A student in a veterinary program noted an inflammatory tinea corporis on the right forearm after working with cows. Culture grew Trichophyton verrucosum. A nine-day course of treatment with oral fluconazole, 100 mg daily, resulted in prompt resolution. Awareness of the clinical presentations of barn itch, and familiarity with wet mount and histopathologic features, and special growth requirements of T. verrucosum will facilitate prompt diagnosis and treatment. Therapy with newer antifungal agents such as fluconazole may offer advantages over grisefulvin.

The clinical management of Candida albicans (C. albicans) infections presents considerable problems, intensified by the few therapeutic choices available for this disease. The increasing resistance to fluconazole (FLC) following its widespread clinical use underscores the need for novel therapeutic strategies. Drug repurposing represents a promising approach to expand the antifungal armamentarium, and bortezomib (BTZ), a proteasome inhibitor originally approved for multiple myeloma, has emerged as a candidate for this purpose. This work systematically assessed the synergistic effects of flucon…

Vulvovaginal candidosis is an affliction caused by yeasts. Symptoms in the vulva are generally associated with the spreading of infected vaginal fluid. To better understand the role of the vulva in these dynamics, in this study we aim to fully identify and characterize non-Candida albicans vulvar yeast isolates. Fifty-four vulvar swabs were obtained from 31 women attending a gynecological consultation. After species identification, fluconazole susceptibility was assessed by the microdilution broth method. Biofilm biomass was quantified using crystal violet staining, and phospholipase and hemol…

Currently, the molecular mechanisms of azole resistance in C. glabrata are unresolved. This study aims to detect azole resistance of C. glabrata after exposure to fluconazole (Diflucan) in vitro. After 50 days of induction, the five susceptible isolates of C. glabrata demonstrated cross-resistance to azoles (fluconazole (Diflucan), voriconazole and itraconazole). Mutations in PDR1 or ERG11 genes are key nodes in azole resistance of C. glabrata. DNA-Sequencing revealed three(3/5) fluconazole (Diflucan)-resistant isolates had undergone missense mutations (R376Q, R772K, E1083K in PDR1 and F135L i…

The dosing of fluconazole for young infants remains empirical because of the limited pharmacokinetic (PK) data. We aimed to establish a population PK model and assess the systematic exposure-response of commonly used regimens of fluconazole in Chinese infants. We included infants with a postnatal age of less than 120 days and received intravenous fluconazole. Both scheduled and scavenged plasma samples were collected, and fluconzaole concentration was determined by a validated ultra-performance liquid chromatography-tandem mass spectrometry assay. Population PK analysis was conducted using Pho…

Candida auris has emerged as a multidrug-resistant pathogen of great clinical concern. Approximately 90% of clinical C. auris isolates are resistant to fluconazole, the most commonly prescribed antifungal agent, and yet it remains unknown what mechanisms underpin this fluconazole resistance. To identify novel mechanisms contributing to fluconazole resistance in C. auris, fluconazole-susceptible C. auris clinical isolate AR0387 was passaged in media supplemented with fluconazole to generate derivative strains which had acquired increased fluconazole resistance in vitro Comparative analyses of c…

Candida auris is a newly emerging fungal pathogen of humans and has attracted considerable attention from both the clinical and basic research communities. Clinical isolates of C. auris are often resistant to one or more antifungal agents. To explore how antifungal resistance develops, we performed experimental evolution assays using a fluconazole-susceptible isolate of C. auris (BJCA001). After a series of passages through medium containing increasing concentrations of fluconazole, fungal cells acquired resistance. By sequencing and comparing the genomes of the parental fluconazole-susceptibl…

The recently emerged pathogenic yeast Candida auris is a major concern for human health, because it is easily transmissible, difficult to eradicate from hospitals, and highly drug resistant. Most C. auris isolates are resistant to the widely used antifungal drug fluconazole due to mutations in the target enzyme Erg11 and high activity of efflux pumps, such as Cdr1. In the well-studied, distantly related yeast Candida albicans, overexpression of drug efflux pumps also is a major mechanism of acquired fluconazole resistance and caused by gain-of-function mutations in the zinc cluster transcripti…

Candida albicans is an important opportunistic pathogen, causing both superficial mucosal infections and life-threatening systemic diseases in the clinic. The emergence of drug resistance in Candida albicans has become a noteworthy phenomenon due to the extensive use of antifungal agents and the development of biofilms. This study showed that budesonide potentiates the antifungal effect of fluconazole against fluconazole-resistant Candida albicans strains both in vitro and in vivo. In addition, our results demonstrated, for the first time, that the combination of fluconazole and budesonide can…

Candida albicans is the most common pathogen that causes balanoposthitis. It often causes recurrence of symptoms probably due to its antifungal resistance. A significant number of balanitis Candida albicans isolates are resistant to azole and terbinafine antifungal agents in vitro. However, balanoposthitis caused by fluconazole- and terbinafine-resistant Candida albicans has rarely been reported. Here, we describe a case of a recurrent penile infection caused by fluconazole- and terbinafine-resistant Candida albicans, as well as the treatments administered to this patient. The isolate from the…

Candida bracarensis is an uncommon Candida species found during an epidemiological study of candidiasis performed in Braga, Portugal. Initially, it was identified as C. glabrata, but recently detailed analyses pointed out their differences. So, little information is still available about C. bracarensis virulence factors and antifungal susceptibilities. Therefore, the main goal of this work is to evaluate the ability of C. bracarensis to form biofilms, to produce hydrolytic enzymes (proteases, phospholipases and hemolysins), as well as its susceptibility to amphotericin B and fluconazole. It wa…
