Rational Use of Analgesic Agents in Elderly Patients: Implications of the Beers Criteria and STOPP/START Algorithms.

INTRODUCTION/OBJECTIVE: Pain affects approximately 30% of community-dwelling older adults and more than half of nursing-home residents but remains undertreated because age-related pharmacokinetic and pharmacodynamic changes render standard analgesic dosing hazardous. This review synthesises the evidence on rational analgesic prescribing in older adults, comparing the American Geriatrics Society 2023 Beers Criteria with STOPP/START version 3. METHODS: In this structured narrative review, PubMed/MEDLINE, Embase and the Cochrane Library were searched (January 2000-May 2026), combining analgesic-

Evaluation of a multiplex genomic point-of-care strategy for rapid support of older people with respiratory infection in a geriatric hospital over a full winter season.

BACKGROUND: In older patients, it is important to rapidly identify those infected by a significant pathogen in order to implement isolation measures and prescribe appropriate specific treatments when available and indicated. The objective of this study was to evaluate a multiplex genomic point-of-care (POC) strategy detecting four viruses, SARS-CoV-2, influenza virus (IV) A and B and respiratory syncytial virus (RSV), in a geriatric setting located 7 km from the central laboratory of the hospital. METHODS: This prospective monocentric study was conducted during the 2022-2023 winter season in

Pygmalion in the nursing home. The effects of caregiver expectations on patient outcomes.

Several characteristics of nursing home care can diminish rather than enhance the clinical status of older residents. In view of evidence from other settings that "interpersonal expectancy effects" can influence outcomes in a variety of relations, we conducted a randomized controlled trial to test the effects of caregiver expectations on the clinical status of nursing home residents. Within 2 weeks of admission, 63 older residents at six nursing homes were given a comprehensive assessment of cognitive, functional, and emotional status. Residents were then randomly assigned to a "high-expectanc

Experiential learning of empathy in a care-ethics lab.

To generate empathy in the care of vulnerable older persons requires care providers to reflect critically on their care practices. Ethics education and training must provide them with tools to accomplish such critical reflection. It must also create a pedagogical context in which good care can be taught and cultivated. The care-ethics lab 'sTimul' originated in 2008 in Flanders with the stimulation of ethical reflection in care providers and care providers in training as its main goal. Also in 2008, sTimul commenced the organization of empathy sessions as an attempt to achieve this goal by sim

Clinical impact of implementing an orthogeriatric nurse in the care pathway for elderly patients with hip fragility fractures.

INTRODUCTION: Fragility hip fractures (HF) in older adults are associated with substantial morbidity, mortality, and functional decline. In Spain, evidence regarding the contribution of orthogeriatric nursing (OGN) roles within multidisciplinary teams remains limited. This study evaluated clinical outcomes associated with the implementation of an orthogeriatric nurse within a hip fracture clinical care pathway. METHODS: A retrospective cohort study was conducted including patients aged ≥ 75 years admitted with fragility hip fracture between 2017-2019 (pre-OGN period) and 2022-2023 (post-OGN p…

A program to reduce fatigue in convalescing elderly adults.

This study is an examination of the influence of a fatigue reduction program on the level of fatigue experienced by elderly individuals convalescing in subacute units after lower extremity injury, surgery, or weakness. Fifty participants with a mean age of 81 were divided into two groups of 25 participants. The experimental group received a fatigue reduction program consisting of planned rest periods after bathing and therapy; high-protein, high-carbohydrate liquid supplement; and a 3-minute back rub at bedtime. Each participant's level of fatigue was tested soon after admission and 1 week lat

[Supporting neurodegenerative disorders at home: The role and challenges of the gerontology care assistant].

Aging often leads to a decline in cognitive abilities and independence, making it difficult to remain at home and increasing isolation. Gerontology care assistants, trained and integrated into specialized Alzheimer's teams, offer personalized support combining cognitive stimulation, relational support, and motor assistance. This multidisciplinary approach ensures coordinated care, preventing loss of independence and improving the quality of life for patients and their loved ones.

[Nutritional Care of Older Adult Residents Living in Long-Term Care Facilities].

An estimated one-fifth to one half (17.5%-50.5%) of the older adult residents living in long-term care facilities are either malnourished or at risk of malnourishment. Malnutrition in older adults is related to frailty, sarcopenia, anemia, falls, morbidity, and mortality. Monitoring nutritional status and instituting appropriate nutritional care plans in long-term care facilities can help prevent the development of disabilities in older adult residents and reduce long-term healthcare expenditures. In this article, after introducing the primary diagnostic tests for malnutrition, the three main

Incidence and duration of urinary catheters in hospitalized older adults: before and after implementing a geriatric protocol.

