We investigated the maximal advancement of the homodigital neurovascular island flap with the digit in full extension and its correlation to the digital length. In 32 adult cadaveric digits, flaps measuring 1 × 1 cm were sequentially elevated to different dissection points. Dissection of the flap to the proximal interphalangeal joint crease, palmo-digital crease, division of adjacent digital artery and the superficial arch resulted in flap advancement of 8, 12, 15 and 18 mm, respectively. The degree of advancement correlated to the length of the finger and was approximately 19% of the finger l

Report on 35 cases of mallet finger treated conservatively: a circular plaster cast was modeled in hyperextension of the distal interphalangeal joint. The immobilisation of the whole finger form the tip to the proximal phalanx was maintained for six weeks. The reported results were good or excellent with the exception of two cases.

BACKGROUND: Divergent carpometacarpal joint dislocations of the fingers are very rare. Due to severe swelling and overlapping of bones on a radiograph of the wrist and hand, dislocations are missed. The purpose of this clinical case report is to highlight this unusual injury to avoid missing diagnosis.
CASE PRESENTATION: We report a case of a 24-year-old Moroccan man, an athlete, who presented divergent carpometacarpal joint fracture-dislocations of the ulnar four fingers after a fall during a national cycling competition. Radiographs showed divergent dislocation and associated fractures. He

Isolated dislocation of the carpometacarpal joint of the index finger is a rare injury. This paper reports a case of an isolated dorsal dislocation of the second carpometacarpal joint not reducible by closed means. At open reduction, the extensor carpi radialis brevis was found to be interposed in the joint, blocking reduction.

Volar fracture-dislocations of the proximal interphalangeal joint are rare injuries caused by an axial force on a flexed digit resulting in an avulsion fracture of the dorsal lip of the middle phalanx with volar dislocation of the joint. This volar subtype is analogous to the more common dorsal subtype with a mirror image fracture on the dorsal lip of the middle phalanx. The main significance in this type of injury lies in the disruption of the extensor mechanism at the central slip. The goals of treatment, apart from restoring a congruent and stable joint, is to restore the extensor mechanism

Mallet finger is a common injury involving either an extensor tendon rupture at its insertion or an avulsion fracture involving the insertion of the terminal extensor tendon. It is usually caused by a forceful blow to the tip of the finger causing sudden flexion or a hyperextension injury. Fracture at the dorsal aspect of the base of the distal phalanx is commonly associated with palmar subluxation of the distal phalanx. Most mallet finger injuries are recommended to be treated with immobilisation of the distal interphalangeal joint in extension by splints. There is no consensus on the type of

The joints of the hand are stabilized by articular contours, tough ligamentous structures, and dynamically by a system of flexor and extensor tendons. Disruption of a joint seriously reduces the ability of the hand to function properly. The history and especially a detailed description of the mechanism of injury is invaluable information for the diagnosis and for the ability to anticipate complications. Radiographic evaluation is required for virtually all potential injuries. Multiple views and especially a true lateral view of the digit, when appropriate, are usually sufficient. The clinical

The aponeurosis of the common extensor tendons is connected by intertendinous connections proximal to the metacarpophalangeal joints. These fibers are responsible for tenodesis effects in case of maximal finger extension and flexion and by this means they diminish the function of the other metacarpophalangeal joints. This mechanism can be described as the Quadriga-Syndrome of the extensor apparatus. These connecting structures also help to stabilize the transverse metacarpophalangeal arch and the position of the extensor tendons over the metacarpal heads. Cadaver dissections as well as operati

Proximal interphalangeal joint resection arthroplasty with Swanson's implant should be considered as a simplified new hinge joint with a simplified shorter but isometric extensor system which will work if the flexor system is intact. The procedure is indicated in cases of severe, ill-tolerated, fixed joint contractures. Functional improvement with restoration of some range of motion can be expected. Patients should be aware that they will not regain a complete range of motion at the PIP joint, there will be limited involvement of the distal interphalangeal joint, and there may be some discomfo

