Functional brain imaging in apraxia.

BACKGROUND: An extensive literature describes structural lesions in apraxia, but few studies have used functional neuroimaging. We used positron emission tomography (PET) to characterize relative cerebral glucose metabolism in a 65-year-old, right-handed woman with progressive decline in ability to manipulate objects, write, and articulate speech. OBJECTIVE: To characterize functional brain organization in apraxia. DESIGN AND METHODS: The patient underwent a neurological examination, neuropsychological testing, magnetic resonance imaging, and fludeoxyglucose F 18 PET. The patient's magnetic

Absolute quantitation in neurological PET: do we need it?

This article addresses the question posed in the title by examining the effects of parameters traditionally associated with improved absolute quantitation, on the analysis of 12 acquired immune deficiency syndrome dementia complex (ADC) patients compared to a normal control group. Results are discussed within the framework of the subprofile scaling model (SSM) for analyzing patterns of regional covariation. It is demonstrated that the ability to extract measures of group discrimination and disease progression are unaffected by (1) limited improvements in image resolution, (2) the use of transm

Effect of barbiturate coma on glucose utilization in normal brain versus gliomas. Positron emission tomography studies.

Glucose utilization by normal and neoplastic cerebral tissue can be measured in humans using positron emission tomography (PET) with fluorine-18-labeled 2-deoxy-D-glucose (FDG). Malignant gliomas are known to exhibit hypermetabolic glucose consumption compared to normal brain. Barbiturate-sensitive cerebral glucose utilization is coupled to neuronal activity, and lesions lacking neuronal activity should be relatively insensitive to barbiturate suppression of glucose utilization. In a study to examine this phenomenon, three patients with cerebral gliomas underwent FDG-PET while awake and during

A Case of Secondary Chondrosarcoma Arising in Osteochondroma Diagnosed With 18 F-FDG PET/CT.

A man in his seventies with an incidentally detected scapular osteochondroma showed progressively increasing FDG uptake on serial 18 F-FDG PET/CT, while the CT component initially revealed no definite signs of malignant transformation. On the final PET/CT, however, CT images showed a periosteal reaction suggestive of extracortical extension, raising strong concern for malignancy. Biopsy was followed by tumor resection, and postoperative pathology confirmed secondary peripheral chondrosarcoma (grade 3). The patient later developed widespread metastases. This case underscores that increased or r

Sharing a whole-/total-body [18F]FDG-PET/CT dataset with CT-derived segmentations: an ENHANCE.PET initiative.

We present a large whole-body and total-body curated dataset of dual-modality 2-deoxy-2-[18F]fluoro-D-glucose (FDG)-Positron Emission Tomography/Computed Tomography (PET/CT) studies, consisting of 1,683 PET/CT images and the corresponding CT-derived segmentations of 130 target regions. This multi-center dataset includes images from individuals without overt disease and patients with a range of malignant and inflammatory pathologies, including arthritis, lymphoma, and melanoma, as well as cancers of the lung, head-neck, and genito-urinary tract. Target regions were first automatically segmented

Incidental Detection of Remote Ictal Hypermetabolism on FDG PET/CT Following Brain Metastatectomy: A Rare Diagnostic Pitfall.

FDG PET/CT is widely used in oncology but may occasionally show non-neoplastic hypermetabolic processes. We report incidental detection of probable ictal/peri-ictal hypermetabolism on follow-up FDG PET/CT in a 39-year-old woman with treated breast carcinoma and prior left temporal metastatectomy. Imaging demonstrated a postoperative cavity in the left temporal lobe and a focal intensely FDG-avid cortical area in the left posterior parietal region without CT correlate or other evidence of active systemic disease. The imaging pattern favored seizure-related cortical hypermetabolism rather than t

FDG-PET in Traumatic Brain Injury and Postconcussive Sequelae.

