BACKGROUND: Multiple myeloma (MM) carries a high venous thromboembolism (VTE) risk; aspirin shows limited efficacy, while factor-Xa inhibitors show promise.
OBJECTIVES: Evaluate apixaban for primary thromboprophylaxis in newly diagnosed MM (NDMM).
METHODS: Prospective single-center study of consecutive patients receiving apixaban, followed for VTE and bleeding.
RESULTS: Eighty-seven patients (median follow-up of 11 months) were included; no major bleeding, deep vein thrombosis, or pulmonary embolism occurred; two developed symptomatic superficial vein thrombosis.

Continuous unfractionated heparin is widely used in intensive care, yet its complex pharmacokinetic properties complicate the determination of appropriate doses. To address this challenge, we developed machine learning models to predict over- and under-dosing, based on anti-Xa results, using a monocentric retrospective dataset. Traditional anticoagulants such as heparin and vitamin K antagonists have been the mainstay of antithrombotic therapy for many years. However, these drugs have a number of well-recognized drawbacks: unfractionated heparin, low molecular weight heparins and fondaparinux are administered parenterally, and vitamin K antagonists require routine coagulation monitoring. Novel, single-target oral anticoagulants have been developed that do not need routine coagulation monitoring, including the direct factor Xa inhibitors rivaroxaban, apixaban and edoxaban, and the direct thrombin inhibitor dabigatran.

In this open label pilot safety study 80 children over 3 months old with deep venous thrombosis were treated with enoxaparin with a target 4 h anti-factor Xa activity between 0.5-0.8 IU/mL.
