Despite many advances in antifungal drug development and therapy, onychomycosis due to nondermatophyte molds (NDM) remains difficult to treat. Using a reference microdilution method (CLSI M38-A), the antifungal susceptibility to bifonazole, ciclopiroxolamine, fenticonazole, itraconazole, ketoconazole, miconazole, terbinafine and tioconazole of 64 molds isolated from toenail onychomycosis was studied. All the strains showed good susceptibility to ciclopiroxolamine. Aspergillus had excellent susceptibility to itraconazole followed by miconazole, ketoconazole, tioconazole, fenticonazole and terbi

At a routine examination, multiple colonies of hyphomycetes of uncommon morphology were isolated from two patients with clinically suspected mycosis of the big toes (and confirmed microscopically by the presence of fragmented/thin hyphae). Based on the detailed study of the morphological and physiological characteristics of the cultures, the isolates were identified as Arthrographis kalrae. To our knowledge, this is the first isolation of the thermally dimorphic keratinolytic fungus to be reported in Slovakia.

Nondermatophytic toenail infection with Scopulariopsis brevicaulis is rare, but may occur often in association with dermatophytes. We report a case of an 84-year-old man who presented with onychomycosis of the big toenail. Histopathologic examination of the avulsed nail showed evidence of S. brevicaulis coinfection with a dermatophyte, despite negative mycology results for the latter. Our case underscores the importance of histopathologic examination of nail specimens as an additional invaluable tool in the diagnosis of onychomycosis, as it may unmask false-negative mycology findings.

Of all superficial fungal infections, onychomycosis is the most difficult to manage. Practitioners of all disciplines realize its chronic nature, difficulty in eradication, and propensity to recur. Topical treatment of onychomycosis, as opposed to oral therapies, offers a distinct advantage by allowing the patient to apply medication directly to the affected area, thus decreasing the potential for serious adverse events, such as drug toxicity and drug interactions. In the past, a multitude of topical antifungal agents were used in the treatment of onychomycosis; however, an acceptable level of

A case of mycetoma caused by the black fungus Madurella grisea in northern Brazil is reported. The lesion was located on the patient's right foot without bone involvement. Clinical samples were collected by opening the fistules with a scalp and the grains removed for microbiological and histopathological analyses. Although mycetoma caused by M. grisea has been previously reported in Brazil, this is the first time that of this fungus has been identify in this particular region of the country.

UNLABELLED: Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a rare but increasingly recognized autoimmune disorder that predominantly affects adolescents and young adults. It often presents with prominent psychiatric symptoms at onset, leading to frequent misdiagnosis as a primary psychiatric disorder. We report the case of a 16-year-old girl who presented with acute psychiatric symptoms, including mood lability, hallucinations, and aggression, initially diagnosed as first-episode psychosis. Her condition rapidly deteriorated under antipsychotic treatment, progressing to neurol

Some patients with relapsing foot eczema and a shoe leather allergy, who fail to show positive results with standard series and shoe wear screening tray patch testing, do not respond to the use of hypoallergenic shoe leather. We assume that relevant allergens are present in hypoallergenic shoe leather. We described an experiment to demonstrate the presence of these allergens. Alcoholic extracts were made of green, black and undyed hypoallergenic shoe leather, and the extracts were fractioned by paper chromatography. The resulting chromatograms were cut into 16 equal paper disks and patch tests

Knuckle pads, first described by Garrod in 1893, 1 are benign, asymptomatic, well- circumscribed, smooth, firm, skin colored papules, nodules, or plaques. They most commonly occur on the dorsal aspect of the proximal interphalangeal joint of the finger, 2 but also may occur on the dorsal aspects of the foot over joints. 3-5 Knuckle pads may be inherited or acquired. 1 While some authors suggest that trauma is not a significant factor, 6 acquired knuckle pads have been associated with repetitive friction and trauma, 2,7 and may resemble athlete's nodules (also referred to as collagenomas). 1 Hi

BACKGROUND: Modern antifungal drugs achieve high mycological and clinical cure rates in onychomycosis of the toes, but little is known about the long-term evolution of the treated patients.
OBJECTIVES: The aim of this review was to analyse the therapeutic results recorded more than 1 year after initiation of therapy.
METHODS: We used two endpoints for the analysis: EP1 (the number of patients with negative mycology after follow-up, divided by the number of patients included at day 0, including all patients lost to follow-up), and EP2 (the number of patients with negative mycology after follo

Tinea pedis is the most common fungal infection worldwide. There are three well-accepted clinical presentations based on the offending organism. Predisposing factors include heat, humidity, and occlusion. Mild disease can evolve to "dermatophytosis complex" through bacterial superinfection. Potassium hydroxide preparation and culture are important diagnostic tests in each case.

A variety of individual nail dystrophies that are not categorized easily in other articles are reviewed. Onychoatrophia, anonychia, onychorrhexis, leukonychia, Beau's lines, onycholysis, onychomadesis, onychoschizia, haplonychia, longitudinal melanonychia, and ventral pterygium are included and pictured clinically. Their clinical description, etiology, associated conditions, differential diagnoses, and treatments are discussed and tabulated.

Although more than 50% of warts resolve spontaneously within 2 years, many, if untreated, have the propensity to spread and cause considerable discomfort. There are several treatments to date; however, most cause some type of tissue damage. Because spontaneous resolution of warts can occur, the role of cimetidine in the treatment of warts remains controversial. Anecdotal reports suggest cimetidine may have a role in the treatment of plantar warts with no side effects or tissue damage. A double blind trial in a large population should be conducted to further verify the effects of treating viral

Neutrophilic eccrine hidradenitis (NEH) has been described in a variety of clinical settings but is most often seen in leukemic patients receiving chemotherapy. We have recently encountered 6 healthy individuals, of whom 5 were children, who developed NEH localized to the feet. The patients were from 9 to 21 years of age. The presenting complaint was rapid development of tenderness of the feet with varying degrees of morbidity. At clinical evaluation, tender, erythematous papules and nodules were confined to the feet, primarily the plantar surfaces. The major differential diagnoses were erythe