Traumatic brain injury (TBI) produces complex and evolving metabolic disturbances that are not fully captured by structural imaging. 2-deoxy2-(18F)fluoro-d-glucose ([18F]FDG) PET provides critical insight into these changes, demonstrating an initial hypermetabolic phase followed by chronic hypometabolism associated with injury severity and clinical outcomes. Repetitive TBI results in widespread metabolic dysfunction across key brain regions. While tau and amyloid PET imaging offer mechanistic insights, their clinical utility remains limited due to inconsistent findings. [18F]FDG PET currently

Rapidly growing mediastinal mature cystic teratoma with increased FDG uptake: challenging the paradigm of indolent growth.

BACKGROUND: Mature cystic teratomas of the mediastinum are typically germ cell tumors with indolent growth, with documented annual growth rates often less than 5 centimeters. We report a case with an abnormal growth rate (Average diameter growth rate 7.14 mm/month) and significant increased 18 F fluorodeoxyglucose (18 F FDG) uptake, which challenges conventional diagnostic concepts. CASE PRESENTATION: A 34-year-old female presented with progressive chest tightness. Serial CT imaging revealed a right anterior superior mediastinal mass that grew from approximately 1.5 cm in long diameter to 6.5

[18F]FDG PET/CT in fever and inflammation of unknown origin, bacteremia, and sepsis: an umbrella review of diagnostic accuracy and clinical impact.

INTRODUCTION: Fever or inflammation of unknown origin (FUO/IUO), bacteremia, and sepsis represent challenging diagnostic scenarios in clinical practice. Over the past decade, multiple systematic reviews and meta-analyses have reported quantitative data regarding the diagnostic accuracy, clinical impact, and cost-effectiveness of fluorine-18 fluorodeoxyglucose positron emission tomography combined with computed tomography ([18F]FDG PET/CT) in these complex infectious and inflammatory conditions. We performed an umbrella review of published systematic reviews and meta-analyses to provide a compr

FDG PET/CT Scan Still Can Help in Evaluation of Prostate Cancer.

Although 18F-FDG PET/CT scan is not the preferred modality to evaluate prostate cancer patients due to generally low uptake of FDG in prostate cancer cells, it can still be helpful in some selected cases with diagnostic dilemma. Here we are presenting an unusual prostate cancer case, status post radical prostatectomy, with heterogenous uptake in osseous lesions on 68Ga-PSMA PET/CT scan and low PSA values. He was referred for further evaluation with 18F-FDG PET/CT scan, revealing widely metastatic lesions in bone and liver, in addition to mildly avid recurrence in prostatectomy surgical bed and

Increased deep muscle activity with interference low-frequency electrical muscle stimulation: evaluation by positron emission tomography.

BACKGROUND: The effects of electrical muscle stimulation (EMS) devices that efficiently promote deep muscle contraction have not been characterized. The purpose of this study was to clarify using [18F]-fluorodeoxyglucose positron emission tomography (FDG-PET) whether interference with low-frequency EMS can promote muscle metabolism in deep. METHOD: A total of 16 healthy males were randomly assigned to the EMS group (n = 8, age = 29.8 ± 4.1 years) or the control group (n = 8, age = 26.1 ± 5.0 years). Individuals in the EMS group received interference low-frequency EMS for 20 min each day over

Comparative diagnostic performance of [68 Ga]Ga-FAPI PET/CT and [18 F]FDGPET/CT in biliary tract cancers: a systematic review and meta-analysis.

BACKGROUND: Biliary tract cancers (BTCs) are aggressive malignancies with poor prognosis due to late-stage diagnosis. While [18 F]-FDG PET/CT is widely used for detection and staging, its sensitivity for certain BTC subtypes is limited, prompting the evaluation of 68 Ga-FAPI PET/CT as a promising alternative. OBJECTIVE: To compare the diagnostic performance of [68 Ga]Ga-FAPI PET/CT and [18 F]FDG PET/CT in detecting BTCs, including primary tumors, lymph node involvement, and distant metastases. METHODS: We searched PubMed, EMBASE, Web of Science, Scopus, and the Cochrane Library until March 2

[18F]FDG PET/CT in a Rare Case of Pancreatic Ewing Sarcoma.

