A Rare Neglected Ipsilateral Humeral Triad: Comminuted Proximal Humerus Nonunion with Avascular Necrosis, Mid-Shaft Fracture, and High Radial Nerve Palsy Managed with Staged Reconstruction.

INTRODUCTION: Ipsilateral humeral shaft and proximal humerus fractures are rare and typically an outcome of high-energy trauma. The coexistence of humeral head avascular necrosis (AVN) and high radial nerve palsy in a delayed presentation makes management particularly challenging and is rarely reported in the literature. This case is important as it highlights a unique combination of bony and neural pathology requiring a carefully staged reconstructive approach. CASE REPORT: A 39-year-old Indian man arrived 4 months after a road traffic accident with left shoulder and arm discomfort, deformit

Masquelet Technique and Proximal Tibial Autograft Utilizing Avitus® Bone Harvester for Severely Comminuted Open Distal Radius Fracture with Extensive Bone Loss: A Case Report.

INTRODUCTION: Distal radius fractures are one of the most common fractures in the United States. Treatment usually involves internal fixation using a volar Henry approach with placement of a volar locking plate. Optimal treatment becomes less apparent when significant bone loss occurs. No case of an open distal radius fracture treated using a staged Masquelet technique involving proximal tibial autograft is available in the literature. Herein, we describe and discuss a case report of a novel technique to treat a large (5 cm) bone defect for an open distal radius fracture. CASE REPORT: A 59-ye

The comparison of Nice knots and traditional methods as an auxiliary reduction-fixation technique in pre-contoured locking plate fixation for comminuted Robinson type 2B clavicle fracture: A retrospective study.

Open reduction and pre-contoured locking plate fixation is a popular treatment option for displaced midshaft clavicle fracture. Lag screw and cerclage are 2 main intraoperative techniques to reduce and fix fragments. However, both lag screw and metallic cerclage have disadvantages. The doubled-suture Nice knot has been reported in many areas of orthopedic surgery for its effectiveness. This study aims to compare the outcomes of comminuted mid-shaft clavicle fractures reduced by Nice knots vs traditional techniques (lag screw or/and metallic cerclage) when bridged with pre-contoured locking pla

Novel exploration of 3D printed personalized total elbow arthroplasty to solve the severe bone defect after internal fixation failure of comminuted distal humerus fracture: A case report.

RATIONALE: Severe bone defect could often occur after removing the fractured fixation plates of comminuted fracture in the distal humerus. The reoperation of internal fixation or conventional total elbow arthroplasty could hardly restore the anatomy structure and function of the elbow. However, a novel exploration of 3-dimensional (3D) printed personalized elbow prosthesis was presented in this work. This is a rare and successful treatment for the severe bone defect after removing the fractured fixation plates of comminuted distal humerus fracture. PATIENT CONCERNS: A 60-year-old male patient

Surgical versus non-surgical treatment of intra-articular comminuted distal radius fractures (AO 23-C2/C3) is associated with better patient-reported outcomes: an instrumental variable analysis using a national Swedish cohort.

BACKGROUND: The surgical rate for distal radius fractures is steadily rising despite limited evidence of its benefits over non-surgical treatment. Using a natural experimental approach, we aimed to compare patient-reported outcomes following surgical versus non-surgical treatment of distal radius fractures. METHODS: Registered in the Swedish Fracture Register by 36 Swedish hospitals from 2013 to 2018, we included a cohort of 13,356 fractures on 13,031 patients aged 18 years or older with distal radius fractures Arbeitsgemeinschaft fur Osteosynthesefragen (AO) 23-A2.1-2, A3, and C1-C3. A biomechanical matched-pair comparison of two different locking plates for tibial diaphyseal comminuted fracture: carbon fiber-reinforced poly-ether-ether-ketone (CF-PEEK) versus titanium plates.

BACKGROUND: Several methods have been proposed to reduce plate construct stiffness and promote secondary bone healing. In this study, we explored the stiffness and strength of the new carbon fiber-reinforced poly-ether-ether-ketone (CF 50) plate compared with the titanium alloy plate (Ti6Al4V). METHODS: Titanium and CF-PEEK locking plates were tested in a tibial non-osteoporotic diaphyseal comminuted fracture model to determine construct stiffness in axial compression, torsion, and bending. Subsequently, constructs were loaded until construct failure to determine construct strength. RESULTS:

Intentional valgus alignment correction using metaphyseal comminution as a "natural osteotomy" during dual-plate fixation for AO/OTA 33-C3 distal femoral fractures with medial knee osteoarthritis: a preliminary feasibility series.

