The MitraClip procedure is an emerging endovascular technique for treating mitral regurgitation and an attractive alternative for patients who are at high risk for open heart mitral valve repair or replacement. We present the case of a failed redo MitraClip procedure that led to acute right ventricular failure in a patient with homozygous familial hypercholesterolemia and a preexisting secundum atrial septal defect. We highlight the sequelae of the failed redo MitraClip procedure and the anesthetic challenges associated with the emergent redo sternotomy and cardiopulmonary bypass procedure req

Many published reports have described pericardial effusions in patients with secundum atrial septal defects. This study investigated their associated prevalence echocardiographically using 40 patients with isolated ventricular septal defects as controls in an adult patient population. Pericardial effusions accompanied atrial septal defects in 13 patients and ventricular septal defects in only 3 patients (p <0.0001). In conclusion, pericardial effusions are much more common (p <0.005) with uncomplicated secundum atrial septal defects than with uncomplicated ventricular septal defects.

Echocardiography of a dog with a cardiac murmur revealed an ostium primum septal defect, a ventricular septal defect, and mitral valve malformation with regurgitation. The mitral valve and tricuspid valve were separated and displaced at the same level as the ventricular septum. The mitral valve had a cleft in the septal cusp. Cardiac catheterization and angiocardiography showed a left-to-right shunt and a "goose-neck sign," which indicated an elongated left ventricular outflow tract. The diagnosis of a partial atrioventricular septal defect with ventricular septal defect was made. Surgical cor

Complete atrioventricular septal defect generally presents early in life with symptoms of congestive heart failure. Untreated, patients almost universally develop irreversible pulmonary hypertension. We present an unusual case of a Down's syndrome patient whose septal defect first presented at 20 years of age.

Surgical treatment of various septal defects has been long established. With the advances in transcatheter therapy dilatation techniques for valvular stenosis and vascular obstruction have become established procedures. Closure of septal defects in the catheterization laboratory has also been introduced; some of these have come into regular use in current practice. In 1967, Porstmann et al reported the use of Ivalon plug to close patent ductus arteriosus (PDA). Since then, several devices have been used including Rashkind PDA ocluder (not being used now), Gianturco coils, detachable coils (for

The purpose of this study was to determine the pharmacokinetics of lidocaine in children with congenital heart disease (CHD). Fifteen children with left to right intracardiac shunting of blood (acyanotic group) and 15 children with right to left intracardiac shunting of blood (cyanotic group) were studied and compared with 15 children without CHD (control group). Lidocaine (1.5 mg.kg-1) was injected into a peripheral vein over 30 sec and serial samples of arterial blood were obtained up to 120 min after completion of the infusion. Total and free lidocaine were analyzed by enzyme immunoassay. T

Creation of a communication between the left and right atrium to decompress the left ventricle can be life-saving after corrective surgery in some patients with congenital heart disease and small left ventricle. After adaptation of the left ventricle, surgical closure of this anastomosis becomes mandatory. We report four patients (2.9 to 8.2 kg) where non-surgical, transcatheter-closure was performed in the first year of life using the Rashkind-PDA-Occluder System (USCI). In a newborn with d-transposition of the great arteries (2.9 kg), in a patient with atrial septal defect (5.1 kg), and in a

OBJECTIVE: The purpose of this study was to examine associations between prepregnancy body mass index (BMI) and congenital heart defects (CHDs).
STUDY DESIGN: These analyses included case infants with CHDs (n = 6440) and liveborn control infants without birth defects (n = 5673) enrolled in the National Birth Defects Prevention Study (1997-2004).
RESULTS: Adjusted odds ratios for all CHDs combined were 1.16 (95% confidence interval [CI], 1.05-1.29), 1.15 (95% CI, 1.00-1.32), and 1.31 (95% CI, 1.11-1.56) for overweight status, moderate obesity, and severe obesity, respectively. Phenotypes asso

AIMS: Congenital heart disease is among the most common congenital anomalies in pediatrics. The aim of this study was to evaluate the prevalence of congenital heart disease in children in Khorramabad, Iran.
METHODS: This is a descriptive-cross sectional study where all the children diagnosed with congenital heart disease by echocardiography were enrolled to the Shahid Madani Hospital. Patient information was collected by means of a questionnaire. Of 1600 children who underwent cardiac counseling, 9.75% presented congenital heart disease. These were most prevalent among the children of 0-28 da

Increasingly, the interventional cardiologist is seeking to close interatrial communications by inserting devices by means of catheterisation. So as to optimise these procedures, it is advantageous to have a firm grasp of the anatomy of the normal atrial septal structures, this then providing the basis to understand the morphology of the holes which can exist between the chambers, not all of which are true septal defects. A true septal structure can be removed without exiting from the cavities of the heart. It is the flap valve of the oval fossa, along with the anterior rim of the fossa, which

Atrial septal aneurysm (ASA) is a localised "saccular" deformity which protrudes to the right or the left atrium or on both sides. It is a rare, but well recognised cardiac abnormality. It is usually an incidental finding or may presents as atrial arrhythmias or arterial embolism. Though it is an acyanotic congenital heart disease but it may result in significant right to left shunt and cyanosis. We describe a patient of ASA with atrial septal defect who presented with clubbing and right to left shunt without clinically apparent cyanosis.

The Doppler approaches, accuracy, pitfalls and quantitation of atrial and ventricular shunts are reviewed. For both levels of shunting, Doppler demonstration of flow through the septum confirms the diagnosis and directional flow information may provide physiologic information. Shunt sizes may be measured directly by Doppler, and should be reflected by chamber dimensions and volumes. The flow disturbances present in complex malformations such as Ebstein's malformation, transposition of the great arteries with ventricular septal defect, atrial shunting before and after balloon atrial septostomy,

BACKGROUND: Isolated coronary sinus atrial septal defect (CSASD) is a rare congenital cardiac anomaly, comprising <1% of atrial septal defects. Elderly patients with this anomaly are even more uncommon and sometimes overlooked.
CASE SUMMARY: A 73-year-old man with a history of electrical defibrillation therapy for atrial flutter presented with worsening exertional dyspnoea. Cardiac examination revealed CSASD without persistent left superior vena cava, showing only moderate tricuspid regurgitation. Surgical repair of the defect and regurgitant valve improved symptoms dramatically.
DISCUSSION:

Atrial fibrillation (AF) is a frequent comorbidity in adults with atrial septal defect (ASD), one of the most common congenital heart defects. However, there are currently limited recommendations for the management of AF associated with ASD. This article describes a case using a planned approach of catheter ablation followed by transcatheter device closure and discusses management options.