Extraosseous Ewing sarcoma (EES) is a rare malignant tumor originating from soft tissue, comprising approximately 20% of all Ewing sarcoma cases. EES commonly involves the trunk or extremities, with pancreatic involvement being extremely uncommon. Because of its rarity, EES of the pancreas may be misdiagnosed initially and requires a multimodality approach. We present the case of a 25-y-old woman who was initially treated for acute pancreatitis. However, her symptoms persisted, and a [18F]FDG PET/CT was conducted for evaluation, the results of which suggested malignancy, which was later confir

Fifteen years of use of functional imaging in multiple myeloma: where we started and where we are going.

Bone disease represents a hallmark feature of multiple myeloma (MM), affecting nearly all patients during the disease course. Morphological imaging techniques play a crucial role in detecting bone disease, whereas functional ones are also fundamental for the differentiation of active from inactive disease and prognostic stratification. The International Myeloma Working Group (IMWG) currently recommends whole-body low-dose computed tomography (WBLDCT) as the first-choice imaging technique for the diagnosis of bone disease, whereas magnetic resonance imaging (MRI) is recommended in cases without

FDG PET/CT in Stercoral Colitis.

Stercoral colitis is a rare inflammatory colitis caused by the accumulation of impacted fecal material. Left untreated, stercoral colitis can progress to fatal complications including ischemic necrosis, ulceration and perforation of the bowel, peritonitis, septic shock, and even death. We describe FDG PET/CT findings of stercoral colitis in the rectum incidentally found on FDG PET/CT in staging a patient with suspected renal cell carcinoma. FDG PET/CT showed fecal impaction in the dilated rectum, asymmetrical rectal wall thickening, perirectal fat stranding, and diffusely inhomogeneous activit

18 F-FDG PET/CT Versus 18 F-FAPI PET/CT in Eosinophilic Liver Infiltration.

A 48-year-old man presented with upper abdominal discomfort for 10 days. Abdominal contrast-enhanced CT showed multiple abnormal enhanced masses in liver, suggesting a probability of malignancies. The patient was then enrolled in a clinical trial of comparison of 18 F-FDG and 18 F-FAPI PET/CT in hepatic lesions. Compared with 18 F-FDG PET/CT, 18 F-FAPI PET/CT showed higher tracer uptake, more lesions in liver. Meanwhile, peripheral blood eosinophils increased remarkably. The hepatic lesions were highly suspected as eosinophilic liver infiltration. Finally, pathologic analysis confirmed the dia

Myositis Associated With Eosinophilic Granulomatosis With Polyangiitis Demonstrated by FDG PET/CT.

A 54-year-old woman with a history of adult-onset asthma presented with prolonged fever, rash, joint pain, and hypesthesia. Laboratory findings showed elevated eosinophils (22%). FDG PET/CT demonstrated uptake in consolidations in both lungs, consistent with eosinophilic pneumonia, and revealed systemic myositis with increased uptake (SUVmax 5.5) in muscles, predominantly in the lower extremities. Based on the 2022 ACR/EULAR criteria, she was diagnosed with eosinophilic granulomatosis with polyangiitis (EGPA). This case highlights the value of FDG PET/CT in demonstrating eosinophilic pneumonia

[18F]FDG PET-CT in Weber-Christian Disease.

A 46-year-old woman with relapsing fever and painful pelvic subcutaneous nodules was referred to our center for a second opinion. Despite antibiotic and corticoid treatment, inflammation markers remained high. FDG-PET/CT was performed to evaluate the possible cause of her clinical condition, exclude systemic disease and evaluate the extension and inflammation of the nodules. FDG-PET/CT showed multiple abdominopelvic subcutaneous soft tissue foci without visceral involvement. The review of a previous nodule biopsy revealed Weber-Christian Disease (WCD), a rare disorder characterized by idiopath