OBJECTIVE: The optimal management of AO/OTA 33-C3 distal femoral fractures combined with symptomatic medial compartment knee osteoarthritis remains uncertain. This preliminary feasibility series explored whether metaphyseal comminution could be used as a "natural osteotomy" window to permit intentional valgus alignment correction during dual-plate fixation, while maintaining anatomical articular reconstruction and fracture stability. METHODS: This retrospective preliminary feasibility series included 17 patients with AO/OTA 33-C3 distal femoral fractures complicated by Kellgren-Lawrence grade

Rhytidectomy approach for treatment of zygomatic arch fractures.

Comminuted zygomatic arch fractures are a challenge to manage without complete exposure owing to their instability. The presence of facial nerve branches near the bone often means that these fractures are often treated by closed methods or left untreated. The disadvantages of such comminuted fracture left untreated cannot be overemphasised. Hence, there is a need of a surgical approach for exposure and fixation of such fractures without limitation of facial nerve injury. We propose and report a case of malunited comminuted fracture of zygomatic arch using facelift approach.

Isolated Bilateral Upper Limb Amelia - A Rare Case Report.

INTRODUCTION: Pediatric subtrochanteric femoral fractures are uncommon, constituting only 4-10% of pediatric femoral fractures. These fractures present a significant challenge due to the strong muscular forces often resulting in malalignment, complicating treatment and reduction. This case is particularly noteworthy as it involves a comminuted subtrochanteric fracture, which is even rarer and demands advanced surgical intervention. This is one of the few reported cases of using a proximal humerus locking plate for treating a comminuted subtrochanteric femoral fracture in a pediatric patient.

Additional fixation using a metal plate with bioresorbable screws and wires for robinson type 2B clavicle fracture.

Simple clavicle fractures can achieve satisfactory results through conservative treatment, and the less frequency of nonunion. Non-union or malunion can occur in displaced clavicle fractures or comminuted shaft fractures. Treatment of displaced comminuted clavicle shaft fractures is performed by holding together the free fragments with interfragmentary screws or wires and fixing them to the clavicle with a plate. Therefore, we performed interfragmentary fixation using open reduction and internal fixation with bioresorbable screws (Mg-Ca alloy, Resomet bioresorbable bone screw; U&I Corp.) and b

Subunit principles in midface fractures: the importance of sagittal buttresses, soft-tissue reductions, and sequencing treatment of segmental fractures.

The patterns of midface fractures were related to postoperative computed tomography scans and clinical results to assess the value of ordering fracture assembly in success of treatment methods. A total of 550 midface fractures were studied for their midface components and the presence of fractures in the adjacent frontal bone or mandible. Preoperative and postoperative computed tomography scans were analyzed to generate recommendations regarding exposure and postoperative stability related to fracture pattern and treatment sequence, both within the midface alone and when combined with frontal

Evaluation of Fibular Fracture Type vs Location of Tibial Fixation of Pilon Fractures.

BACKGROUND: Comminuted fibular fractures can occur with pilon fractures as a result of valgus stress. Transverse fibular fractures can occur with varus deformation. No definitive guide for determining the proper location of tibial fixation exists. The purpose of this study was to identify optimal plate location for fixation of pilon fractures based on the orientation of the fibular fracture. METHODS: One hundred two patients with 103 pilon fractures were identified who were definitively treated at our institution from 2004 to 2013. Pilon fractures were classified using the AO/OTA classificati

A biomechanical evaluation of locked plating for distal fibula fractures in an osteoporotic sawbone model.

BACKGROUND: Supination external rotation (SER) injuries are commonly fixed with a one third tubular neutralization plate. This study investigated if a combination locked plate with additional fixation options was biomechanically superior in osteoporotic bone and comminuted fracture models. METHODS: Using an osteoporotic and a comminuted Sawbones model, SER injuries were fixed with a lag screw for simple oblique fibula fractures, and either a one third tubular neutralization plate or a locking plate. Samples were tested in stiffness, peak torque, displacement at failure, and torsion fatigue.
