Distal arthrogryposis, specific facial dysmorphism and psychomotor retardation: a recognizable entity in surviving patients with the fetal akinesia deformation sequence.

Two non-related patients, a boy and a girl, are described suffering from distal arthrogryposis and facial dysmorphism consisting of flat face, hypertelorism and telecanthus, small mouth with thin, downturned upper lip, micrognathia, cleft palate and simple, low-set, posteriorly angulated ears. Feeble fetal movements (case 2), polyhydramnion (case 1) and lung hypoplasia (case 2) were present. On follow-up, both children were severely developmentally retarded. These findings are consistent with the Fetal A/hypokinesia Deformation Sequence (FADS). Survival beyond the neonatal period in this heter…

[Prenatal sonographic diagnosis and clinical consequences in small intestine obstruction].

Fetal intestinal obstructions can be diagnosed and differentiated from other intra-abdominal lesions with prenatal sonography. The characteristic signs are persistent echo-free areas in the fetal abdomen which frequently show peristaltic movements, and the accompanying polyhydramnios. A vaginal delivery near term is desirable and tocolytic agents and therapeutic amniocentesis should be used in the event of premature labour and maternal distress due to polyhydramnios. Premature labour also necessitates the induction of pulmonary maturation with corticoids. At least weekly ultrasonic controls ar

Heterotopic brain tissue in the submandibular region and lung. Report of two cases and comments about pathogenesis.

Two cases of extracranial heterotopia of the brain are reported. Case 1, in an otherwise normal female baby, had a brain heterotopia in the submandibular region. Case 2, in a still-born female baby with anencephaly, had the heterotopia in the lung. The lesion in case 1 is believed to be an encephalocele that has been pinched off through a cleft in the bone or through a foramen of the skull; the pathogenesis is similar to that of the so-called "nasal glioma". Concerning the pathogenesis of case 2, contrary to the generally supported embolism theory, observation of serial sections of the tumor i

[Fetal anesthesia in cesarean section for patients with congenital diaphragmatic hernia].

We experienced two cases of anesthesia for cesarean section in patients with prenatally diagnosed congenital diaphragmatic hernia (CDH). Fentanyl/midazolam anesthesia was selected for fetal sedation at birth. The blood samples were obtained from the maternal vein, umbilical vein and umbilical artery for measuring blood drug concentrations during anesthesia. Midazolam concentrations in umbilical vein were 137 and 256 ng.ml-1. Spontaneous breathing and movement of the newborns were restrained in both cases and these sedations facilitated preoperative general management for newborns with CDH.

Ensemble Transfer Learning for Fetal Head Analysis: From Segmentation to Gestational Age and Weight Prediction.

Ultrasound is one of the most commonly used imaging methodologies in obstetrics to monitor the growth of a fetus during the gestation period. Specifically, ultrasound images are routinely utilized to gather fetal information, including body measurements, anatomy structure, fetal movements, and pregnancy complications. Recent developments in artificial intelligence and computer vision provide new methods for the automated analysis of medical images in many domains, including ultrasound images. We present a full end-to-end framework for segmenting, measuring, and estimating fetal gestational age…

The effect of music on the cardiac activity of a fetus in a cardiotocographic examination.

BACKGROUND: Music therapy as an adjunct to treatment is rarely used in perinatology and obstetrics, despite the proven therapeutic effect. A … observed in the number of uterine contractions, accelerations, episodes of higher variability, and fetal movements after listening to the music. CONCLUSIONS: Listening to classical music can serve as a successful method of prophylaxis against premature deliveries, indicated by the lower number of uterine contractions, and in stimulating fetal movement in the c …

Abnormal development of the intercostal muscles and the rib cage in Myf5-/- embryos leads to pulmonary hypoplasia.

The aim of our study was to investigate the importance of pulmonary distension and fetal breathing-like movements executed by the contractile activity of the intercostal respiratory muscles for proper lung growth and maturation. Lung development in Myf5-/- embryos, lacking the rib cage and functional intercostal musculature, was compared with wild-type controls at embryonic days 14.5, 16.5, and 18.5. Our data revealed that Myf5-/- embryos suffered from pulmonary hypoplasia in part due to the decreased number of proliferating lung cells and in part due to the increased number of terminal deoxyn

Effects of maternal posture and cognitive-behavioral interventions on labor outcomes in primigravidas with abnormal fetal head position: a randomized controlled clinical trial.

OBJECTIVE: Fetal head malposition can result in neonatal and maternal complications. Fetal head malposition occurs frequently in labor. Appropriate and timely managements are required. Maternal posture and cognitive-behavioral interventions could improve labor outcomes in primigravidas with fetal head malposition. METHODS: A randomized controlled clinical trial was performed between January and December 2022 (Chinese Clinical Trial Registry, ChiCTR2100049359). Eligible primigravidas were randomly assigned into four groups. Group A received traditional perinatal care. Group B, C, and D receive

Benefits of Resistance Training During Pregnancy for Maternal and Fetal Health: A Brief Overview.

