Tracheostomy in free-flap reconstruction of the oral cavity: can it be avoided? A cohort study of 187 patients.

BACKGROUND: Head and neck surgeons are moving away from routine tracheostomy in free-flap reconstruction. We reviewed prophylactic tracheostomy use in patients undergoing oral cavity or oropharynx free-flap reconstruction to identify patient groups who avoided tracheostomy. Secondary aims were to describe complications associated with and without tracheostomy. METHODS: A retrospective cohort study was undertaken, using a prospectively maintained database. Inclusion criteria was free-flap reconstruction for an oral cavity or oropharyngeal defect, excluding partial or total laryngectomy. Variab

Anaplastic transition within the cancer microenvironment in early-stage oral tongue squamous cell carcinoma is associated with local recurrence.

The cancer microenvironment (CME) promotes malignant progression of cancer cells by stimulating cell growth, migration and invasion. Cancer-associated fibroblasts (CAFs), prominent features of the CME, interact directly with cancer cells and facilitate epithelial-mesenchymal transition (EMT). The present study examined the spatial distribution of CAFs and EMT on cancer cells in patients with early-stage tongue squamous cell carcinoma (TSCC) and their association with local recurrence. The present study included 14 patients with early-stage TSCC who had undergone glossectomy between 2006 and 20

Systematic review of site of obstruction identification and non-CPAP treatment options for children with persistent pediatric obstructive sleep apnea.

OBJECTIVES/HYPOTHESIS: Although adenotonsillectomy is accepted as a first-line therapy for pediatric obstructive sleep apnea (OSA), there is currently no consensus regarding optimal methods for identifying the sites of obstruction or treatment of children with persistent disease after surgery. With this in mind, our aim was to systematically review the English-language literature pertaining to these issues. STUDY DESIGN: Systematic review. METHODS: We searched all indexed years of Pubmed, Cochrane CENTRAL, DynaMed, UpToDate, CINAHL, and Scopus for English-language articles containing origina

[Cervical cystic lymphangioma: still a challenge].

INTRODUCTION: Cervical lymphangiomas can not only cause severe sequelae (aesthetic, phonatory or deglutory) but can also be life thereatening due to airway compresion or massive bleeding. This paper analyzes our surgical results, the value of prenatal diagnosis and the use of new techniques such as the EXIT procedure for airway control in sereve cases. PATIENTS AND METHODS: We retrospectively reviewed the medical record of patients with cervical lymphangiomas treated in our center between 1986 and 2009, according to our Clinical Documentation Database. Data referred to prenatal diagnosis, int

External beam radiation followed by planned neck dissection and brachytherapy for base of tongue squamous cell carcinoma.

BACKGROUND: Surgical resection of tongue base cancer can leave the patient with significant functional deficits. Other therapies, such as external beam radiation followed by neck dissection and radiation implants, have shown equal tumor control with good functional outcome. METHODS: Between March 1991 and July 1999, 12 patients at Oregon Health Sciences University, the Portland Veterans Administration Medical Center and West Virginia University School of Medicine Hospital were treated with external beam radiation followed by neck dissection and Ir192 implants. Two patients had T1 disease, two

An Oral Cavity Mass in a 5-Year-Old Girl.

BACKGROUND: Lymphatic malformations are rare congenital vascular anomalies that most commonly affect the head and neck regions. Cervicofacial lesions extending into the oral cavity or upper airway can result in dysphagia, bleeding, malnutrition, and airway obstruction. Emergency assessment is particularly challenging because significant anatomic distortion can exist even when physiologic parameters remain stable, necessitating thorough airway evaluation and early anticipation of potential clinical deterioration. CASE REPORT: A 5-year-old girl who had recently emigrated from Peru presented wit

Virtual reality-guided simulation of brachytherapy: Two case reports.

PURPOSE: Brachytherapy requires precise applicator placement, especially in complex cases, where standard imaging may not provide sufficient guidance. Virtual reality (VR) technology has emerged as a valuable tool for pre-simulation, enabling improved anatomical understanding and procedural planning. We reported two cases, in which VR pre-simulation was used to enhance brachytherapy planning in challenging anatomical regions. CASES DESCRIPTION: Case 1: A 51-year-old woman with carcinoma of unknown primary and poorly differentiated adenocarcinoma had previously undergone chemotherapy and radia

Gaps in evidence: Management of pediatric obstructive sleep apnea without tonsillar hypertrophy.

OBJECTIVES/HYPOTHESIS: Persistent obstructive sleep apnea (OSA) is demonstrated in 40% of children after adenotonsillectomy. We previously evaluated the basis of management decisions in children with OSA without tonsillar hypertrophy and found that 61% of decisions were non-evidence based. The aim of this study was to identify gaps in evidence for the management of children with OSA without tonsillar hypertrophy. STUDY DESIGN: Case series. METHODS: We recorded all real-time decisions made by pediatric subspecialists from eight disciplines that participated in an upper airway clinic and manag

Guided tongue reconstruction: Feasibility analysis of additively manufactured surgical toolkit on a silicone tongue model.