This study examined the incidence and duration of urinary catheters in acute care older adults before and after the implementation of a protocol developed to make clinicians aware of the appropriate use of catheters and the parameters for catheter removal. A total of 187 patients (99 pre-intervention, 88 post-intervention) age 65 and older admitted to a community hospital were assessed for the insertion of an indwelling urinary catheter using retrospective record review. A significant reduction was found in the incidence of indwelling urinary catheters in the post-intervention sample (from 33%

Human sexuality in long-term care.

Sexual expression is a basic human need, a normal part of life that is integral to who we are as human beings. As we age, we continue to be challenged to grow in every area of our lives, including our sexuality. In institutionalized settings, however, human sexuality can be an area in which growth is not fostered, but restrained. Challenges leading to restraint include limited privacy, concerns about the consent of cognitively impaired sexual partners and conflicts with the personal values of institutional staff. Challenges exist in many areas, including inappropriate sexual expression toward

Technology to promote safe mobility in the elderly.

New technologies designed to help prevent adverse events related to the mobility of geriatric patients (ie, patient falls, bed-rail entrapment, patient handling, and wandering) are described. Technology offers the potential to eliminate or mitigate preventable adverse events that interfere with treatment, delay rehabilitation, potentiate impairment, and compromise patient safety. Unchecked, these adverse events can have a negative impact on patient health, functional status, and quality of life. It is not surprising that the elderly constitute the population at highest risk for adverse events,

Oral health and healthy ageing: a scoping review.

BACKGROUND: Good oral health is an important part of healthy ageing, yet there is limited understanding regarding the status of oral health care for older people globally. This study reviewed evidence (policies, programs, and interventions) regarding oral health care for older people. METHODS: A systematic search of six databases for published and grey literature in the English language by the end of April 2022 was undertaken utilising Arksey and O'Malley's scoping review framework. RESULTS: The findings from oral health policy documents (n = 17) indicated a lack of priorities in national he

Severe Impairment Rating Scale: A Useful and Brief Cognitive Assessment Tool for Advanced Dementia for Nursing Home Residents.

To investigate the utility of the Severe Impairment Rating Scale (SIRS) as a cognitive assessment tool among nursing home residents with advanced dementia, we conducted a cross-sectional study of 96 residents in 3 nursing homes with Functional Assessment Staging Test (FAST) stage 6a and above. We compared the discriminatory ability of SIRS with the Chinese version of Mini-Mental State Examination, Abbreviated Mental Test, and Clock Drawing Test. Among the cognitive tests, SIRS showed the least "floor" effect and had the best capacity to distinguish very severe (FAST stages 7d-f) dementia (area

[The health of the elderly that care for elders].

Home care for the elderly has a great ally in the family caregiver, in general a spouse. However, these spouses frequently have health complaints themselves. To understand the meaning of health for elderly spouses who are caregivers, to know the changes in their health after taking this role and to learn how they take care of their own health, the authors developed an exploratory qualitative study in which elderly spouses caregivers were personally interviewed. The speeches were analyzed according to the Discourse Analysis on Collective Subject. The result is that they understood health as hav

Evaluation of the tolerance of ceftriaxone by subcutaneous route in patients ≥75 years old in geriatric departments: a prospective observational study.

OBJECTIVES: The subcutaneous (SC) route provides an alternative to the IV or oral route for drug administration in the elderly. The benefits of SC administration have been proven for hydration but are still debated for antibiotics because tolerance remains uncertain, especially in the frail geriatric population. Here, we aimed to improve current knowledge concerning the tolerance profile of ceftriaxone SC administration at both the systemic and cutaneous level, as well as in terms of pain. PATIENTS AND METHODS: This was a prospective descriptive study of SC ceftriaxone tolerance in a geriatri

Detecting depression in Alzheimer's disease: evaluation of four different scales.

Depression is a frequent condition in Alzheimer's disease (AD). The prevalence of depressive symptoms depends on the severity of dementia and the instruments used. Our aim was to assess the prevalence of depression dependent on the severity of dementia by four different scales: The 15-point Geriatric Depression Scale (GDS), the Montgomery and Asperg Depression Scale (MADRS), the Cornell Scale for Depression in Dementia (CSDD) and the Nurses Observation Scale for Geriatric Patients (NOSGER). The study population consisted of 316 patients with Alzheimer's disease from a psychiatric out-patients
