Ovarian carcinoma in pregnancy remains a rare event. Although concern regarding the gestation complicates therapy, platinum drug-based combination chemotherapy is often deemed warranted in such cases. We report the antepartum use of cisplatin, followed by carboplatin, for an ovarian serous cystadenocarcinoma. Fetomaternal hemorrhage is a rare, potentially catastrophic event for a fetus. Leakage of the fetus's blood into the mother's circulation can cause fetal anemia, hydrops, and even death. The prevailing symptom is decreased fetal movement, and signs can include a sinusoidal electronic fetal monitor pattern, a positive Kleihauer-Betke test, or changes in fetal Doppler blood flow. A mother's report or perception of decreased fetal movement coupled with a nonreactive nonstress test or abnormal ultrasound findings should prompt an investigation into underlying causes.

Thirty healthy term gravidas in active labor received a paracervical block (PCB) with the ester-type local anesthetic, 2-chloroprocaine (2CP). Good to excellent pain relief resulted in all but 1 case. The duration of action was short (mean 38.9 min), requiring repeat blocks in 6/30 cases. Three cases of occult abruption after maternal trauma are presented. In each instance, there was no vaginal bleeding, and abdominal findings were subtle. By maintaining a high index of suspicion and monitoring the fetus continuously, the obstetrician may minimize the fetal complications of placental abruption in traumatized pregnant patients.

A preliminary study was made of the potential usefulness of the pre-ejection period (PEP) as an antepartum indicator of fetal well-being. PEP was evaluated in 108 subjects over a 2-year period. It is essential to correct for the effects of heart rate and gestational age on PEP before attempting to relate it ot fetal status. From the control group of 95 fetuses within our study, an equation to correct PEP to a standard heart rate of 140 beats per minute and gestational age of 40 weeks was derived: PEP140,40 = PEP + 0.36(FHR-140) + 1.4(40-GA). A 40-month retrospective review of 677 postterm pregnancies was undertaken to determine the usefulness of 24-hour urinary estrogen per gram of creatinine (E/Cr) in monitoring such pregnancies. The cutoff value between normal and low E/Cr values was chosen at the tenth percentile (18 mg/gm). The corrected perinatal mortality rate (PMR) in post-term pregnancies with normal E/Cr was 0.23% (1/426) and was not different from that at term (0.23%; 4/1,775).
