Effect of intramedullary nail length on the biomechanical performance of internal fixation for subtrochanteric femoral fractures.

OBJECTIVE: To compare the biomechanical performance of different lengths of intramedullary nails combined with reconstruction plates for fixation of Seinsheimer type IV subtrochanteric femoral fractures using finite element analysis, with additional evaluation of osteoporotic bone conditions. METHODS: A three-dimensional finite element model of Seinsheimer type IV subtrochanteric femoral fracture was constructed from CT data of a 51-year-old male volunteer. Three PFNA intramedullary nail lengths combined with a reconstruction plate were modelled: long-nail (320 mm), medium-nail (240 mm), and

Finite element mechanical analysis of rigid internal fixation selection for distal femur fractures.

Background and Objective.Compared with lateral locking plate (LLP) fixation, dual-plate (DP) fixation has not demonstrated superior clinical outcomes in the treatment of distal femur fractures. We propose that this discrepancy arises from unclear indications. This study examining the influence of varying metaphyseal defect sizes on the mechanical behavior of the two fixation constructs, provides insight into defining their appropriate clinical indications.Methods.Finite element analysis was conducted to compare the biomechanical performance of LLP-only versus DP fixation in AO/ASIF type C2 fra

Open Reduction and Stabilization Without Internal Fixation for the Treatment of Displaced Radial Neck Fractures.

Background Radial neck fractures are a rare injury in both adults and children. In adults, dislocated fractures of the radial neck are commonly treated using open reduction and internal fixation with headless compression screws or plate osteosynthesis. In children, isolated fractures are most commonly treated using percutaneous fixation. However, these methods are associated with the risk of posterior interosseous nerve palsy and the need for wire removal. Thus, firm fixation without the use of internal fixation is preferable. In this study, we aimed to evaluate open reduction and stabilizatio

Minimally Invasive Internal Fixation of Femoral Shaft Fractures-A Biomechanical Study with a Disruptive Technique.

(1) Background: In polytrauma patients, femur fractures are usually stabilised by external fixation for damage control, later being treated with definitive plate or nail osteosynthesis. Screw/rod systems established in spinal surgery might be inserted for internal fixation, providing sufficient fracture stability that subsequent intervention is unnecessary. This was to be investigated biomechanically. (2) Methods: The unilaterally applied spinal internal fixator (IF) was subjected to load and deformation analysis on artificial femurs with 32-A3 fracture according to AO classification. Distance