Stiffness of the interphalangeal joints of the fingers is a constant feature of Apert's syndrome. Because of this stiffness, the author has used split-thickness skin grafts when correcting Apert's syndactyly, thinking that contraction of such grafts post-operatively would not cause any joint contracture or finger deviation. This paper reports the results of eight patients whose average age at first surgery was 6 months. Separation of all digits was accomplished before the age of 2 years. A dorsal rectangular flap and interposing triangular digital flaps were utilised to create the web space an

Injury to the proximal interphalangeal (PIP) joint is one of the most common hand injuries encountered by orthopedic surgeons. The injuries are often misdiagnosed, resulting in delayed treatment and permanent impairment. The different types of PIP-joint fractures and dislocations have been well described, and a variety of therapeutic options have been proposed. Early detection and aggressive rehabilitation are the cornerstones of successful treatment. This is the first of a two-part series reviewing PIP injuries. We describe the anatomy and biomechanics of the PIP joint and review the classifi

Displaced fractures at the distal end of the middle phalanx require reduction if function is to be restored at the distal joint. Fractures at the base of the distal phalanx take on additional significance because of the insertions of the flexor and extensor tendons. The palmar articular fracture, when displaced, needs surgical replacement to restore flexor profundus function. In contrast, the displaced dorsal lip fracture, the mallet fracture, can be treated closed with the expectation of excellent results.

OBJECTIVE: To assess the evolution and progression of osteoarthritis (OA) in the finger joints using anatomical changes on standard radiographs.
METHODS: Data obtained from 85 patients enrolled in a prospective study were used to evaluate systems to score the morbidity and the progression of the disease over 3 years. Posteroanterior (PA) radiographs of the metacarpophalangeal (MCP) and interphalangeal (IP) joints were obtained at entry and after 3 years. Assessment of the progression of OA over time is based on: (1) the increase in incidence of OA in previously normal joints during the study

We report on three cases of a nonpainful flexion contracture of the proximal interphalangeal joint of the little finger. The word camptodactyly means bent finger, and the condition can occur either as an inherited or as a sporadic disorder. Simple camptodactyly should be borne in mind in the differential diagnosis of Dupuytren's contractures, other nontraumatic contractures of the fingers, and sclerodermia.

PURPOSE: To identify the risk factors associated with early implant fracture of silicone metacarpophalangeal (MCP) joint arthroplasty using the volar hinge silicone implant for patients with rheumatoid arthritis.
METHODS: We retrospectively reviewed 113 fingers of 31 hands that underwent MCP joint arthroplasty between 2008 and 2014, with a minimum follow-up of 3 years,. An implant fracture within 3 years after surgery was regarded as an early implant fracture. Patient records were reviewed for potential risk factors of age, affected fingers, ulnar drift angle, and range of motion of the MCP j

INTRODUCTION: Several isolated closed dislocations of the dorsal interphalangeal joint are reported, and sometimes they may be associated with other fractures/dislocations. Less is known about the open dorsal dislocation of the distal interphalangeal joint.
HYPOTHESIS: Open dorsal dislocations are stable after reduction and prone to develop dorsal interphalangeal joint stiffness.
METHODS: Twenty patients with open dorsal dislocation were treated by open reduction and volar plate repair. The diagnosis was made, and the displacement was noted. Associated injuries, fractures, and dislocations w

PURPOSE OF REVIEW: Arthrodesis of the thumb carpometacarpal, metacarpophalangeal, and interphalangeal joints are well-established options in the management of arthritis, instability, and/or deformity. This review includes a general review of the indications and technique of arthrodesis and recent review of the literature.
RECENT FINDINGS: New literature has focused on the role of arthrodesis in managing carpometacarpal arthritis and compared its results with ligament reconstruction and tendon interposition. However, no conclusive evidence demonstrates the superiority of arthrodesis. Several n

Flexible implant arthroplasty of the PIP joint in post-traumatic disorders can have very successful long-term results provided that the recommended surgical indications, operative techniques, and postoperative rehabilitation programs are carefully adhered to. Stability of the collateral ligament system and integrity of the extensor and flexor mechanism are essential prerequisites. A central slip splitting, central slip sparing, and palmar approach are described. The latter is preferred in post-traumatic conditions provided that the extensor mechanism is intact.