Research demonstrates resistance training is not only safe but also beneficial for pregnant women. However, exercise recommendations for pregnant women still minimize the importance of resistance exercise and provide minimal guidance. With a large increase in strength-focused sports among women, it is critical to re-evaluate the risk/benefit ratio of these exercises and ensure the latest recommendations reflect the latest clinical research. The purpose of this review is to highlight the safety and benefits of resistance training for both maternal and fetal health, particularly focusing on rece

Rapid-eye-movement sleep in jittery infants.

BACKGROUND: The pathogenesis of neonatal jitteriness (JT) remains unknown. Neonatal JT could be one of the symptoms associated with drug withdrawal syndrome due to maternal medication. To study the influence of chemicals and environment on brain development in the fetal period, Swaab and Mirmiran proposed "behavioral teratology". JT could be one of the targets for this concept. Swaab and Mirmiran postulated that rapid-eye-movement sleep (REM sleep) is useful for studying behavioral teratology. AIM: We aimed to determine the neurophysiological alterations in infants with JT by investigating RE

Heterogeneity in fetal akinesia deformation sequence (FADS): autopsy confirmation in three 20-21-week fetuses.

Fetal akinesia deformation sequence (FADS) is a rare condition characterized by intrauterine growth retardation (IUGR), congenital limb contractures, pulmonary hypoplasia, hydramnios and craniofacial abnormalities. The present report comprises an autopsy study of three fetuses to illustrate the variable clinical manifestations and neuropathological findings. Fetus 1 had arthrogryposis and no movement on fetal ultrasound examination. Aborted at 21 weeks, the fetus showed micrognathia, bilateral joint contracture with pterygia at the elbow and axilla. Growth retardation and pulmonary hypoplasia

Hypoglycaemia in pregnancy.

Normally there is a very close relationship between maternal and fetal glucose concentrations during both early and late gestation. Maternal hypoglycaemia during pregnancy will therefore not only affect the mother herself but also the conceptus. As can be judged from the literature, acute hypoglycaemic episodes are only rarely seen in non-diabetic pregnancies. In recent years it has become increasingly evident that insulin-dependent diabetic patients, whether pregnant or not, run a much increased risk of having severe hypoglycaemia (SH) attacks (i.e. the patient needs the assistance of another

Perfusion studies of glyburide transfer across the human placenta: implications for fetal safety.

OBJECTIVE: Gestational diabetes affects up to 5% of women. Oral hypoglycemics have been avoided because of the assumption that their placental transfer may cause fetal-neonatal hypoglycemia. A recent randomized trial could not show measurable glyburide levels in umbilical blood despite maternal treatment with regular doses of glyburide. The mechanism underlying this phenomenon is not known. The objective of our study was to document, using a human placenta perfusion model, whether glyburide is actively effluxed from the fetal to the maternal circulation. STUDY DESIGN: In vitro perfusion studi

Toxicity of lidocaine in adult, newborn, and fetal sheep.

The relative central nervous system and cardiovascular toxicity of lidocaine was compared in adult, newborn, and fetal sheep during continuous infusion of lidocaine into the jugular vein at the rate of 2 mg x kg-1 x min-1. An identical sequence of toxic manifestations occurred in the adult, newborn, and fetus: convulsions, hypotension, respiratory arrest, and circulatory collapse. Doses necessary to produce these manifestations were highest in fetuses and lowest in adults. For example, in order to elicit convulsions, 5.8 +/- 1.8 mg/kg of lidocaine was required in the adults, 18.4 +/- 2.2 in th

Ultrasound features of congenital listeriosis--a case report.

We present a case of congenital listeriosis in a twin pregnancy. Presentation was prompted by decreased fetal movements and an ultrasound examination which demonstrated features similar to those observed in an adult with inflammatory conditions of the bowel, namely, small amounts of ascites, dilated loops of bowel and thickening of the bowel wall. Such ultrasound signs may be useful in consideration of the diagnosis of congenital infection and prompt diagnostic tests and appropriate interventions that assist in early therapy.

Umbilical cord blood biomarkers of neuronal injury in sleep-disordered breathing during pregnancy.

Sleep-disordered breathing (SDB) during pregnancy may compromise fetal development and brain maturation. Ubiquitin C-terminal hydrolase-L1 (UCH-L1) and glial fibrillary acidic protein (GFAP) are biomarkers of neuronal injury and cellular stress and may provide insight into fetal vulnerability in this context. UCH-L1 and GFAP concentrations were measured in umbilical cord blood from 85 neonates born to women who underwent hospital-based polysomnography during the third trimester. The apnea-hypopnea index (AHI, and rapid eye movement (REM)-AHI)) and mean nocturnal oxygen saturation (mSaO2) were
