Revision Open Reduction and Internal Fixation of Ankle and Syndesmosis Malunions.

Malreduction of the fibula and syndesmosis remains a common complication associated with the surgical management of ankle fractures. This malreduction leads to profound alterations in the biomechanics of the ankle, leading to chronic pain and premature degenerative changes of the ankle. It is possible to correct many fibular and syndesmotic malunions with reconstructive surgical techniques, of which there are several for revising and salvaging the malreduced ankle mortise and tibiofibular syndesmosis.

Fracture Line Morphology and a Novel Classification of Pilon Fractures.

OBJECTIVE: Currently, there is no research that includes a comprehensive three-dimensional fracture mapping encompassing all types of Pilon fractures. Moreover, the existing classification systems for Pilon fractures exhibit only moderate to fair consistency and reproducibility. Additionally, some of these classification systems fail to accurately depict the morphological characteristics of the fractures. This study aimed to create a fracture map encompassing all types of Pilon fractures by three-dimensional fracture mapping. In addition, this study conducted a finite element analysis of the n

Surgical Treatment of a Type IIIA Open Tibia Fracture in a Patient with Fibrodysplasia Ossificans Progressiva: A Case Report.

CASE: A 64-year-old woman with fibrodysplasia ossificans progressiva (FOP) sustained a Gustilo-Anderson type III open tibia and fibula fracture. She underwent debridement, irrigation, open reduction and internal fixation, and primary closure, followed by postoperative corticosteroids. A nondisplaced peri-implant fracture slowed recovery 3.5 months postoperatively, but she improved steadily and subjectively regained 90% of baseline function at 1 year, without significant heterotopic bone formation. CONCLUSION: In FOP patients with open fractures, standard surgical management and careful postop

Irreducible Salter-Harris type II fracture of the distal tibial epiphysis.

An 11-year-old boy was admitted to the hospital after he fell to the ground from the roof, complaining of pain in his left ankle. Radiographs showed a Salter-Harris type II fracture-separation of the distal tibial epiphysis together with a transverse bending fracture of the distal fibula. An attempt at closed reduction was unsuccessful. At open reduction, torn periosteum and the posterior tibial tendon were interposed between the tibial metaphysis and epiphysis. After the tendon was deflected to its normal position, fracture-separation of the distal tibial epiphysis was easily reduced. At the

[Osteosynthesis with compression as an alternative method in the internal fixation of malleolar fractures].

The authors discussed a group of patients with malleolar fractures operated on between 1974 and 1977. Compressive osteosynthesis was used as an alternative method of treatment. They compared their group of patients with a much larger group of patients that were surgically treated for malleolar fractures in the traumatology clinic in Ljublana between 1974 and 1977. This second group was divided according to the type of fracture, the type of surgical treatment, the time of treatment, and early and late complications. The authors also discussed the specifics of early postoperative functional trea

Recognition, Treatment, and Outcome of Calcaneal Fracture-Dislocation.

BACKGROUND: Calcaneal fracture-dislocations are rare but potentially disabling injuries that are regularly overlooked at first presentation. To date, only about 50 cases have been reported in the literature. METHODS: Over a period of 8 years, 10 patients (average age 61.7 years) with acute fracture-dislocations of the calcaneus were treated at 2 level 1 trauma centers. The calcaneocuboid joint was involved in 9 patients. There was a concomitant fracture of the lateral talar process and of the tip of the distal fibula in 7 patients each. Open reduction and internal fixation was performed in 9

Unstable Ankle Fracture Fixation Using Locked Fibular Intramedullary Nail in High-Risk Patients.

Open reduction internal fixation is the gold standard for unstable ankle fracture fixation; however, complications in patients with multiple medical comorbidities are common. Intramedullary nail fixation of the fibula can help to mitigate these difficulties. A retrospective chart review was performed on all patients who underwent fixation for unstable ankle fracture between January 2015 and March 2016 at our level I trauma center. Comorbidities in the patient sample included were one or several of diabetes, renal disease, hypertension, advanced age with osteoporosis, hemorrhagic blisters, and

The treatment of unstable fractures of the ankle using the Acumed fibular nail: development of a technique.

Techniques for fixation of fractures of the lateral malleolus have remained essentially unchanged since the 1960s, but are associated with complication rates of up to 30%. The fibular nail is an alternative method of fixation requiring a minimal incision and tissue dissection, and has the potential to reduce the incidence of complications. We reviewed the results of 105 patients with unstable fractures of the ankle that were fixed between 2002 and 2010 using the Acumed fibular nail. The mean age of the patients was 64.8 years (22 to 95), and 80 (76%) had significant systemic medical comorbidit

Early Corrections after Failed Ankle Fracture Fixation.

Even minor residual fragment malpositioning after internal fixation of ankle fractures is associated with a worse prognosis. Frequent causes for non-anatomical reduction are fibular shortening due to comminuted fractures or poor bone quality, translational or rotational malpositioning of the distal fibula within the tibial incisura with unstable syndesmosis injuries, and inadequately addressed bony avulsions of the tibiofibular syndesmosis. After operative treatment of fracture dislocations with syndesmotic fixation, in case of complex fracture pathoanatomy, and with suspected non-anatomic red

Fibular insufficiency fracture: an under-reported complication of advanced tibialis posterior dysfunction.

We present a case of fibular insufficiency (stress) fracture in a patient with pre-existing asymptomatic advanced tibialis posterior dysfunction. The patient had severe planovalgus (flat foot) deformity but no pain before the injury; hence had never sought any treatment. She was first treated conservatively for the fracture which failed to control the deformity and had ongoing symptoms. She eventually required tibio-talo-calcaneal fusion. To the best of our knowledge, this is the first case report of this condition in the English literature. In patients with severe tibialis posterior dysfuncti

Tibial fractures involving the ankle in children. The so-called triplane epiphyseal fracture.

Triplane fractures of the distal end of the tibia in fifteen children (average age, thirteen years) represented 6 per cent of 237 consecutive epiphyseal fractures of the ankle. Thirteen children were treated by closed methods (including manipulation) and two had open reduction of the fractures. At an average of twenty-six months after injury, three of fourteen patients showed roentgenographic evidence of premature symmetrical epiphyseal closure with less than 0.5 centimeter of shortening and no angular deformity. Of twelve children examined clinically, three had a 5 to 10-degree external rotat

Stress fractures in athletes.

During the 14-year period of 1971-1985, 368 stress fractures in 324 athletes were treated. The series contained 268 fractures in males and 100 fractures in females; 32 fractures occurred in children (less than 16 years), 117 in adolescents (16-19 years), and 219 in adults. Forty-six fractures were incurred by athletes at an international level, 274 by athletes at a national or district level and 48 by recreational athletes. Of the total cases, 72% occurred to runners and a further 12% to athletes in other sports after running exercises. The distribution of the stress fractures by site was: tib

Flouroscopic Control of Tibial Torsion After Intramedullary Nailing: A Technical Trick.

After intramedullary nailing of tibia shaft fractures, torsional malalignment greater than 10 degrees occur in up to 41% of operated legs. The reason is the difficult clinical assessment of rotation intraoperatively, the large variation in absolute torsion of the tibia, and the absence of established reliable methods to fluoroscopically evaluate tibial rotation and compare with the contralateral side. We present here a fast and low-tech intraoperative method on how to achieve identical tibial torsion of the operated and noninjured side. The method can be used for tibia shaft and metaphyseal fr
