parotid gland. Technical approach and consequences for the 7th cranial nerve. Apropos of 22 cases].

Second or more surgical procedures on parotid are usually difficult and may induce injury on the facial nerve. The authors report their experience about 42 patients. The choice between a total parotidectomy or a surgical excision of a tumor depends of the number of surgical procedure, the type of initial procedure and the histological type of the tumor. Second surgical procedures for a wrong initial histological diagnosis (11 cases) were always a total parotidectomy with facial nerve preservation. The risk of a partial or total facial palsy is higher after several recurrences (3 of 8 cases). T

[Isolated extracranial meningioma of the facial soft tissues].

BACKGROUND: Isolated primarily extracranial meningiomas are extremely rare tumors of the head and neck region. The pathogenesis of these benign lesions is not fully clarified. Most likely these tumors derive from ectopic meningeal or arachnoid cells following the nerve sheaths of cranial nerves along their course through the skull base. PATIENT: We report on a patient presenting with a slow growing tumor of the soft facial tissue. The tumor was resected completely via an external approach. RESULTS AND CONCLUSION: The patient has been free of tumor recurrence for one year. The pathogenesis of

[A clinicopathological retrospective epidemiological analysis of benign tumors and tumor-like lesions in the oral and maxillofacial region, diagnosed at the University of Szeged, Department of Oral Medicine (1960-2014)].

INTRODUCTION: In a clinicopathological retrospective epidemiological study we investigated benign tumors and tumor-like lesions located in the orofacial region, diagnosed at the Universiy of Szeged, Department of Oral Medicine. METHOD: During a 54-year period (1960-2014), 14 661 biopsies were taken. The included subjects were 7491 patients diagnosed with benign tumors and tumor-like lesions. RESULTS: The average age of patients was 55.3 years, 2823 (37.7%) patients were male and 4668 (62.3%) female. The male : female ratio was 1 : 1.65. Most of the patients included in the study were aged 51

Birt-Hogg-Dubé syndrome: clinical and genetic studies of 10 French families.

BACKGROUND: Birt-Hogg-Dubé syndrome (BHDS) is an autosomal dominant genodermatosis predisposing to the development of multiple fibrofolliculomas (FFs), pulmonary cysts, spontaneous pneumothorax and renal neoplasms. The association of BHDS with various nonrenal neoplasms has been reported but remains controversial. OBJECTIVES: To report the clinical features and germline mutations in 22 patients from 10 unrelated families with BHDS investigated during a 5-year prospective study by the Department of Dermatology at the University Hospital of Montpellier, France. Also, to define more clearly the

Malignant parotid tumors in patients previously treated for childhood cancer: clinical and imaging findings in eight cases.

OBJECTIVE: The purpose of this study was to describe the clinical and imaging characteristics of malignant neoplasms of the parotid gland in patients who had been successfully treated for childhood cancer. MATERIALS AND METHODS: Eight cases of parotid carcinoma were identified among survivors of primary childhood cancer treated at two centers. Medical records and imaging studies were reviewed to identify the frequency of second parotid tumors, to assess possible risk factors for their development, and to identify any specific MR or CT features of these tumors. RESULTS: Parotid tumors (mucoep

Facial reanimation by muscle-nerve neurotization after facial nerve sacrifice. Case report.

INTRODUCTION: Recovering a certain degree of mimicry after sacrifice of the facial nerve is a clinically recognized finding. The authors report a case of hemifacial reanimation suggesting a phenomenon of neurotization from muscle-to-nerve. CLINICAL CASE: A woman benefited from a parotidectomy with sacrifice of the left facial nerve indicated for recurrent tumor in the gland. The distal branches of the facial nerve, isolated at the time of resection, were buried in the masseter muscle underneath. The patient recovered a voluntary hémifacial motricity. The electromyographic analysis of the moto

Pathological features of the facial nerve in patients with facial palsy of varying aetiology. Light and electron microscopic study.

Facial nerve specimens obtained during reinnervation surgery from 31 patients with facial palsy of varying aetiology were studied. In patients who had a facial palsy after attenuation during acoustic schwannoma removal the facial nerve had normal populations of myelinated nerve fibres in the sensory zone but their numbers and sizes were severely reduced and fibrosis was increased in the motor zone. The latter also had large numbers of endoneurial tubes with thin unmyelinated axons. When the facial nerve had been severed in tumour removal, it had practically no remaining nerve fibres. In three

Pathological features of human facial nerve after central injury during tumor removal, with special reference to the non-motor component.

After injury to the intracranial portion of the facial nerve, undegenerated myelinated nerve fibres were seen to form a distinct bundle in the peripheral part of the transverse fascicular area at the level distal to the geniculate ganglion. The numbers and diameters of the fibres were similar to those seen in the nervous intermedius portion of the facial nerve in experimental animals. The average number of these apparently non-motor fibres was 2500. Their average diameter was 2.4 micrometer.
