The purpose of this study was to evaluate carpal joint contact patterns after perilunate injuries and potential associations between functional and radiographic outcomes. Twenty-two patients with a computed tomography (CT) scan at least 2 years postoperatively were reviewed (mean follow-up 15 years). Assessment of carpal degenerative changes was done using Kellgren-Lawrence grading and CT-derived joint space area, which was calculated as the total area with interbone distance less than 2 mm. Increased joint space area signified a greater area with joint space narrowing and cartilage loss. Fift

Traumatic posterior dislocation of the hip associated with a fracture of the posterior acetabular wall and of the neck of the femur is a rare injury. A 29-year-old man presented at a level 1 trauma centre with a locked posterior dislocation of the right hip, with fractures of the femoral neck and the posterior wall of the acetabulum after a bicycle accident. An attempted closed reduction had failed. This case report describes in detail the surgical management and the clinical and radiological outcome. Open reduction and fixation with preservation of the intact retinaculum was undertaken within

We report a rare case of simultaneous dorsal dislocation of 4 ulnar carpometacarpal joints and dorsoradial dislocation of the trapezium with an associated fracture of the scaphoid tuberosity. The injuries were diagnosed early and treated successfully with closed reduction and transfixation using Kirschner wires. The functional results were excellent at 17-month follow-up.

An extremely rare fracture-dislocation of the elbow is presented in which the coronoid and olecranon processes of the ulna were fractured and the head of the radius dislocated posterolaterally. Roentgenograms are the key to diagnosis. Rigid internal fixation and early exercises resulted in a good functional result.

Between 1956 and 1984, 196 patients with perilunate fracture dislocations were treated at the Unfallkrankenhaus Wien-Meidling. The authors were able to examine 56 of these patients on average 12.5 years after the accident looking specifically at carpal instability. The results showed three patients with dynamic and 21 patients with static instability of the wrist. After having examined the X-rays of all the patients we formulated the following plan: If the dislocation is adequately reduced by closed manipulation, conservative management is successful. But if there remains a pathological radiol…

Bosworth fracture-dislocation is a rare and complicated type of ankle fracture accounting for 1.5 % of all ankle fractures. Diagnosis and treatment are often complex and attention is required to ensure favorable outcomes. We report here a case of a Bosworth fracture-dislocation with incarceration of the distal fibular fracture in a Volkmann's fragment of the in an 18-year-old male who fell from his motorcycle at low velocity causing a supination and external rotation of the foot. The fracture dislocation was initially reduced with a novel minimally invasive technique immediately followed by ap…

CASE: This report presents 2 extremely rare cases of anterior hip dislocations with associated greater trochanter and acetabular fractures. Both were treated with prompt reduction of the dislocation and open reduction and internal fixation (ORIF) of the greater trochanter fracture. The second patient also underwent ORIF of the acetabulum fracture. Harris Hip Score was 72 at 2.5 years and 95 at 15 years, respectively.
CONCLUSION: We recommend prompt reduction of the hip dislocation, open reduction and internal fixation of the greater trochanter and acetabular fractures, and addressing any asso

INTRODUCTION: Atypical hangman's fractures are coronally-oriented vertical fractures of the posterior body of C2. Though these are not uncommon injuries, there is a paucity of data investigating the management of these fractures, especially when they occur in association with subaxial fracture dislocations.
CASE PRESENTATION: A 50-year-old male suffered a cervical extension injury when he dove into a shallow swimming pool while intoxicated. Initial examination demonstrated 2/5 strength in the right deltoid and biceps and 3/5 strength in the left deltoid and biceps with no motor or sensory fun…

Traumatic hip dislocations are common in high-energy motor vehicle accidents. We present a case of a 43-year old man who sustained posterior hip dislocation with posterior wall acetabular fracture and ipsilateral intertrochantric fracture following a motorcycle accident. Urgent open reduction and internal fixation of the hip fracture-dislocation and fixation of intertrochantric fracture with a dynamic hip screw were done. To our knowledge, such an injury has been rarely reported in the literature. Possible mechanisms of injury and operative procedures are discussed. Radiographic follow-up afte

The authors present a case of fracture dislocation of the ankle joint with the proximal part of the fibula entrapped behind the tibia ; Bosworth's fracture. This fracture was described for the first time in 1848 by Hugier, and classified by Bosworth in 1947. In our case, diagnosis was not done in a first time, and closed reduction failed. This fracture was operated on and open reduction was performed. At the present time, the functional result is good.

Scaphotrapeziotrapezoidal fracture-dislocations are extremely rare injuries, requiring early recognition and treatment. We present a case of a 54-year-old man with scaphotrapeziotrapezoidal fracture/dislocation following a motor vehicle accident, resulting in dorsal dislocation of the first and second carpal-metacarpal joints through the trapezium and trapezoid. The identification of the dislocation was delayed, and a month after the initial injury, fixation was attempted elsewhere but had redislocated at the first postoperative visit. Subsequent fixation was successful, using both radial and

Posterior Monteggia fracture-dislocations are uncommon injuries that can result from a fall onto an outstretched hand. Often, these injuries are associated with coronoid and/or radial head fractures. When this is the case, direct fixation can require 3 separate fascial incisions: posterior to address the proximal ulnar fracture, lateral to address the radial head fracture, and medial to address the coronoid fracture. We illustrate a transolecranon surgical approach for fixation of a type IIA posterior Monteggia fracture-dislocation with associated radial head and coronoid fractures. In this ap

BACKGROUND/AIM: Trans-scaphoid perilunate fracture-dislocation (TSPFD) is a rare injury. TSPFD is a fracture-dislocation that severely disrupts the anatomical structure of the carpal bones and may occur as a result of a high energy trauma of the wrist or a fall on an open hand. In this study, the aim is to provide midterm clinical and radiological evaluations of cases diagnosed and treated as TSPFD.
MATERIALS AND METHODS: Eleven patients diagnosed with TSPFD as a result of wrist trauma were treated surgically and were analysed retrospectively. Clinical and radiological follow-

The present study investigated the pathogenesis and treatment strategies for pilon fractures with ankle dislocation. A total of 58 patients (47 males and 11 females) who had sustained pilon fractures with ankle dislocation were treated. The mean patient age was 48.1 years. Using the AO classification, 8 cases were type B2, 17 were type B3, and 33 were type C3. The dislocation was medial in 13 cases, lateral in 9, anterior in 11, posterior in 14, and longitudinal in 9. Radiologic examinations were conducted to evaluate the postoperative reduction, dislocation correction, fracture healing, and i

BACKGROUND: Heterotopic ossification (HO) is a serious complication of traumatic elbow injuries, particularly fracture-dislocations. Limited data exist in the literature regarding the risk factors associated with HO formation in these injuries. The purpose of this study was to review the incidence of HO after fracture-dislocation of the elbow and to identify potential risk factors associated with its formation.
METHODS: Twenty-seven patients (28 elbows) were surgically treated for elbow fracture-dislocations during 8 years, with an average follow-up of 14 months. Records were reviewed with at
