Varying physiologic ventricular resynchronization with changes in atrial rhythm in a patient with a right-sided accessory pathway and right bundle branch block.

We describe varying physiologic ventricular resynchronization owing to differences in atrial rhythm in a patient with the right-sided accessory pathway and pre-existing right bundle branch block.

The role of the size of infarcted area on two kinds of vulnerable window in two dimension ventricular tissue.

AIMS: Sudden cardiac death (SCD) is mainly induced by ventricular arrhythmia, especially among patients with myocardial infarction (MI). Previous studies have shown that ventricular tachycardia and fibrillation are thought to be caused by re-entrant waves of excitation. Although in heterogeneous tissue, the traditional vulnerable window of unidirectional block and the vulnerable window when bidirectional propagation was initiated could coexist, little studies are based on effects of infarcted areas on both vulnerable windows. METHODS AND RESULTS: The electrophysiology remodeling was based on

Source-Sink Mismatch Causing Functional Conduction Block in Re-Entrant Ventricular Tachycardia.

Ventricular tachycardia (VT) caused by a re-entrant circuit is a life-threatening arrhythmia that at present cannot always be treated adequately. A realistic model of re-entry would be helpful to accurately guide catheter ablation for interruption of the circuit. In this review, models of electrical activation wavefront propagation during onset and maintenance of re-entrant VT are discussed. In particular, the relationship between activation mapping and maps of transition in infarct border zone thickness, which results in source-sink mismatch, is considered in detail and supplemented with addi

Electrocardiographic patterns of left bundle-branch block caused by intraventricular conduction impairment in working myocardium: a model study.

UNLABELLED: By definition, the electrocardiographic (ECG) patterns of left bundle-branch block (LBBB) represent distinctive changes in duration and shape of the QRS complex caused by intraventricular conduction delay in the left ventricle (LV) due to structural abnormalities in the His-Purkinje conduction system and/or ventricular myocardium. However, impaired conduction in the working myocardium is not taken into consideration in the practical ECG diagnosis. Because the degree of LV myocardium impairment could be of importance for clinical evaluation of patients, we studied the effects of blo

Axis deviation without left bundle branch block.

It has been rarely reported changing axis deviation in the presence of left bundle branch block also during atrial fibrillation and with acute myocardial infarction too. It has also been rarely reported changing axis deviation with changing bundle branch block with onset of atrial fibrillation during acute myocardial infarction. We present a case of axis deviation without left bundle branch block and without atrial fibrillation and acute myocardial infarction in a 65-year-old Italian man. To our knowledge, this is the first report of axis deviation without left bundle branch block and without

Benefits, unresolved questions, and technical issues of cardiac resynchronization therapy for heart failure.

This review aims to provide a synthesis of the published evidence regarding the rationale and clinical benefits of cardiac resynchronization therapy (CRT) with implantable atrial-synchronized biventricular pacing (BVP) devices in patients with moderate to advanced heart failure and intra- and interventricular conduction delays. In addition, it addresses clinical and technical issues that have yet to be resolved, such as the selection of the most suitable candidates for CRT; the usefulness of combining BVP with automatic defibrillation backup; the value of CRT in patients with atrial fibrillati

[Type I and II heart block].

Second degree blocks are divided into type I blocks, with an increment in conduction time for two consecutive conducted impulses, and type II blocks which present with constant conduction times for consecutive conducted impulses. This distinction is not only of theoretical interest but also of practical interest, since type I blocks located in the node are benign as a rule, whereas type I blocks in other locations and type II blocks are of poor natural prognosis and require pacing. Major increments of the PR interval, occurrence at night and young age are in favour of type I blocks. 2/1 blocks

Hemorrhage in cardiac conduction tissue in premature infants.

Six premature infants had hemorrhages that were limited to cardiac conduction tissues. Obstetric complications occurred in each; four were growth retarded, and four had perinatal asphyxia. Two babies survived beyond one week. Four survived less than 24 hours. Bradyrhythmias occurred in four. Extracardiac lesions included infections and renal failure in two infants, hyaline membrane disease in one, hypoplastic lungs in two (one with renal agenesis and placental amnion nodosum), and placental villous edema in two. Cardiac hemorrhage occurred in sinoatrial nodes (n = 1), atrioventricular nodes (n

Cardiac conduction defects in polymyositis: electrophysiologic studies in four patients.

The clinical electrophysiologic findings in four patients with biopsy-proven polymyositis and bifascicular block are described. Atrioventricular (AV) block developed in two of these four patients, and intracardiac electrophysiologic studies documented the site of spontaneous block to be distal to the His bundle in one, whereas in the second marked His-ventricular prolongation was found, and distal block developed at low paced atrial rates. Sinus node and AV nodal function were normal in all four. These electrophysiologic data provide the first documentation of the site of AV block in patients