OBJECTIVE: Virtual Surgical Planning (VSP) for tongue reconstruction promises patient-specific optimizations, but its utility is limited by the difficulty of translating a digital plan to a deformable organ. This study evaluated the feasibility and anatomical accuracy of a patient-specific, additively manufactured surgical toolkit through benchtop experiments on soft-tissue phantoms. METHOD: Four distinct hemiglossectomy reconstruction cases were planned using VSP and executed on patient-specific silicone tongue phantoms. The guided workflow, performed by an independent surgeon, incorporated:

Incidental diagnosis of Mounier-Kuhn syndrome during anesthesia: A case report.

RATIONALE: Patients with Mounier-Kuhn syndrome (MKS), a rare disorder characterized by tracheobronchomegaly (<0.3% prevalence), develop pathological tracheobronchial dilation secondary to elastic fiber atrophy. MKS predisposes patients to recurrent infections and presents significant airway challenges during general anesthesia. Early radiological recognition is critical to prevent perioperative crises. This case report aims to highlight the critical importance of radiologically identifying MKS preoperatively to avoid potentially life-threatening perioperative airway complications-an aspect tha

Schwannoma (neurilemmoma) of the tongue.

CONCLUSION: Schwannoma of the tongue always presents as a painless mass on the lingual surface and exhibits no gender preference. A schwannoma on the base of tongue is usually larger at presentation than one on the tongue itself, since it is asymptomatic initially. Complete resection is achievable and recurrence is uncommon. OBJECTIVES: To describe the patient demographics, presenting signs and symptoms, tumor location, imaging studies, management, and therapeutic outcomes of schwannoma of the tongue, an uncommon tumor of neuroectodermal origin that may arise from any myelinated nerve fiber.

[Surgical ENT therapy of oropharyngeal amyloidosis].

BACKGROUND: The term "amyloidosis" refers to a disturbance of metabolism with a pathological deposit of extracellular protein. There are two types of amyloidosis, both of which can have oropharyngeal manifestations. The amyloid deposits cause dysfunction of affected tissues and could result in a life-threatening obstruction of neighbouring organs or the upper aerodigestive tract. CASE REPORT: The present paper reports about the symptoms, diagnosis, and surgical treatment of oropharyngeal amyloidosis. The possible life-threatening obstruction of the upper aerodigestive tract by amyloidosis is

[Perioperative prophylaxis with cefmenoxime and metronidazole in patients with head and neck tumors].

Twenty-one patients with carcinoma of the larynx, hypopharynx and the tongue underwent surgery under perioperative prophylaxis with cefmenoxime and metronidazole. Healing was uneventful and the clinical and the chemical parameters remained normally. The level of the antibiotics was effective against most of the bacteria cultured from tracheal aspirate and the posterior wall of the oral cavity. Effective perioperative antibiotic prophylaxis is necessary in head and neck surgery: the use of simple antibiotics in short courses should be assessed.

Sepsis of the cheek over a subcutaneous forehead flap.

In two cases, a temporal flap was folded medially and passed through a subcutaneous tunnel superficial to the parotid gland. In both cases sepsis developed at the point at which the flap entered the mouth. In one case a spontaneous perforation occurred and the other required drainage. If one elects to fold the flap medially and pass it superficial to the zygomatic arch, the route described by Lewis and Remensnyder, i.e., deep to the masseter, is preferable.

Quality of life and functional outcomes in tongue cancer patients: a long-term, prospective, comparative study.

BACKGROUND: The tongue has an indispensable role in communication, swallowing and breathing. Tongue cancer treatment involves direct resection of the tumor and surrounding tissue, which can limit many essential functions of the tongue. There are few patient-reported quality of life studies involving tongue cancer exclusively. There is also a lack of data on the outcomes of quality of life regarding different reconstructive methods, adjuvant non-surgical therapies and other predicting factors. Our objective is to assess the quality of life, functional status, and predicting factors in patients

Anatomic-histologic study of the floor of the mouth: the lingual lymph nodes.

OBJECTIVE: The lingual lymph nodes are inconstant nodes located within the fascial/intermuscular spaces of the floor of the mouth. Oral tongue squamous cell carcinoma has been reported to recur and metastasize in lingual lymph nodes with poor prognosis. Lingual lymph nodes are not currently included in basic tongue squamous cell carcinoma surgery. METHODS: Twenty-one cadavers (7 males, 14 females) were studied, aged from 57 to 94 years (mean age 76.3 years). The gross specimen of the floor of the mouth was divided into blocks: A (median nodes), B, B' (parahyoid), C, C' (paraglandular). Serial
